Bernstein Medical - Center for Hair Restoration Patient ODQ has brown hair and a donor density of 2.3 hairs/mm2. He had an advanced Norwood Class 3 pattern of hair loss. The patient wanted to lower his hairline and thicken the forelock area. Results after one hair transplant session of 1,950 follicular unit grafts.

Hi David,
Good question. He has been on finasteride and got good regrowth on the back part of his scalp, so it was not necessary to transplant there. All the grafts were placed in the front/mid-scalp so, in a sense, even though he is a Class 6 technically, he was transplanted as if he were a NW 4A. He plans to remain on finasteride.

At 10 days, grafts are permanent, so you may scrub as much as you like. For cosmetic purposes, this is generally a good idea as any residual crusts on the scalp or transplanted hair may be noticeable. Removing the crusts will also decrease any tendency to pick and scratch.
With respect to the other question, if you bang your head and it doesn't break the skin or cause a hematoma (bleeding under the skin), the grafts should be fine. With regard to getting in the ring with Tyson - I wouldn't advise it.

Hi Gillenator,
What you are saying is correct. Perhaps my comment on folliculitis was too concise and thus misinterpreted. I was lumping non-infectious and infectious folliculitis together when I said that incidence of folliculitis was relatively common. The non-infectious, low-grade inflammation of at least some hair follicles is what is very common. The infectious folliculitis – one that can affect growth after a hair transplant and that requires treatment with antibiotics – I agree is relatively rare. For this reason, and to prevent antibiotic resistance, prophylactic antibiotics are now not routinely recommended for hair transplantation.
In addition to the post-op suggestions that patients can use to prevent folliculitis mentioned above, the surgeon can reduce the incidence by keeping implanted grafts slightly elevated. This is a practice currently employed by most hair transplant doctors.
I suspect that the recent increased incidence of folliculitis may, in part, be due to the increased % of procedures performed by FUE (rather than just the increased volume of procedures) where hair fragments may be included in the transplant, inciting inflammation and predisposing to infection. This can be mitigated by using stereo-microscopes to check and trim the extracted grafts. Unfortunately, stereo-microscopic dissection is a skill that is second nature to hair transplant teams that were trained in FUT/strip and now do FUE, but not necessarily a honed skill of practices that only have performed FUE.

Wanted to let you know about the latest news from Dr. Bernstein. Here is a sample of the news release:
Hair transplant pioneer Dr. Robert M. Bernstein and his colleague Dr. William R. Rassman have received a patent on a new method that improves the outcome of Follicular Unit Extraction (FUE) — the type of procedure used in half of all hair transplants performed world-wide. The key invention is the addition of a delay between the creation of recipient sites and the insertion of follicular units into those sites. The delay allows the healing process to commence before grafts are inserted, resulting in increased success of the transplant and an improved outcome.Dr. Bernstein introduced the concept of “pre-making recipient sites” into medical literature in a 2012 publication in Hair Transplant Forum International, has discussed the idea extensively, and presented findings at the 2015 ISHRS Annual Scientific Meeting. However, this is the first time he has patented a hair restoration technique.

Hi Guys,
My surgeon asked me to actively scrub away any remaining hair after 2 weeks, as he felt waiting for more than 3 weeks predisposes to folliculitis.
She said that follicles are well and truly safe after day 10.
Any thoughts ?

Around ten years ago I did a study to specifically answer this question (Bernstein RM, Rassman WR: Graft anchoring in hair transplantation. Dermatol Surg 2006; 32: 198-204.). We found that at ten days post-op grafts were no longer at risk of being dislodged. We therefore tell patients to be very gentle when washing your recipient area within the first 10 days. After that you will be able to wash normally without losing grafts. You can read the study here and watch a video describing the study and its findings here.
Folliculitis, a low-grade inflammation or infection of the hair follicle, is relatively common and may occur up to three months after your transplant. It is typically caused by picking or scratching crusts or scabs, or poor hygiene, but may occur even without any precipitating factors. The best way to prevent folliculitis is frequent postoperative showering of the recipient area and, of course, not picking or scratching your recipient area.

Results after one hair transplant session of 2,290 follicular unit grafts. Note the low part, giving a “lift” to the hair. A second planned session will add hair to the patient’s right side so that the part can be raised. This will enable the patient to part his hair higher and have the effect of decreasing the lift to the hair.

Bernstein Medical - Center for Hair Restoration Patient CKC is a Norwood Class 5 with fine, brown hair and a donor density of 1.8 hairs per mm2 which is slightly below average. CKC is a health care professional who had very definite ideas regarding how his hairline should be planned in his hair replacement procedure. His specific concern was that his temples would show enough recession for his age. Note how balanced and natural his final result looks. Photos taken one year after his second hair transplant showing younger appearance even now that he is older. CKC had 3,770 grafts over the first two sessions..

I know there are countless other threads, as acknowledged in my original post. The point of this thread is to bring them all together and get an answer for all the different shapes rather than, as you say, having countless threads dependent upon the shape of the hair being shed.
It would be good to have one resource where guys could come to and this thread could be the definitive shedding hair and worrying tread.
Any other guys want to share their thoughts?

Bernstein Medical - Center for Hair Restoration Patient EVI is a Norwood Class 6 with medium fine, brown hair and a donor density of 2.5. The patient wore a hairpiece until his hair transplant. The plan for the first session was to connect his persistent forelock to the sides and add coverage to the top of his scalp. We used 2,044 follicular unit grafts. A second hair transplant session will be used to add density in the crown.
