Bernstein Medical - Center for Hair Restoration Patient ZBO was a Norwood Class 5 with straight, white hair and a donor density of 2.0 hairs/mm2. His first procedure consisted of the transplantation of 2,496 follicular unit grafts. ZBO had a second hair transplant procedure of 2,068 grafts for a total of 4,564 grafts over the two procedures. The “after” photos were taken one year following his second procedure.

Hi All,
Wanted to let you know the latest news from Dr. Bernstein.
Dr. Bernstein closed the 2017 ARTAS Users Meeting with a discussion of five advanced techniques in robotic hair transplant procedures that he developed at Bernstein Medical. His presentation covered the benefits of pre-making recipient sites, long-hair FUE, tensioner placement, feathering edges in harvesting, and robotic graft selection. The “Hair Restoration Pearls” presentation included case studies, photographs, and videos demonstrating the techniques to the audience of hair restoration physicians. The two-day affair, held in Coronado, California, was a huge success with over 260 attendees from around the world representing 204 robotic hair restoration practices.

Excellent questions. Patients with significant hair loss will get a fuller look by combing their hair to the side or diagonally back. This is different than a comb-over, where the person drops their part to an unnaturally low position to use non-transplanted hair from the side to cover the top. But grooming post-transplant is a patient preference. When planning a transplant I am always aware of future hair loss and the importance of keeping some reserves to address it. I don't believe in extreme dense packing since it risks poor growth due to vascular compromise. It also risks a widened donor scar in FUT and harvesting outside the permanent zone in FUE. I prefer applying appropriate coverage to the entire area one needs to transplant in the first session, achieve optimum growth, and then add additional hair to the same area to increase density at a future date. This will ensure that the first procedure will have a natural distribution and have the best chance for maximum growth. I am against using very high numbers of grafts just for the sake of it. In performing a hair transplant, I like to keep in mind the saying of the architect Mies Van der Rohe “Less is More.”

Results after 2 hair transplant sessions totaling 3,328 follicular unit grafts. The patient’s appearance of fullness was enhanced by side-to-side weighting in the hair restoration.

Bernstein Medical - Center for Hair Restoration Patient RBL had two hair transplants at Bernstein Medical – Center for Hair Restoration. The first was a transplant of 2,675 follicular unit grafts. The second session, one year later, consisted of the transplantation of 2,031 grafts for a total of 4,706 over the two procedures. Note that the added density provided by the second session enabled RBL to wear his hair shorter. The “after” photos were taken one year after his second procedure.

Thank you. This patient had been on finasteride and minoxidil for two years prior to his procedure and he continued both medications after surgery. In general, patients with a diffuse pattern of thinning (i.e. no completely bald areas) should always begin combined medical therapy for at least a year prior to considering surgery. In many cases, surgery can postponed or avoided completely. Unfortunately, some patients cannot tolerate finasteride or choose not to take it due to concern about potential side effects. Minoxidil, although useful, will generally not significantly alter the progression of hair loss when used alone.

Wanted to share a review of a recent study, the first ever to attempt to measure the effect of a hair transplant on other people's perceptions of the recipient. Here is a brief synopsis:
A recently published study is the first to measure the perceived benefit of hair transplantation on a patient’s age, attractiveness, successfulness, and approachability – key factors that play an important role in workplace and social success. The pilot study, published in theJournal of the American Medical Association Facial Plastic Surgery, found that hair transplant recipients were perceived by others to be 3.6 years younger following their hair restoration surgery. The data indicate that the person’s attractiveness, successfulness, and approachability also showed statistically significant positive changes as a result of hair restoration surgery.You can read the full review article on the Bernstein Medical website.

Bernstein Medical - Center for Hair Restoration Patient HYC had dark brown, slightly wavy hair when he visited us for his first procedure more than a dozen years ago. He had two sessions totaling 3,038 follicular unit grafts. Several years later HYC came in for a third hair transplant of an additional 1,252 grafts for a total of 4,290 grafts over three sessions. The “after” photos were taken 13 years after his first transplant, 12 years after his second session, and one year after his third hair transplant.

Hi Rusty,
This is an excellent question. You are correct, widened scars can be improved in two ways: they can be re-excised to make the scar finer, or hair can be placed into the scar to make it less visible. If the scar has thickened (called a hypertrophic scar), transplanted hair won’t grow in it until it is made thinner. This can be accomplished with injections of cortisone directly into the scar. It often takes a series of 2-3 injections spaced at least a month apart to sufficiently flatten a scar so that it can receive grafts . Once the scar is thinned, hair obtained through FUE may be transplanted into the scar. You can read more about this topic on my blog: What are Options for Fixing Wide Donor Scars? Bernstein Medical - Center for Hair Restoration Patient VTZ is a Norwood Class 7, with salt and pepper, straight, fine hair and a donor density of 2.4. VTZ had two scalp reductions and an unsuccessful hair transplant with plugs many years ago of which most did not grow. VTZ also had significant sun damage with multiple keratosis on his scalp. These were treated prior the hair transplant. The treatment removed the keratosis and also improved the quality of his scalp. Because of the sun damage and scalp reduction scars, a conservative first session was performed using 1,668 grafts. A year later we performed a second hair transplant of 1520 grafts and three years later the patient had a final session of 1100 grafts. The results are shown after three hair restoration sessions.

Treatment and Camouflage of Sun Damage
VTZ presented a challenge in that he had severe actinic (sun) damage on his scalp that had to be treated before a hair transplant could be performed. The sun damage alters the blood supply in the scalp and can cause poor growth, particularly if the grafts are packed too densely. The “Before” photo was taken when the patient first presented to our office and before the scalp was treated for sun damage with 5-flurouracil. The “After” photo was taken after the medical treatment and 11 months after his first hair transplant of 1,668 follicular unit grafts. A conservative number of grafts were planted in the first session to insure that there would be maximum growth in the treated scalp. A second session of 1,520 grafts was performed on 2-16-05. These results will be posted here when the transplant has grown in.
