Dense-packing FUE performed by Dr. Feriduni - 3029 FU in a single day session

Follicular Unit Transplantation performed on a 33 year old Caucasian male with:

 

- NW Class V-Va

- Donor density of 92 FU/cm??

- Hair diameter of ~75 microns

 

Treatment plan:

 

- 1st FUE procedure with 3000 FU to reconstruct the hair line and mid head area

- 2nd FUE procedure with 3000 FU to restore the crown area and increase the overall density

 

Performed procedure (May 3rd, 2009):

 

A Follicular Unit Extraction with 3029 FU in a single day session, '3 step procedure', i.e. an intermittent follicular unit extraction (iFUE), with:

 

* 831 single hair FU

* 1562 double hair FU

* 636 triple hair FU

 

Extraction performed with a 0.9mm FUE punch with Versi handle?„?. No microscopic preparation of the follicular unit grafts;

Parallel incisions (single and double units) were made in custom-sized blades technique (~Cutting Edge blades of 0.7mm ??“ 0.9mm);

Dense packing technique with a density of 50 FU/cm?? in hair line and feather zone (single units), a density of 40 FU/cm?? in density zone (double units) .

 

This patient is definitely an exceptional case as he has an extremely high donor density/yield. In general this procedure would also be performed in a 2 successive days session.

His second FUE procedure is planned in the month of April 2010.

How on earth did you do 3,029 FUE grafts in one day, Dr. Feriduni? I'm not saying your number is inflated, because it really looks like 3,000 grafts were both placed and grew, but that has got to be some sort of record. How long was the session? Why were you able to extract so many from this particular patient?

Dr. Feriduni,

 

I too would like to know how you extracted and transplanted over 3,000 follicular units in one day via FUE. Is this a typo, or since he's an exceptional case, were the follicles extremely easy to extract?

 

These are very impressive results. I also appreciate you presenting photos showing the donor area extractions within the universal safe zone.

To TC17 and Bill:

 

This amount of follicular units is the maximum amount that we could extract up to now in a single day session. We were able to transplant this amount of follicular units in one day thanks to several positive factors :

- The follicular units were very easy to extract, we extracted

more than 600 FU an hour;

- Microscopic preparation was not required;

- It was not a problem for the patient to stay in a lying

posture for many hours. The operation was performed in a

timeframe of 12 hours.

 

We performed an intermittent Follicular Unit Extract or IFUE in 3

steps:

1. Extraction of the FU followed by premade incisions (2500 inc.)

and subsequently the implantation of the FU

2. Extraction of the FU followed by the implantation of the FU

3. Extraction of the FU followed by the incisions and subsequently

the implantation of the FU

 

And we finally added the finishing touch to the hair line using the 'stick and place' technique.

 

To Bill:

I personnally think it is important to stay within the universal safety zone and I always try to do so.

Is the Dr.Feriduni operating by himself or does he have any other assistant surgent during this operation? I am interested in having FUE and I am wondering if Dr.Feriduni will be operating on his own or will a seperate Dr. be operating?

To ted.j : I indeed perform all my operations myself, there is no other doctor operating but me.

It is however team work, I have a team of 6 nurses that assist me on every operation. Four of them have an experience in hair restoration surgery for 8 years and two of them for more than 13 years.

The "stick and place" technique describes the process in which the technicians make a recipient site incision and then immediately place the graft into it. By placing grafts immediately into fresh incisions that have not had time to contract or coagulate, incisions can potentially be smaller relative to the size of the typical graft.

These results are very impressive. Congrats to Dr. Bijan Feriduni for this procedure. I bet the patient is really happy.

 

One question I do have would be, on a FUE session of this size or multiple sessions of FUE. How does scaring look like and does the Doc have any pics of buzzed hair a year out of one of these mega FUE sessions. From the pic of donor scar area, I really didnt see anything? I only ask because I plan to have a FUE procedure this year sometime.

 

This session makes the FUE session of 1500-2000 I want seem like a cake walk.

To nsuede : I have added pictures of this patient with a shaved scalp, taken more than 1 year after his first FUE procedure, so you will be able to observe the micro-scarring.

Highly important to prevent a moth eaten effect is the punchsize, in my opinion the punchsize may never exceed 0.95mm. In this patient 's case I used punches with sizes 0.8mm (to extract the single hair FU), 0.85mm (double hair FU) and 0.9mm (triple hair FU)
