Regular Member Dr. William Lindsey Posted July 6, 2010 Regular Member Share Posted July 6, 2010 This fellow had a 1500 graft strip by a national chain a few years back. He presented with a very wide scar and sparse hair up front. We discussed the pros and cons of scar excision with his hair transplant and we both agreed to go for as much hair as we could get and leave scar repair for the future. Shown are preop and periop pics, a pic he emailed me around 3 weeks out, app 6 weeks showing most has fallen out, and then last week at 8 months. Interestingly, he had promised to come in early for a scar check, given his trouble with the first transplant; and ofcourse, like most guys, he didn't show. But, what prompted him to call was scar problems...and fortunately he was able to see his pre-existing scar, and that we sewed right up to that scar. And his trouble is actually a few ingrown hairs in his old scar. We treated those and I asked him to stop by in a month for a check. Dr. Lindsey McLean VA William H. Senior Member Blake Bloxham Posted July 6, 2010 Senior Member Share Posted July 6, 2010 Dr. Lindsey, Thank you for presenting this case. The results look excellent, and I'm sure the patient is thrilled to move past the first, unfortunate experience. Nicely done. Regular Member Dr. William Lindsey Posted July 20, 2010 Author Regular Member Share Posted July 20, 2010 I posted all of the in-focus pics...suitable for the presentation, so no top down pics. Thanks for the comments. Interestingly, he had a real scar complaint when he came in for the procedure and we spent a fair amount of time reviewing that if he was a "bad scarrer" before, he could be again and he ought to come in for the scar checks I always ask patients to attend, but few actually do. So then when he called at 7 months saying his scar was awful...and I reminded him of his old scar, it was like turning on a light..."oh yeah, I had a scar before" So we treated the scar and he was due back last week for another check/possible injection but didn't show. Hopefully we'll get him in and make sure the old scar trouble is either under control, or we'll excise it. Dr. Lindsey McLean VA William H. Senior Member BigBill1234 Posted July 20, 2010 Senior Member Share Posted July 20, 2010 How do you treat the scar without revising it?
Regular Member Dr. William Lindsey Posted July 21, 2010 Author Regular Member Share Posted July 21, 2010 In his case, its a combination of hypertrophy treated with steroid injections, and ingrown hairs in the old, not new, scar, that can be extracted. Regular Member Dr. William Lindsey Posted September 14, 2010 Author Regular Member Share Posted September 14, 2010 He popped in again for me to recheck his OLD scar, which responded pretty well to one steroid treatment. His hair continues to thicken at these 10 month pics. Dr. Lindsey McLean VA William H. Regular Member Dr. William Lindsey Posted September 22, 2010 Author Regular Member Share Posted September 22, 2010 Thanks He was an ideal candidate. First he had good donor hair. Second, he was really sparse in the recipient area. Third, he followed instructions preop and postop, and fourth, while he'd had a bad experience elsewhere, he was motivated to try a possibly more thought-out plan. And he had success. Regular Member Dr. William Lindsey Posted September 27, 2010 Author Regular Member Share Posted September 27, 2010 Thanks Waveskier. Again I think we did a great job, but the patient was a great candidate and he grew quickly. Had this taken 18 months to achieve...as it does sometimes, he may have been less enthusiastic. Fortunately even in those cases, a picture, even a poorly taken one, is worth way more than a thousand words. Dr. Lindsey McLean VA William H.
