The patient is a white male in his mid 40's with a NW 5A pattern. All grafts were intact follicular units and there was only 1 session. Donor repair was with a running subcutaneous (in the fat) closure with no knots and the surface was closed with a running nylon suture. A trichophytic closure was done. Follow up is at one year.

55 y/o WM with 2048 FU grafts in a single session to the frontal half of the scalp. Follow up at 11 months.

Patient had extensive baldness but reasonably good donor hair.

To David and Spanker, The density at and just behind the hairline was around 40/cm2 with less density centrally. He has already had a 2nd session and will send those pics in later.

42yo caucasian male with frontal half balding was treated with 2,300 FU grafts to the balding area. Follow-up pictures are at 13 months.

45 yr old WM presented with severe NW VI-VII hair loss. Was treated with a combination of finasteride and minoxidil for 1 year, followed by 1 session of 2440 follicular unit grafts.

This patient is a 42 y/o caucasian male who desired a thick moustache. His donor hair was good, so 400 grafts were placed into the moustache region.

40 y/o WM is presented before and 14 months after a single session of follicular unit grafts to the frontal half of the scalp.

46y/o male with one previous mini-graft session was transplanted to the frontal and midscalp. Followup up was at 10 months.

The drug effect is very significant in the posterior 2/3, making up 95% of the effect. You can see that the posterior area has predominantly finer hair, while the frontal zone has more terminal type hair. Most of the frontal zone and nearly all of the hair line was the effect of grafting. The purpose of this posting is to demonstrate that combining medical and surgical treatment is better than either alone. He is one of those patients where the response to treatment was excellent in every aspect. Wish I could get this result on everyone. But it will be critical for him to stay on the meds. As of now he still has plenty of donor area left, but doubt it could cover well all of the areas of his natural genetic baldness. Plan to use his donor only when and where it is needed.

The patient is a 45 y/o WM who first presented as a NW6 in 2003. Considered somewhat of a borderline candidate at the time due to his extensive baldness. Started a combination of finasteride, minoxidil and ketoconazole shampoo, and he started growing hair impressively. He discontinued the medicine and presented again in 2006 with pretty severe loss. Pt was restarted on the medical regimen and was transplanted with 1950 FU grafts, mostly to the frontal zone. Only have 2 pictures of him before the combined treatment.

36 y/o WM with a single session of 1600 grafts in a single session. Grafting was confined to the frontal zone.

70 y/o WM wanted to have facial framework. 2320 FU grafts were tranplanted to the frontal and midscalp. Follow is at 6 months.

James, It is not the only thing you can do at this point. Please consider starting finasteride (1 - 1.25mg) daily along with 5% Rogaine twice daily to your entire scalp. Give it at least a year and conscientious use. I treat patients constantly with this regimen and am often surprised with the results. If you get a reasonable growth (but not enough to be satisfactory), these additional hairs allow the camouflage agents like Toppik, sprays and DermMatch to be very helpful. I don't want to give you false hope as I know it is no sure thing; but I think it is too early to give up yet. If you delay medical treatment very long then any chance of success will be lost. I have seen some amazing results. The only way to find out if you will be one of them is to start IMMEDIATELY. Good luck. But I agree with the other writers. If regrowth is not to be, remember that the primary problem socially is not with your hair but with your self confidence. If you don't respect yourself, then how can you expect women to do otherwise? Forget the ones that have to have a guy with hair. You don't need anyone that shallow anyway. Just take what you do have and build on it. You'll be fine.

James, I would strongly recommend that you see a hair doctor or a dermatologist and start both finasteride 1mg daily and 5% minoxidil twice daily. You may also alternate Nizoral 1% shampoo and Head & Shoulders shampoo. Use these for at least 12-15 months before evaluating their effect. Be sure to use the minoxidil (I like the Rogaine foam) over the entire top of the head and also the temples. The response varies from person to person but when the response is good, it can be quite dramatic. But you won't see anything for at least 3 months. If you decide to do this, start immediately. Every month some weak hairs drop below the line where they cannot be retrieved. Agree that you are not a candidate for hair transplantation. Not even close.

54 year old WM with vertex transplantation of 1800 grafts in a single session.

45y/o WM had 3 sessions of round grafts to the frontal hairline at age 25. His hair has receded and now the grafts are isolated and look unnatural. One session of 2171 follicular unit grafts were placed in the frontal and midscalp.

60 y/o WM with one session of 1960 FU grafts in a single session. Follow-up is at 13 months. He is happy and doesn't want any more at this time- but could use a small 2nd session sometime in the future.

The patient is a 58 y/o WM with primarily vertex loss. He wanted a reasonably thick vertex restoration. Normally I like to wait until after age 45 to restore the vertex but will consider each patient individually. This restoration is thicker that I would want in a younger man for fears that progession, if extensive, would give a nest look.
