I will be more than happy to talk to you personally and will try to answers all your querries to the best of my knowledge. you need to understand one thing, FUE is not really a scar free surgery. If you want to keep your head shave as much as its in the pictures, you might have 3000 to 6000 small tiny rounded scars visible. But yes as soon as you grow a longer or 1/2 a cm long hair you will not see a scar. I sincerely believe so, I have submitted my documents, I was actually looking for a bunglow on either Al Wasif road or Jumera rd, luckly I have got one now and its a very very nice clinic. I am sure my patients would like the ambiance in and around that place. But, it takes time to do things in a right way and right time.

Lets do it like this I will see you, will see the donor scar, will see how much can be taken and then once we have the grafts we will see how and where to place it to maximise your out put and yet do not cross the limits. My point is there are four stages in any ones learning curve 1) What to do 2) How to do 3) when to do The last stage that makes you different from others is "when not to do" what ever we decide would be after indepth consultation, examination and then execution keeping things up the sleve for future

  5. Dear Dakota3 I totally agree with you, but the results are not yet over. He got a third session done at a progressive thining pattern towards the crown. In a years time he would not have a bald area but a progressive thinning from front to back. I know he has a big area of baldness on back, but does that mean we should all say no to those people and deny them the right to have HT. I think if the patient is well versed with the limitation of a hair transplant and provided the donor area of minimum 5000 grafts available then we can provide them with a gentle transformation. In most if not all cases they regain their lost self esteem and confidence. Its all about the lost self esteem and confidence. Yes you can see that he has a bald crown and this can tell about the hair transplant. I am sure you would have seen a ludwigs type of baldness in men. They have the front 1/3 to 2/3 head covered with hair and back is all gone, that is natural. My concept of natural ness is about the way a hair line (transition zone) is created. if i cannot see a single line from front and no harsh looking hair line, i am happy. Type C density means about 50-70 grafts/sq cm. normally its about 90 to 120 depending on the ethnic origin. but at the end of the day its about making the patient happy within the limits of ethical practise. So no lies no big promises and standing by your commitment/words.
  6. there is no magic when doctor is putting the grafts as oppose to well trained tech. Its all about how you make sites which will control the angle of growth and density. Tech just have to know how to handle the graft and all they have to do is put it in a slit that is made by doctor. Its a matter of time and experience. If a tech has 10 years of experience and doctor 4, trust me i would have it done by the tech, you do not need much of the intelligence to put a graft in a slit just care and gentleness. Having said that, if i am doing needle and place as we go, then ofcourse the doctor needs to do all the grafts, because he has to control the angle of exit of the graft, which is paramount for a natural looking hair transplant. I do not think that all the top surgeons in the world do placing themselves. they always have techs to do or one side is done by them and other by tech. Some doctors do placing because they want to cut the cost as well. So, in my experience a good trained tech is as good as a doctor if she is doing exactly what she or he was trained for. I know some disagree with me and I respect that point of view but i do not think just because a docor is doing placing his results are superior then those who do not.

Pakistan is my back bone Dubai is just 3 hours flight so not that far. will be in dubai only for a week or so only, I do about minimum 3 surgeries daily in Pakistan and I cannot see myself leaving this part. But ofcourse for reaching out to some celebrities from around the world i need to be at a place where they are comfortable to come. That clinic is mainly geared for them. unfortunately, the price will go up as the over heads will rise. The over heads in Pakistan is really low, i have tech who are 10 years experienced and 5 to 7 years others. I have 17 tech, and i do about 70 to 90 surgeries a month with them. We have develop a system, I am sure there is a lot of room to improve. I do lesser no of surgeries if i have to do in between the hair surgery. as most of my patients are type V to VII its really no problem to go through the site in no time. its like a fun, i just finished my three surgeries it was 1.30 pm when i done. I started at 10 am. today as i was doing a denser packing than what i usually do, i choose not to have 4 surgeries.
  8. Dear JJ dont be confused by that. The reason some doctors would quote 4000 some 2500 is based on 1) are they telling you for that surgery or total number of grafts. 2) If I can do 2500 grafts with my team, i will quote you 2500 only while others who can do 4000 will tell you 4000. I am a person who do not believe one step surgery. I think if i do 5000 grafts in one session as i have done for a patient, and i do 5000 grafts in two sessions my results would be 30% better then one step. May be I am not as good as other. I believe that hair transplant is 20% science and 80% arts what i do is after doing the ground work in first surgery i place the remaining grafts stretegically based on the hair style you want and how the grafts have grown. In this way i can give a much better illusion than the surgeons who do 5000 in one go. Just like an artist after doing the painting they step back look at the picture as a whole and do some retouching, it is this retouching that can makes some time the whole difference. Second, by doing so i can give an illusion of 20% more density. Mind you as we all charge per graft, so if we do it in one session we save more, because the cost of the disposables is twice in two surgeries as oppose to do in one go, so the money incentive is really not there. The only incentive is better results, at least in my hands. hope i have made you understood my point of view.
  9. Dear Friends I was asked by MTD to express my personal professional views about this case. I think it has been a wonderful results considering you started very early in your life. I can see that you are now touching Type Vish but then we really cant tell the true picture as you are on meds. Keeping that in mind and looking at your picture of crown, I guess you would loose all the top hairs and the donor will go down further by about a cm or so. This can also be predict by looking into your family tree and see how bald were your uncles, father or grand parents, that would be more or less the future of your baldness and only then we can predict the level of baldness you would achieve. once we are happy that we will have enough of donor area for future loss only then we proceed to next point that is The question comes what do you want from the crown? If you want the same density as you have on top then we are seriously looking at another 4000 plus grafts. Reason, the crown is a challenge for doctors is cause it has a birds eye view and hence you need twice as many grafts as you would need for the front. so then the problem comes you probably wont have enough donor to support your demand. on the other hand if you want just enough of hair on the crown that you should not look bald rather thinned out look, then I am sure you would be happy with this surgery. Seond problem, the wide scar, in order to make a good donor scar i need enough mobility that the proposed width of scar that we have marked shoud over lap by 0.5cm. This should bring the best possible donor scar. now ifyou have a density of lets suppose 100 grafts/sq cm, i would need 20 sq cm strip which will bring the number of available natural grafts to about 2000. If you have a wide scar of 0.5 cm, then i would need to take a 20 cm long and 1.5 cm wide donor strip. This is all science and maths. If you have enough mobility to get 1.5 cm wide strip then i guess we should b able to put about 1500 grafts on crown but in a fashion that they should give an illusion of 2500 grafts. This is done by controlling the angle of exit of hair and the curvature of shaft. this is the arts part of the surgery. 1500 grafts for crown is theoretically nothing and would give just 30% more improvemnt than what you already have. the fact you have light brown colour makes the life a bit easier cause the light colour hair tend to blend better than dark colour which tend to stand out on a light colour skin. If you keep on using the meds I am sure you would stay happy, but please do keep in mind that before commiting to crown you have enough donor area fpr front in future. SO this is how I would eal in a case like yours See the donor area, if you have a viable donor area that can donate about 4500 to 5000 grafts, I would be happy to take 2000 out and leave the 3000 for future for the front. I personally woud not commit more than 1500 for the crown till you cross 40 and have no meds for more than a year. i hope i have been able to convey my point of view, though i am not good in explaning things while writing it down so it might not convey the same concept that I have in mind and that can only be done once we are fce to face...... take care
  10. Well do respec all of your comments Today I had a person comming from Germany, a caucasian german in my office for a nose job and a hair transplant. Anyway, I am opening a clinic soon in Dubai, but to be honest, I do almost 3 surgeries daily so not that i am short of patients but cause i want to reach out to the world.
  11. Patients like him are tough decision. As one should knowwhen not to do. But once I can see if the donor area an donte 5000 to 6000 graftsin total and the expectations are to have some hair rather than full dense packing then I do take chances. He is very happy and as one must see its all thinning looks rather than bald looks, so now instead of type VII he looks like type IV vertex, and after third surgery that was maily fo cwn, he will have a look of generalised thinness appearence. Hope this would be appreciated. The man advantge an density that I can achieve with less number of grafts is because, i still judiciously use 3 to 5 hair grafts. This makes same number of hairs but lesser number of grafts and hence better denser look in the end. Of course without looking pluggy or un-natural. Naturalness is what i really care for not density
  12. This mid 40's gentleman with type VII MPB was discouraged from having a hair tranplant surgery. He was referred by his friend, after doing a long consultation i made him realize what can be achieved with a hair transplant. Especially if he had a type C density in donor area He had two surgeries done and last picture is before the third procdure. he is ever so happy with his new, no bald but thinned out appearence
  13. well i was asked by Rajtoor t give my input In last one week I hve done a persn from Maldeves, a bangaladeshi, from Kabul, a doctor from Bahrain, and loads of pakistanis from aroad, today there are two pakistanis from london/manchester and a prson who is settled in Leister UK, but from Malavi> hisbrother hadhis sgery from Dr Feller, he came for FUE, ut his skin was too soft nd capping was way to high so second day we did 2807 grafts and first a we did 537 FUE. Asfar as gurntee is concern its about the results not the life. Having said that, yu are psonaly pickd from the hotel and dropped in the hotel and all th time you are in my clinic with me there as well. So if some thing happens I will be with you (LOL). YES, wheI open clinic in Dubai which is almost iminent the price will be that of Dubai, about 2 to 3 US$ a graft. but i will be accessable to the world. about two weeks back I had a gentlemn fom Spain, A caucasian who runs 5 saloons in Spain. He went happy. so thats what it is
  14. well if the hair is thick then it has an advantage of looking dense with relatively less hair but it can look severly unnatural if they are not taken care of. So its a double edge sword Al I have to say istalk t the doctor and see how he would address your concerns. His way of giving you the solutions would tell you if he has the understanding of hair transplat o not
  15. dear lahore you are absolutely right about dermatologist. Unfortunately, in Pakisan most of the main surgeons who are performing hair transplant surgery tend to advertise themselves as plastic surgeon. It was with that reference that i cautioned people. anyway, most important thing is to see the doctor and review his patients and also see what the net has to say about him. With respect to your surgery, all i can see is a good work done, but to pass the comment on the results is far tooo early. This is just immidiate post operation result. As a surgeon, my concerns would be too low a hair line for type VII MPB. Ideally if you want to do Type VII MPB (i delievered a lecture in ISHRS 2007) you need to go one step at a time. You need to develop the sidehumps fist to make it type VI, then make the top part making it type V, and then type IV and so on. Giving a type II ortype III hair line and leaving the back as type VII does not look very natural in long run. Its just a tip I want to give. Having said that, hair transplant is just one of the surgeries a cosmetic surgeon performs along with so many other surgeries. In US most of the dermatologist have 1 to 2 years of plastic surgery or cosmetic surgery or some surgical back ground. Where as in Pakstan, most if not all the dermatologist do not have any idea of surgery at all. So it becomes an ethical question whether they should be operating at all. Just the safty espect of it

I do not agree with the comment that emperor have made I quote "It is my opinion that you were not a good candidate for surgery, given the large balding area, the apparent tightness of your scalp, and your age." the problem could be in the understanding of limitaion of hair transplant surgrey as well. Unfortunate, has got a generalised thinness of the donor area so he will always be thinner on the grafted area as well. I tell people that if they agree to have about 30% to 50% of aparent density of their grafted area as compar to the donor area then they should get the surgery done. No matter how bald they are, they must understand the concept of hair transplant. Not necessarily they will have a 50 to 60 grafts /sq cm, but they might just want to be a man who is thinning rather than a bald man. 4 months is tooo early to comment. in my experience what ever you see at the end of 4 months its about 15% of the total impact. so wait.........

  18. exactly the point ZAZ he had 6700 grafts in two session and trust me his first surgery which was about 4200 was not really that great either, it was good as still it was 4200 but he needs another 1500 to 2000 to really finish the job. coming back to your case you just had 3000 and your baldness was more than rana's so you need more grafts to have same result. I can assure you that the day you will have anything around 7000 grafts you would look really great but have to give the time. thank you for your support. i am always there for my patients.
  19. well as the web site he is mentioning is in my part of the world i know that though it is front ht and back hair piece but those patients are routinely advertised as strictly ht cases. I do not worry cause the better the results you show the more chances that you will fall in the trap. you cant fool every one all the time. After all if i am the patient and i am not getting the same result as that is shown on your adds i would come back and complain. so in long term it does not work and seriously dent your reputation. lots of doctors in developing countries and also some in develop countries use these gimmics but what they forget is they have to jump that high if they set a very high target. HONESTY IS THE BEST POLICY after all.

  21. Dear Dr Ashfaq I really tried to refrain from attempting to give my views over your newly formed comments. In my opinion there are 4 stages in the carrier of any person but more so in a carrier of a Doctor. Stage 1) what to do you obviously know what to do and that is hair transplant. Stage 2) how to do, I am sure that buy now you would have learned from some one how to do Stage 3) When to do, I am sure that most of the time we know that this is the time to do a hair transplant of on a MPB patient or an allopecia pt. Stage 4) This is a stage not many professional will ever achieve and that is when not to do Firs of all a 65 year old gentleman with a phillidelphia positice chromosome chronic myeloid leukemia diagnosed in 2006 and has been on treatment till 10th August 2009. its been only a month and we are rushing into the hair transplant surgery. No wonder Bill was surprised on the thread. The answer that a kid should be knowing is leave him alone for time being. Let him spend couple of years in a complete remission phase and then if all fine one can ponder on the idea of hair transplant in that gentle man. CML is not that simple a problem and two months is tooooooooo short a time to even give him a consultation. Money is not every thing.
  22. There is no second guess about the density, simply see your before pictur. Measure from side to side and front to the last graft. Calculate the area (sq cm) then divide the grafts in that area. It will show you roughly how many would fall per sq cm. Then go back, count the roots in say about three areas infront and couple of areas back then take a marker count the roots and you will know a very rough guess. Otherwise take a marker and a friend who has lot of ptients as ask him to count all the roots between the two dense area, again a rough estimate. Now my personal view, usullay that would be a result of about anything aroung 2500plus minus grafts if you spread. Mind you we have roughly 100 grafts /sq cm. The human eye cannot appreciate any change of density till 50% is down and then the water shed line is achieved. The next 10% loss will make a huge impact. If I just put another 2000 to 2500 over there you would be amazed, reason being I spread according to my main plan and that can only be done once the plan is completed. Still I am all out there for you. so do not worry for a sec.
  23. Dear TC YES, the thicker hair follicles are a double edge sword. In order to keep the looks very natural, you have to be very slective in the follicles that you are gonna transplant in the very front. This is where you have to use the angle of curvature that I highlighted in ISHRS back in 2005 and got the best technique award for that in 2006 in San Diego. Also you might have to use some follicles from certain area of scalp which are softer. At the end its all about how you see things and your artistery. But if you know the problem you can solve it, if you do not know the problem you cannot address that at all.
