  1. This is a 40 year old patient who requested restoration of his receding frontal hairline. He also desired a conservative density enhancement in his thinning crown. He underwent a total of 2457 grafts. The frontal region received 2057 grafts and 400 grafts were placed in the crown. Presented is his 2 year postop result.
  2. This 41 year old male requested reinforcement of his receding frontal hairline. A total of 2266 grafts were used to strengthen a relatively narrow zone along his frontal hairline. Presented are 2 year postoperative photos along with an immediate postop graft placement photo.
  3. fortune11 – He does have donor left but the recommendation is to delay any additional surgery until there is a clear need. empirestate – The restoration was performed using strip harvesting. The harvest plan was designed to use old scars so as to maintain the appearance of his donor area. KO – Selective debulking using FUE helped to soften the frontal appearance. Gram – I don’t have photos which show much difference than what is seen here. Thanks to all for your positive comments.
  4. This patient presented with a history of having undergone hairline restoration using outdated plug transplantation. For years he had been dissatisfied with the harsh appearance of his pluggy hairline. His surgeon attempted to refine the hairline with a session of single hair grafts that were placed in front of the plugs, but the patient continued to be bothered by an abrupt hairline and an unnatural pluggy appearance throughout the original graft zone. His hairline repair in my office consisted of approximately 2500 grafts which were placed with the intention of softening the pluggy appearance along the hairline and in the central plug zone.
  5. This is a 60 year old patient who returned for a 5 year postoperative visit. He had previously undergone placement of 3250 grafts to reestablish his frontal hairline and to enhance density within his thinning frontal and crown regions. Multiple views are presented to demonstrate his density gains, including part views through the frontal hairline.
  6. This is a 63 year old professional hairdresser who requested restoration of his balding scalp. He presented to my office with a history of using hair dye to camouflage his graying hair. He underwent a total of 5250 grafts which were performed in two sessions, each one year apart. The initial 3000 graft session provided restoration of his frontal hairline. The second session used 2250 grafts to enhance density in his midscalp and crown. Presented are photos taken three years following his original procedure. He opted to discontinue using hair dye following regrowth of his transplanted hair.
  7. This is a 45 year old male who requested restoration of his thinning crown. He underwent a single session of 2343 grafts. Presented here are his one year postoperative photos.
  8. This 24 year old patient requested frontal hairline restoration. His history included the use of finasteride and minoxidil to assist stabilization of his thinning process. A total of 2320 grafts were extracted from his safe donor region and used to redefine his frontal hairline. Postop photos show his progress at 8 months.
  9. This is a 34 year old patient who requested hairline restoration. He also inquired about filling a left temple defect that resulted from a forceps delivery at the time of his birth. He underwent restoration of his hairline region and the left temple defect using 3063 grafts. Presented here are 8 month postoperative photos. He was fortunate to show strong early growth. Also presented are immediate postoperative and 10 day postoperative graft placement photos.
  10. Plans for hairline shape and location can vary widely depending on age, pattern of loss, donor graft availability and patient expectations. Attempting to achieve an aggressively low (adolescent) hairline can be risky, especially for those with advanced patterns and limited donor reserves. Additionally, a very low hairline may look out-of-place for some men as they age. This is a 49 year old patient who presented for frontal hairline restoration. His goal was to obtain a very low hairline. Following a long consultation he agreed with my recommendation to begin the restorative process with a more conservative higher location, albeit still relatively low for someone his age. He underwent a frontal hairline restoration session of 3353 grafts. He returned one year later with what appeared to be a decent result. Although he was happy with the restoration, he expressed a continued desire to pursue the lower hairline he had requested one year earlier. I agreed to his request and he underwent a secondary session using 1498 densely packed grafts which lowered his hairline approximately one centimeter. The patient returned one year later and expressed satisfaction with the final outcome. He favored a product to “spike” his hair. Presented is his final result of 4851 grafts which shows him both with and without a hair product.
  11. This 46 year old patient requested treatment for his balding crown region. He underwent a session of 3000 grafts to achieve coverage in the balding zone. Graft placement was designed to recreate his natural crown whorl. Presented here are one year postoperative photos.
  12. hsrp10 - Excellent observation regarding the possibility of the patch being related to alopecia areata. However, this was a slowly progressive thinning process which was consistent with an atypical presentation of male pattern baldness. The remainder of his examination confirmed that. Rootz - Spanker was spot on correct with his comment. Not only was the hairline reestablished with dense-packing, the central hollow was filled with higher density grafting and the surrounding areas which showed evidence of miniaturization were reinforced with additional grafts. 1978matt - As you suggested, the graft zone was shaved for this case. He had a very haphazard follicular unit distribution. I felt that this along with the goal for localized dense-packing warranted a shaved approach. Thanks to all for the positive comments.
  13. This is a 26 year old female patient who requested a lower hairline. Her examination revealed stable hair throughout the scalp and excellent donor hair characteristics. She underwent a highly-refined dense-pack procedure in which 2161 grafts were placed along her frontal hairline. Presented here are 17 month postoperative photos along with a 24 hour postoperative graft placement photo.

This is a 37 year old patient who presented with a history of slowly eroding density along his entire frontal hairline and within the central tuft/midscalp region.His goal was to create a natural, high density appearance throughout the entire thinning zone.His restoration consisted of a 3000 graft session using highly refined grafts to dense pack the frontal hairline and central tuft/midscalp regions.Presented are one year postop views.These include a part view directly through the bald central tuft to demonstrate the density gain achieved.
  15. This is a 30 year old patient who requested strengthening of his weak and moderately recessed frontal hairline. A total of 2514 grafts were extracted and utilized to reestablish and define the frontal hairline region. Presented are 8 month and 1 year postop photos.
  16. This 48 year old patient presented to my office with a long history of wearing a hair piece. Years earlier he chose to use a hair piece so as to maintain a low hairline with high density and full scalp coverage. He wanted to continue using his hair piece, but was concerned about the appearance of his frontal hairline. He requested that the frontal hairline be restored in a way that would allow him to use his hair piece to maintain high density, full scalp coverage behind the frontal region. The patient wanted to maintain a discrete appearance during the recovery phase. He was asked to grow his frontal hair in the proposed region of the hairline restoration and use the hair piece to maintain scalp coverage behind this zone. A total of 2,689 grafts were placed in the frontal hairline region using high-density grafting and a no-shave technique. The back edge of the graft zone was designed with a sharp edge to allow meticulous abutment of the hair piece to the graft zone. The plan was developed with the possibility of him continuing hair restoration behind the newly grafted hairline at some future date. Future grafting would allow him the option of using a smaller hair piece over the crown or possibly dispensing with the hair piece entirely. Presented here are one year postoperative photos. Comb-back views and hair-part views demonstrate the density match between the graft zone and his current hair piece. The hair-part views also show the tight integration between his hair piece and the grafted region. He was satisfied with the frontal restoration and expressed a desire to continue moving the graft zone back.
  17. This 53 year patient requested frontal hairline restoration. He was particularly bothered by recession along the entire length of his frontal hairline and by diffuse thinning within his frontal tuft. His primary goals were to reestablish a new hairline and to enhance overall density in the frontal region. A total of 3559 grafts were used for the hairline restoration. Presented are one year postoperative photos. Also included are immediate and short-term postop views.
  18. No need to worry, you have some bruising that has migrated down into your eyelid region. It is not uncommon for fluids to move downward away from the surgical site and finally settle in the upper eyelid. In your case there is a bit of blood that have moved into the discolored region. I would expect the swelling to resolve over the next few days, but the bruise may take a bit longer to fully clear. The purple color may transition into a yellow-tinge color as the blood is metabolized. You can use a concealer over the area if the discoloration bothers you, but rest assured that this is temporary.
  19. This female patient underwent facelift and browlift cosmetic facial rejuvenation, after which she developed a serious scalp infection in the region of her browlift incision line. The patient was managed by her cosmetic surgeon over a prolonged period during which antibiotics and local wound care were used to clear the infection. She was, however, left with an unsightly large scar in her frontal hairline region. Her examination upon referral to my office revealed a very dense, full-thickness, frontal scalp scar. The scar tissue lacked mobility and was fixed to the underlying bone. The patient was quite distraught over the disfiguration that resulted from her cosmetic facial procedure. She was also extremely concerned that additional surgery could worsen the situation. Various options for repair were discussed with the patient, and it was decided that a staged grafting approach would best suit her personal needs and concerns. A staged repair using two sessions of relatively low-density grafting was recommended because of concerns relating to poor circulation within the large scar zone. The graft zone received a total of 1500 grafts in two sessions that were separated by one year. Presented here are 10 month postop photos. It is interesting to note that the overall color, texture, and mobility of her original scar were significantly improved by the addition of healthy soft tissue into the troubled zone.
  20. This is a 52 year old patient who requested frontal hairline restoration. Graft placement was performed without trimming or shaving the recipient site to accommodate his desire to mask evidence of the restorative process. A total of 2278 grafts were placed into the target zone. Presented are nine month postoperative photos.
  21. This is a 30 year old patient who presented with extensive hair loss that extended from his frontal hairline into the crown region. He underwent full zone restoration with a single session of 4478 grafts. The primary focus of graft placement was directed to density enhancement in the front half of his balding regions. Presented are 10 month postoperative photos that attempt to show a full representation of his restoration.
  22. This is a follow-up on a 49 year old male who initially underwent a frontal restoration with a total of 2622 grafts. Two years later he received another 2250 grafts with distribution focused primarily in the midscalp and crown regions. His final result was accomplished with a total of 4872 grafts.
  23. I recently had the pleasure of seeing one of my patients for his one-year postop check. Most every one of our members and frequenters of this site know Spanker, who holds well-deserved respect for his dedication to the hair loss community. Spanker and I have had several conversations over the last several months which have clearly demonstrated to me his deep passion for educating individuals with hair loss. I have a great admiration for the depth of his knowledge and his commitment to our community. During our meeting last week, we decided to join forces with Spanker taking on the role of an online representative for my practice.   24. This patient initially presented at age 57 with a request to restore his frontal hairline. He was a lifelong performing musician and had hoped to restore a bit of his youthful hairline glory to complement his appearance on stage. He received a total of 3870 grafts for his hairline restoration. I saw him annually for 3 consecutive years and was able to take photos with different hairs styles and hair lengths over that time period. Also presented are some views with the hair parted directly through the graft zone for inspection of the density gain that was obtained. The part views were taken during his 3 year postop check at which time he had just celebrated his 60th birthday.
