"Jotronic" wants to know whether I charge the patient only for the anagen hair?

Let me clarify the point here- in large cases where 4000 grafts are being harvested from the scalp in a single session, we will never get so many anagen hair. From an average donor site I get about 3000 grafts which lie in the anagen phase. Yes, when the requirement is less than 2500 grafts, all my patients pay only for anagen hair. The average FUT session (unlike FUE) is of 2500 grafts!

In response to the second query raised by “ Jobber1900 “- I almost never shave the head in preparation for anagen selective harvest. I prefer visual selection using 6x magnification (as mentioned in my post under reference) since most of my patients are from out of town and if I tell them to shave and come, I would not be able to do a proper consultation and maintain a photographic record. I have been shaving the head in local patients coming in 3 days prior to the procedure occasionally. I feel the latter approach is easier for the doctor since only the robust anagen hair grow out and the procedure becomes faster and easier to accomplish. Given a patient walking in for consultation upto 48 hours before the procedure, I would like to shave the head in preparation for an anagen selective FUE harvest.

I agree that the word "miracle" has been inappropriately used given that the majority of forum members are from the West. In India the term may be loosely used and does not signify much. It is a cultural thing. I request the moderator to kindly modify the header of the present thread to just- "Dr Bhatti- Anagen selective FUE". I would be grateful since I shun controversy.

The perfect mix would be 3;1::scalp:body hair

I have seen that body hair somewhat acquires the straight (linear) growth pattern of scalp hair after 2-3 years. It does look a bit coarse initially.

Yes in these advanced types of MPB, we do FUE but utilize more body hair.

The scalp permanent donor site has around 20,000 grafts available for harvest. Since we should never harvest more than 25-35 percent, we have 5000-6000 grafts available in a lifetime. Since in this patient we have harvested 3000 roughly, he has another 2000 which can be safely harvested without any apparent loss in donor density. However, in FUE the donor is limited only by the doctor's/ client's imagination since the whole body is available for harvest of hair!

As a followup, beard hair can be taken without significant visible scarring in Caucasians. As I said, in Indian brown skin I would never advise it except from the shadow area of the beard.

My client walked in a few days back and I just could not recognize him. I remember him now having come from a considerable distance to meet me. A young man very successful in his field, the hair loss troubled him so much he was preoccupied with it most times and it had started to affect his professional output. Oh, I love hybrid hairlines even in those who have an adequate donor scalp.

This 22 years old girl sustained childhood accidental burns due to which her left temple hairline was broken. She hid this area with hair styling and it considerably psychologically affected her early growing years. Reconstructed by 500 FUE grafts in the first stage. She has now come for adding another 700 grafts for make the hair look better before she graduates from college. The first procedure was done some 18 months back.

If you study the preop pictures, you will realize the patient had considerable parietal recession (area behind the temples) consequent to wearing a clip-on hair piece for over 30 years. This had made his hairline on the sides drop considerably due to traction alopecia. Now, given a person with type 6 MPB with exceedingly low donor density, all hair available are a boon. I never considered the pubic region till the patient suggested this after we had depleted all good donor hair from the body. As long as the hair are softened by scalp hair, it really does not matter what hair you choose. In case you need any further clarifications, I would be much too happy to answer.

I do anagen selection only for scalp hair.

I do not do it for body hair due to lesser number of grafts available.

The above client received around 4000 FUE grafts and we did anagen selection for the frontal hairline only and the crown regions which are most visible. 3000 grafts were anagen selected.

To make it very brief, the most apt definition of Anagen Selective FUE Technique is- " The preferential harvest of the most robust FUs lying in the Anagen Phase"

How do they grow better hair? -

1. Anagen follicles are in Full Bloom

2. Anagen follicles resist Shock Loss

3. Anagen follicles are less fragile to handling

4. Anagen follicles give a denser look

How are anagen hair selected?-

1. Visual selection- the sirgeon uses 6x magnification

2. Mechanical selection- the hair is shaved 36 hours prior to the procedure. Only the anagen follicles will grow out in 36 hours.

In contrast while using the strip technique, you are committed to take all follicles that lie in the ellipse which is cut out. Whether they are in anagen, telogen or catagen phase, you have to pay for all.

In my FUE technique, the client pays for only Anagen FUE grafts.

The question raised by "agenteye" - Why not take more hair from other parts of the beard when the patient is so well endowed with it?

Well, in colored skin, the contrast of scars which are white with the background of brown skin is sometimes so stark that I would never recommend beard hair from the exposed areas for hair transplant in such cases. I shall now soon be posting a Caucasian client who received over 2500 FUE grafts from all areas of the beard and there are no noticeable scars.

In Caucasians the grafts harvest from the beard are in large numbers.

In black and brown skin large beard harvesting is a BOG NO-NO..

Results seen are after 6 months.

The scalp had very low density.

approx 3000 grafts done in a healthy 46 years male. No Finasteride. Only Rogaine, Biotin for past 5 months.

Traction alopecia in Sikh males is a common entity of hair loss in Punjab, India and in countries where Sikhs have migrated to- esp. in California, Vancouver, Toronto, New Zealand and Australia. Sikhism is a distinct Indian religion in which the male wears his hair unshorn and ties it as a top knot. Due to the constant pull on the hair strands, there is immense scarring and roots disappera over time leading to a broken hairline. Since this occurs in adolescent years, the patient is psychologically devastated if he now decides to cut his hair short in keeping with modern trends in Indian Society. Hair Transplant in such cases is a challenge due to a lot of scarring. Here I show a case done 5 months back and his result after just one session of dense packing.

This Caucasian male underwent a single day session for 4050 FUE grafts. All harvesting was done by the doctor himself. The pictures show the preop, perop and postop condition after a span of just 3 months. This early pleasing result is due to the fact that with FUE the doctor can select the follicular units that are in full bloom (anagen phase follicles only). These grow early, since they are resistant to shock, dessication and handling. The results are most pleasing. FUT surgeons even when they decry FUE technique have no logical rebuttal to selective anagen transplant.
