I say this as a somewhat expert on women and cosmetic surgery; being a veteran of more than 7000 female facial plastic procedures over the past 14 years. So when a woman comes to the office for hair I have a different approach than with men. With men, if I take a guy with little hair and give him some, or alot of hair; in general they are happy or elated respectively. If I take a woman and give them anything less than THEY see at the checkout counter on Vogue, or Cosmo; they may think that I am a war criminal. I find women are most satisfied with hair who have a discreet region that can be "fixed"; such as a part line, sideburns which have been removed by the aggressive plastic surgeon down the street, or from some traumatic injury. Diffuse male pattern balding in women is in my opinion, the hardest of hair patients to treat; like a redo redo redo nose job in facial plastics. Much like tae kwon doe, I try to pick the fight that I have a shot at winning. Meaning, I don't like to be the first guy on sparring night to take on Master Beck who is 22 year younger than me. But I am happy to go full contact with almost all of the other "old guys" like me. Same with female hair: I want a discreet battle that I think that I can win by making the lady happy at a year. That means VERY thorough preop consultations on expectations, downtime, and potential future loss elsewhere that may not be fixable to the density they desire. So this lady came in last month. She has some diffuse thinning and I was prepared to not offer her any procedure, but she said what I like to hear: "what really bothers me are these 2 triangles by my widow's peak". At that point I told her I think that I can help her. So we had 2 preop discussions and she had 1700 grafts placed. Pictures are shown. She came back at a month for a scar check, and it looks fine. She was so happy that she opted to get her eyes done at that followup and I report that she gets her eyelid sutures out tomorrow morning; and then we'll see her at New Years; and next summer for pics.
