Both and it all depends on the size of the practice.  Consider a practice that does several cases per day with only one doctor.  So as to save time a doctor may do sites first on one patient while extractions are being done on another case simultaneously.  

Trust is a big component in the overall scheme of things.  Why choose a particular clinic?  Do your research and view photos of results, particularly of cases similar to your own.  

There are a couple of ways you can determine what was harvested.  If FUT, by the size of the segment.  With FUE, did they just harvest a bit from the sides? Or sides and entire back area?  Ask.  Eventually the overall result will also provide a clue.  2000-2500 grafts will typically provide coverage to the front and area behind it, unless the grafts were placed diffusely throughout the entire area.

Why wouldn't they be?

Usually you find the opposite - more punches are made but follicles not extracted and implanted. This is often where grafts are transected and left to heal.

Extracted grafts are only useful on your head and are a finite resource so there's no logical reason for a reputable clinic not to impact them once they've been extracted.

On me the Dr and I agreed to a general plan on which areas to cover. Then he did all the extractions. While he was extracting grafts, the techs were sorting them and checking them to make sure they were good grafts, and counting how many grafts we ended up with. Once the extractions were done and the final number of grafts known, the Dr started making the incisions discussing with me any minor changes to the original plan based on if we got more or less grafts than he estimated at the beginning. The incisions were counted by a tech as the Dr was making them so they could keep track and make the correct number of incisions.

It honestly depends on whether the clinic makes all the incisions before implantation or if they employ the stick and place technique.  If they employ the stick and place technique, a few extractions are made, a few incisions are made and then a few grafts are implanted into those recipient sites.   The process then repeats itself.

If the clinic doesn’t employ stick and place, all the extractions are made, all the incisions are made and then all the follicles are placed.  This is the way we typically perform the procedure though we typically extract a few as a test to make sure there aren’t any issues.
