You'll only know if you see a stream of blood running down your head. The common rule is; no blood = no issue. After 72 hours, it is impossible to dislodge grafts. The no blood = no dislodge rule is true for the vast vast majority of situations. Also, if there is bleeding, it will be alot blooder than you think can come out of 1 tiny hole! In other words, its a dislodged graft is something where you'll know it when you see it.
