Deduct 10% travel discount from the total. Then adds 13% tax on top on that amount. Eg for 1500 grafts. $4 x 1500 = $6000 with 10% travel discount. 0.9 x $6000 = $5400 And then 13% tax on top. $5400 x 1.13 = $6102 Is this correct? And you will need to get this completed by your Dr. They seem very strict about this.
