Dr. Mohebi---2439 FUT grafts on Class III Patient--Post-op: 10 Months Elapsed

On 10/07/2009, Dr. Parsa Mohebi performed a FUT Hair Restoration procedure on a 26 year old male with:

• Hair loss pattern -- Class III

• Hair color -- Blond

• Hair character -- Straight

• Hair thickness – Average

 

Treatment Plan as of 9/17/2009 (date of consultation):

• Continue using Finasteride medication

• Begin scalp exercises

FUT Hair Transplant procedure-- recommended 2000-2300 grafts for the front/hairline areas

 

HAIR TRANSPLANT PROCEDURE PERFORMED:

FUT (strip removal) procedure performed on 10/07/2009.

 

A Follicular Unit Strip Transplantation with 2439 total FU:

• 405 – single hair FUT

• 1236 – double hair FUT

• 798 – triple hair FUT

• 0 – quadruple hair FUT

 

Size of Donor Strip Cut:

• 18 x 1.5cm

Thank you for posting this fantastic restoration. It looks like great coverage for the number of grafts. Do you have any immediately post-op photos showing graft placement?

Dr. Mohebi,

 

This patient was only 26 at the time of his surgery, so why did you lower the hairline so much and bring his temples forward? It seems unnecessarily risky to me.

Good question. We do perform microscopic examination on all our patients to check the level of miniaturization of the hair on different areas of the scalp. This test helps us predict where the patient is heading in terms of future hair loss pattern.

 

This patient has 100 percent miniaturizaiton on the front and corners but not any evidence of significant miniaturization in the other areas (0 to 20%), which is within normal range, so we know he will never be going to the advanced stages of hair loss and can safely restore his hair line.

Dr. Mohebi,

 

I appreciate you responding to my question, but I'm afraid I'm even more confused.

 

You first say that microscopic examination helps predict where the patient is headed in terms of future loss, but then say that based upon this particular patient not exhibiting miniaturization in certain areas, that you know that he won't have an advanced pattern. (emphasis added) Which one is it?

 

How confident are you in predicting where a patient will end up? If a patient is 27 and is showing a NW 3v pattern, but upon miniaturization is showing a NW 5 pattern, are you confident that he will never become a NW 6 or NW 7? What if the microscopic examination says one thing, and family history says another? Are you saying that by age 26 a patient who upon microscopic analysis does not show miniaturization in a specific zone will never lose hair in that zone?

You brought up important points. When scheduling a patient for hair transplant, it is not just enough to know the current pattern and other factors should be considered such as:

 

 

 

  1. Miniaturization study that has been a great help predicting where they are heading.
  2. Family pattern of baldness, it is also important to know whether or not the other member of family had similar pattern at their age and what has been the final pattern of the others.
  3. Compliance with the medical treatment.

 

 

Although miniaturization study is very helpful to determine where a patient is heading, we do not ignore other items when finalizing our surgical plan. The patient that we have presented here has minimal miniaturization except for the front and corner areas, he did not have a significant family history and has been on finasteride even before seeing us and he was willing to continue the medication.

 

We don't expect this patient to progress toward class VI or VII thus we can be more liberal with his hairline design.
