  1. I'm someone who's usually pretty optimistic but honestly this looks really bad. your pretty much destined for nw7, and it looks like the donor is thinning as well. I understand how hard it must be especially at such a young age but have you tried going bald. also the fact that finasteride is not stopping or at least slowing down the loss is not good. there are clinics that can probably get your hair back to a state in which there is decent coverage however you have to ask yourself is it worth the thousands and the fact is you will probably need multiple surgeries as you continue to lose hair. In my opinion going bald is the best thing to do in this case. your case couldve been acceptable if you were a lot older but at 21 its a very different story. have you visited a dermatologist and got everything checked out?

    Hi Mike,
Thanks for your honest opinion. I agree that NW0 is too aggressive but what about this markup picture I've attached below? Could this be done with 1800 to 2000 grafts? This markup just eliminates the widows peak look (M shape) by filling in the temples and creating a straight line. I've moved the hairline back from the previous picture with the blue line. Thanks

there's a reason why there's many people here adamant that you shouldn't get a straight hairline. it looks unnatural and not aesthetically pleasing.

    Hey, thanks for your reply.
I agree that most people I've come across don't have a square hairline although I have come across some guys who do and I've always found that look to be very aesthetic and I've wanted to attain that somehow. If I were to pursue that hairline, how many grafts do you think it would take? I'm guessing 1500 grafts isn't enough to achieve the markup picture that I just posted?

Compared to the previous markup picture (in blue pen) that I posted, I moved my hairline back a little while keeping the square front so I was hoping this could be achieved with 1500 to 2000 grafts but maybe I'm underestimating the usage?

underestimating the amount of grafts and not aesthetically pleasing. the worst hair transplants are the ones where everyone can tell you've had something done because it looks unnatural. also you gotta think of it like this, 10 years from now do you think a completely straight and squared hairline will look natural on a 34 year old?

Hey John, thanks for your reply.

Honestly for me I don't want my hairline lowered in comparison to what I naturally had but I do want to bring in my "side hairline" and to fill in the temples even a little bit while creating a straight line from the front. Like maybe something like this? (image attached). Would this be achievable with say 1500 to 2000 grafts? With regards to my regiment I do use fin of 1mg every other day. For minoxidil I used the foam version for a little bit last year but only for like 2 months which obviously is too short. I want to use oral minoxidil and I'm thinking of switching to or even adding topical fin and maybe dermarolling to my routine. I wish I had started fin earlier but I have been using it on and off for 2.5 years now and I have been much more consistent over the past 6 months. Thanks, I really value your time and help.

this looks extremely unnatural and you can lose hair further back from the transplanted area. get something a bit more conservative with around 1500 grafts where you will fill in the sides but still keep that natural hairline shape. the reason people don't do square hairlines is because it does not look natural. hair transplants are a cosmetic procedure in which they are trying to give you natural results as well as improving how the hair looked from before. Search up machine gun kelly hair transplant.  hes got good results in which his hair looks a lot better but does not have a square hairline. most people wouldnt even know he got something done if they didnt see him before the transplants.

OP has better hair at 35 than I did at 15. Surgery should be last resort, I wouldn't do it in your case.

completely agree. I get that op is trying to strive for perfection but hes got to realise hes still an nw1.5 at worst. hes got a lot of great hair that doesn't need to be changed. hes got a really nice matured hairline that hasnt gone further back.
