From these photos i really cant tell if he has patches on the back of his head or not. This needs to be documented much better. With better lightning on the donor area and with the camera pointing upwards, not downwards to his head at the donor.
Patient sent the photos i asked, including the camera poiting upwards in the occiptal area as you sugested.

You have to show the donor area on the back with proper lightning and with the right angle.

Hello guys, I understand the worries about his donor area. I should indeed have posted more photos of the donor. Here you go, photos with different lighting and taken by the barber, by patient's wife. Btw the patient never complained about his donor even if the previous FUT scar is slightly more visible.
I've also asked him specifically photos of his donor.

actually this is a good quotation 🤓

@Dr. Felipe Pittella would this result still be possible if the patient refused minoxidil, and instead did dutasteride mesotherapy or topical finasteride?
absolutely... same reason before...

Would you say its possible to have a result like this if you're only willing to take topical minoxidil and not oral minoxidil if you're NW7?
absolutely.
I like to tell my patients that:

Most on this forum would have told this person to shave and never consider a transplant. Really goes to show how important the surgeon skill + a conservative hairline is
This is one of the most important comment so far.

I've asked some photos of the beard for feedback on scarring (he just said theres absolutely no scars and the skin feels good quality on the harvested area). It seems that the extraction from face and beard using a small punch may act as a "skin peeling" procedure. All patients refer improvement of skin quality on harvested areas.
soon more updates

One of the Best Norwood 7 results Ive seen. Top 5 for sure. Would be very interested to see his 4th surgery.
Lets go! Finally. We are now 5 months from the 4th procedure.
I'll post the strategy here first. video first and photos below.
STRATEGY
Increase density on these areas and advance little bit the hairline. BHT beard leftovers as donor, plus sideburns and chest and abdominal hair.leftovers from beard (notice no scars from previous procedures)
INTRAOP
Increasing density by transplanting in between previously transplanted grafts
IMEDIATE POSTOP
SECOND DAY POSTOP (notice fast beard recovery)

This is an impressive result. Congrats.
I cannot help but ask: Usually body and bear hair does not look great on scalp, I have seen only very few exceptions. Did you feel comfortable with him doing so because of specific characteristics of his body/bear hair?
He beard hair looks relatively straight.
Thanks
Yes and I do BHT with beard as donor on every high norwood classes (in the PRECISE scale a class 5 or higher). I do not chose certain types of beard as a pre requisite to BHT. I mean, I do it regardless of the kind of the beard. Of course the expected result should take this into account though

All in all, I made a trip out of it and it was an incredible experience. Vitoria and Rio are both amazing cities.
It is really worth it. Try to do the trip BEFORE the surgery though 😅😅😅😅😅😅😅. How is everything going @Digs49 ?

I’d have to change in Rio, but that’s ok. If it’s in Linhares, would that be of the clinic’s choosing or the patients?
It's a 40 min flight from Rio to Vitória. If you're willing to visit Rio, I'd advise you to do it before the surgery, so that you don't need to avoid sun exposure. As for the clinic, don't worry your procedure will be performed in Vitória.

Dr, in this particular case, is it possible to lower the hairline 10-20mm?
Yes it is and I did it in this last procedure 1 month ago. I'll post updates soon

I was on it a long time ago for about 11 years, but I was still thinning, although probably slower than I would have been without it. Being a NW 7 and no useable scalp donor left I knew I needed to use a lot of body hair and Finasteride was thinning my body hair, so I stopped using it. I’ve since had over 6500 body grafts transplanted to my scalp. I will never go back on Finasteride because at this point it wouldn’t be worth thinning out the body hair grafts that were transplanted to try to improve the natural scalp hair.
Very interesting case. You would benefit a lot by the use of oral minoxidil because it'll increase the diameter of your body-hair and therefore the diameter of the transplanted grafts. Helping your future HT and also your performed ones.

Looking forward to seeing more results @Dr. Felipe Pittella
long time no posting right, my friend? I've been very busy though. Lot of good cases to post but no time. I'm preparing for a presentation on the 2023 World FUE Institute in Bangkok so really no time. Hope soon to post. Thank you for asking though

Hello, guys. Look who is back for the forth procedure!
He came from Spain, staying a couple of weeks here in Brazil. His surgery will be next week. If wanted I can add the surgery details.
Took the chance to photograph his crown, donor and front with rooms harsh lighting.
Also he sent me these nice photos with longer hair.If you want to see some specific detail of the procedure, just let me know!

Dear Community,
Thank you for tagging me here.
First and foremost, I’m indeed sorry for not reaching the full expectation of the patient. However, as I told him in our last video call, we are not yet done. I’m willing to work with him to achieve the best result possible for him. Taking into account the patients expectations and claims combined with the real possibilities that can be offered by the FUE technique. So I offered him and I’m offering again, the possibility of a touch-up and further refinement of the areas of concern with no cost. This is not a privilege of this particular case, this is how I would treat any patient who’s expectations haven’t been fully met by my work. Fortunately, this is the first one, in almost a decade.As for the strategy, allow me to clarify. The patients forehead was larger than I have estimated in our video call and in the photos submitted for evaluation. Thus, the 4500 grafts I initially estimated was not going to be enough to meet my standards of density and coverage. So, with the patients consent we decided to transplant a higher number of grafts with no additional costs to the patient. The additional grafts were free. The complete details of this strategy with the precise area covered and density applied can be found in the description below in the photos.
So, the total area to cover was 170cm². The areas were divided as described below.
The density placed in each area and the amount of grafts implanted was:
Front: 50 to 60 (55) grafts/cm² -> 67cm²x55g = 3685 graftsTemples: 16 grafts/cm² -> 22cm²x16g = 352 grafts
Middle: 10 grafts/cm² -> 39cm²x10g = 390 grafts (***implanted only 250)
Crown: 35 grafts/cm² -> 42cm²x35g = 1470 grafts
TOTAL GRAFTS IMPLANTED: 5758
From the upper perspective:
From the Crown perspective:
Anterior perspective:
I hope you understand that if i'd transplanted any amount of grafts under the amount I've actually transplanted it would grow out low density areas and the result would be even further from the patients expectations. To be honest I'd like to put a higher density in the crown and temple areas but for that I would need even more grafts...
In regards of the periauricular area (left and right), if wanted I can also post some photos to show how it was before the extractions (long hair and harsh light).
Anyways, I'd like to reaffirm that I'm willing to try to reach the patients expectations with any further refinements necessary in order to reach the best result possible for him.
Feel free to ask any question regarding this strategy.
