  16. MY 6000 MEGA-SESSION EXPERIENCE Hello. My name is Umut Kartal. I am an M.D. and an anesthesiologist at the ASMED Surgical Medical Center. I have also been a hair transplant patient myself. My surgery was performed almost a year ago, and I wish to share my experiences with you. I have been working at ASMED for about a year and a half. Immediately after beginning my work, I realized that hair transplant surgery was more serious and difficult than I had previously imagined. Regardless of these difficulties, I could see that my colleagues at ASMED were performing successful hair transplants. By the end of my first month of work there, I decided to have a hair transplant myself. I was a Norwood 6, and at times, I really missed the beautiful head of hair that I had had in my youth. I shared these feelings with Dr. Erdogan, and upon his recommendation, I started to take finasteride. After four months of using finasteride, I had become a good candidate for a hair transplant. During the consultation prior to my surgery, Dr. Erdogan took measurements and made calculations on the caliber of my hair and its density. Then, he calculated the donor capacity and compared it with the density. Unfortunately, the results of the calculations fell fairly low, but even with them being so low, I was pretty sure about the potential result. Dr. Erdogan proceeded to design a conservative frontline in order to establish good equilibrium and homogenization. That frontline was exactly what I wanted. So, after taking photos from different angles, I was ready for the surgery. I was very anxious beforehand, but with the support of my colleagues and some medication, I confidently went forward with my decision. The first step was the washing of my head, and soon after, I found myself in the operating room. The head washing was actually one of my favorite parts of the whole procedure. Now I can understand why women enjoy going to the coiffeur! In the operating room, my blood pressure, heart rate, oxygen saturation and electrocardiogram were taken and monitored by my colleagues. Then, local anesthesia was administered to my scalp, and as it took effect, I felt complete numbness. Surgical assistant, Dilek, performed my extractions. During the extraction phase, I was given a wonderful and relaxing foot massage. Then, I watched an action movie from a personal TV beside the surgical bed. In the middle of the extraction phrase, the surgical team gave me a short break, and I had a fabulous shoulder massage. The massages were my second most favorite aspects of the surgery. After the subsequent part of the extraction, I had lunch. After lunch, my colleagues administered local anesthetics to my recipient area. Dr. Erdogan performed the incisions and then the implantation process began. Because of the extra large number of grafts (3000 grafts per day for two days), the surgery took a little bit longer (almost 8 hours each day). In the afternoon, I experienced a burning-type pain in the donor area. In addition to this, I was subjected to some aches and pains in my body due to the long duration of the surgery and the lack of movement. Anti-inflammatory painkillers were effective in both cases. During the surgery, and in order to protect myself against hypovolemia, hypoglisemia and hypothermia, I drank plenty of water and juice, ate an adequate amount of food and used blankets. At the end of the surgery, I underwent laser therapy before Dr. Erdogan performed the last check. At the end of the day, my patient assistant/translator gave me a shampoo set, some painkillers, a large hat with the ASMED logo, an ice pack, ATP spray and neck pillow. She explained every possible detail about their usage. Then I went home. The first night, I was a little bit restless because of the neck pillow. The pillow's purpose is to wake you up whenever you want to turn the side. I also took an anti-inflammatory painkiller that night for pain in the donor area. During the second day, the procedure was duplicated. This time at the end of surgery, final photos were taken and my surgery was perfectly completed. Of course I was tired, but I was also curious about the results. My wait began as I watched intently day by day. The first ten days were dedicated to washing. Each day, I washed my scalp with a special foam and shampoo. My colleagues helped me a lot during this stage as I experienced a lot of itching. Sometimes I took oral antihistamines to ease this discomfort. On the tenth day, my colleagues washed my scalp in a certain way as to encourage the falling off of the scabs. At that moment, normal life returned for me. The redness of my skin had not yet subsided, but I did nothing for this problem, and it eventually stopped spontaneously. In approximately one month’s time, I lost the hair that had been implanted with each follicle and the new hair began to appear. During the first three months, my physical appearance was not exactly attractive. However, on the fourth month, I suddenly realized that there was a huge difference, and I told my colleagues that if the result that I had now experienced was all that would ever occur, that it would be enough for me. Later, my appearance of course improved even more. Presently, I have no redness, but I do have lots of hair. There is a big difference between how I saw myself in the past and how I see myself now. My friends say that I look ten years younger and that I am more handsome now too. Thank you Dr. Erdogan and thank you to the wonderful team at ASMED.   18. Greetings everyone! I would like to present myself to those who have not yet had the opportunity to meet me. I am Alessio, Dr. Koray Erdogan's native Italian patient assistant. What should I say? Before beginning work at this clinic, I was involved with something completely different and this world of hair transplant was completely unknown to me. It was so unknown that I could not believe that I had any problems related to hair loss. Fortunately, in my family, no one was in bad shape hair-wise. Sure there were cases of a slight decline in density, but nothing serious, maybe close to a NW3 at the most. Just a bit of thinning at the temples and nothing more. I don't want to go into my family situation too much. But what I do want to do, is focus mainly on how and why I decided, (even me), do undergo this surgery. A while back the doctor was looking at me and said, "Even you need a little bit of adjustment". From the moment that he said that, everything began...the staff tried to convince me as to why the surgery would be beneficial to me, how I would look, etc. Truthfully, while looking at myself in the mirror, I saw the same handsome guy that I always was. Yes, it was true that I had a small issue that was slightly visible in afterthought, but with all sincerity, nothing that really bothered me. Being in constant contact with people who are much worse off than me and who are feeling at a disadvantage because of the condition in which they are living, I looked for empathy amongst my patients. I have to say that in viewing certain cases, I commented to myself, like all of you have commented to me, “Well, if he does it, it’s must be fine” and admitting to myself that all of us are perfectionists and want to have perfect hair and beards (our only trick in spite of women)! Little by little, I began to look at things in a new way and with a different perspective. I looked at the patients and the excellent work that takes place here at ASMED. I looked at the results of dozens of people that arrive here from around the world each day and I asked myself, “What if I underwent this adventure as well? What would the result be?” One day, while there was an Italian patient at the clinic, the doctor asked me to sit down in front of him. He began to design a frontline saying that the change would go well with my facial features and that I should think about it. When I went into the bathroom to clean off the lines that had been drawn, I looked at myself in the mirror and I actually liked what I saw more than I would have expected. Zap! Then and there, something went off in my head that “put fleas in my ears” as we would say in Italian. Several days passed, then months. Every time that I saw a new patient who was satisfied, as always, with the results, I thought. Every time I considered the way in which the entire staff worked and the innovation that was brought to each case each time to obtain the most optimum results with the least damage possible damage (I refer to the use of punches because of the white dots), I thought. I finally made my decision and presented myself before Dr. Erdogan saying that I wished to have the surgery if it was possible. We decided together to perform the operation one month later in September. I was truly ready to undergo surgery in one of the best clinics (if not the best, in reference to manual FUE) and I couldn’t wait to see the result (just like all the impatient patients). And so, it was decided! The 11th of September 2015, I underwent surgery with 2800 total grafts in the frontal zone as had been anticipated. On the momentous day, fortunately, I was not anxious or worried because of the 100 or more surgeries that I attended. I also knew that I was in the hands of the best, and so, I threw myself headfirst. Dr. Umut Kartal, our anesthesiologist, gave me a painkiller to help the anesthesia take greater effect and a sedative to relax my body (since I had to remain relatively immobile barring several pauses and massages) for several hours. The first session was for 2 to 3 hours in a sitting position for extraction, and the second, 3 to 4 hours, was in a supine position for the incisions and implants. We began at 11:00 a.m. and finished at 7:00 p.m. Thinking about everything that I had ever told all those patients, reassuring them about the anesthesia, the surgical room and the various phases of the operation, believe me…living the experience myself made me realize that something that is being told to you is very different than feeling it and experiencing it yourself. Everything went as I had expected, except the anesthesia. I thought I would feel a lot (thinking that fortunately I have a high pain threshold), but it wasn’t that way. The nurses that were on my case had hands like angels (I want to give a little praise to my colleagues). During the surgery, there were times, (within the limits of where they had applied the medication),I felt something, but nothing unbearable indeed. After the whole thing was over, I actually attended a birthday party for my brother and cousin that evening. We even managed to celebrate my surgery! At about a month’s time after the operation, the transplanted follicles began to fall (alas) with very mild redness. (I must say that as long as the follicles did not fall, there was nothing). Pimples also appeared in the recipient area as was somewhat expected. (Just as a reminder, I had had folliculitis in the occipital area, and after my hair was cut, I realized just how bad it was). So, Dr. Erdogan gave me an antibiotic and in one week, everything vanished. During the operation, it was decided that there would be no extractions from this area, thereby limiting the area from which the grafts could be extracted. Fortunately, I didn’t need a lot of multiple follicles since it was the frontal area that was being reconstructed and where mostly single and double follicles are in prevalence. The key word is patience. I knew that the road of anticipation is a bit difficult (2 to 3 months of “well where is this hair?”) However, I know that from the 5th or 6th month, up until the 10th month, (as I often tell my patients), I will have a “boom”. Consequently, all in all, I made the right decision to correct a condition that could have bothered me maybe in a few years. Together with Finasteride (as everyone well knows, 40% of the success of the operation in some cases is also due to the assistance of the medication), I will be set for the rest of my life (I hope)! Before concluding, (I hope to have pleased everyone with my words and explanations), I wish to thank Dr. Erdogan from the bottom of my heart for giving me this possibility. If I hadn’t known him, I probably would have felt uncomfortable at some point in future years with a situation that would haunt me.   22. My patients have been in forums from around the world since 2004. I like and respect the idea of forums because everyone is free to write whatever they want to express in their own way. I also do not believe that doctors should be present in these forums and I am fairly new to this one. However, I decided to respond to some comments with explanations in order to prevent misunderstanding and also for educational purposes. I would like to share some wet photos of the same patient. Wet photos under flash makes everyone unhappy, however, there are also necessary to detect miniaturization and to help us predict what will happen in the future. This patient has been taking finasteride for 10 years and had undergone an FUT procedure of 2000 grafts by another doctor in 2010. Most probably, he will be a Norwood 6 in time even under medication. He also desires to cover his frontline because of his previous FUT surgery. His hair loss area is minimum (180 cm2), his average caliber is 60 microns and his average calculated density is 2,2 hair/uf. Even if he decided to have me perform this surgery, 5000 grafts would probably not be enough to correct his problem in the future because for good graft coverage, we require 45 grafts/cm2. No one wants to live life under constant medication, however, there is no perfect technique in hair transplantation that can effectively address Norwood 5 or Norwood 6 baldness with original density. Donor management in FUE is becoming a more important issue everyday due to the potential for increased HT graft numbers. It is for this reason that we welcome our patients 1 day prior to surgery and at that time examine their density, their calculated density, the caliber, the coverage value and the donor capacity manually. These calculations are first made with long hair and are then repeated with shaven hair with the assistance of a computer. In 2015, "Coverage Value" became the new index for determining the minimum follicle number requirements for both donor and recipient areas with acceptable coverage density. I do not perform all extractions and graft placements by myself, but I do exclusively create all placing plans as well as perform incisions. Over the years, we have performed more than 4000 FUE cases using the same system and the same surgical team. Since 2004, more than 1000 of our patient cases have received over 4000 grafts. It is impossible to make every patient perfectly happy due to many factors including the patient's personal expectations, however, as an experienced doctor heavily involved in FUE research and innovation, I, along with my FUE colleagues learn new things and develop new techniques every day. I do not intend to respond to comments here on a continual basis, but on this occasion, I felt that it was necessary. Thank you for your time.
