Dr. Keene did recommend that he take Finasteride to stabilize his existing hair, and he did so for some time. Then recently, he decided to stop for a few months. He has decided to start taking it again because he did notice that he was starting to lose more hair again aside from the transplanted hair. People can only speak for the results they have seen. You can look at the before and after photos for both doctors, and get some idea from that, as well as meet with the doctors for a consultation. That way you can get a feel for your personal preference on who you would like doing your surgery.

She had recommended that he have 3500 grafts as a minimum to provide thinning coverage, which does significantly impact appearance when you have little to no hair in an area before. But the patient opted for a lower number of grafts. His results were excellent with the number of grafts he had for the area covered--and he is very happy with his results. The thinning coverage he achieved makes him look years younger. It helps that the hair in the back of the head is the last area to turn gray, so when it is transplanted to the front, the color alone helps restore a younger appearance for men who don't want to dye their hair when it is turning gray.

Dr. Keene does not always require that you take medication, she simply recognizes that you may continue to lose additional hair in the future, and are considerably less likely to do so if you take Finasteride. So it depends on whether or not you would still benefit from surgery as to whether or not she would require it prior to doing surgery. We know that hair we transplant is permanent. But we don't know whether your existing hair is (except in the back of the head). So that is why she recommends the medication. Not all of our patients take Finasteride, but she recommends it because she knows it works. If you don't want to take it, she will work with that in mind.

I just wanted to add my 2 cents about the list of doctors you mentioned. Dr. Keene has an excellent reputation, and is known for having helped educate other doctors about hair restoration techniques that result in a natural, aesthetically pleasing appearance. At the time that doctor Keene started doing hair restoration surgery, many doctors were still doing plugs. She started back in the mid 90's doing micrografts, and aligning the hairs to match native hair so the results were completely natural. She has continually refined her techniqes, sharing and exchanging ideas with her fellow hair restoration surgeons, and continues to provide only state of the art procedures. She has thousands of very satisfied patients, and is recognized by her peers as one of the experts in the field of hair restoration.

Dr. Keene is one of the most accomplished hair restoratioin surgeons available, in my opinion, based on my many years of working with her and from seeing the results of her patients, as well as other doctors' patients. She does mainly the strip procedure for a number of reasons, (FUE is not scarless, it increases transection so provides less yield, it decreases the number of grafts that can be harvested from a safe zone, where we know the hair is permanent--for more details on this feel free to schedule a conference call with her) but she also does FUE occasionally, when it is appropriate, and preferred by the patient. Both procedures result in only very small scars in the donor area, and can result in a virtually undectable donor scar. In the patients where the donor stretches, it can happen with the FUE technique as well, and then it is harder to do an excision because the scars are distributed over a larger area.

When posters say that there is a problem with saying that lateral slits are used when appropriate, I think its possible they don't really understand what lateral slits are. Any Dr. who has done natural hair transplants slants the blade to match the angle of hair growth, and Dr. Keene has done that since she began doing hair restoration surgery. However, when a flat blad is used, turning the blade horizontally, like this _ rather than vertically i.e. like this l is what is meant by lateral.

So, one case for example of where there is no discernible difference is if you make a horizontal incision or a vertical incision and insert a one haired graft in the incision, you are not going to see any difference when the graft grows out--a one haired graft cannot fan out. The idea is that if a graft of more than one hair is placed horizontally the fanning effect makes it look like more from the front of the head. But what about from other angles? In some areas, like the crown, it helps to create a swirl pattern to slant the blades horizontally instead of vertically. At any rate, sometimes the lateral incision is useful, but sometimes it is not, and that is why Dr. Keene says she uses it where appropriate.
