34 year old African American male who has always had a high hairline. He has not lost any hair, and there is no male pattern baldness in his family. He wished to have a lower, thick hairline.

The first picture shows two preoperative views. Note the location of the pitted scar which is 3cm below his hairline. The second picture shows the preoperative markings and the immediate result showing the hairline lowered 2.8cm. The remaining paired pictures show his result before and after only 11 weeks.

This type of procedure is best done on males who have this situation, namely a high hairline, a flexible scalp, and no history of hairloss. This is not a good procedure for male pattern baldness.

The last patient shown on the 2nd page of the photo album is represented here today. He was a class II baldness. In 1998 and 1999 he had 2300 grafts done to correct prior plug grafts. Over the years he lost more hair and wanted more coverage adjacent to his part line. In January of 2012 (see first five (5) pictures) he had 1681 grafts done to the crown and the periphery of his prior grafted area. He is shown 1.5 years after this last work. We have kept up with the progressive nature of his male pattern baldness, which is a good representation of what younger patients have to consider.

I don't know what you mean by a "tuck", but you are quite correct in describing this operation as a type of scalp (or forehead ) reduction . The operation moves the hair bearing scalp forward and the overlapped forehead tissues are removed. If the temples need advancement or the forehead needs narrowing, up to 800 grafts can be done with this operation without adding any additional donor scarring. The operation has evolved into a sophisticated one and is best applied to women who have no ongoing hair loss. It is not an operation for male pattern baldness.

Strip Harvest

65 year male who had 2300 grafts with strip harvest in 2011. His result was first posted 7 months after procedure. He had a second grafts session a year later with the donor harvest including the old scar. He is now 2 years after his first procedure and one year after his second one (see the comparison views) The new donor scar is as good as the original one. the benifit of a second graft session is shown. The last 5 pictures show his present appearence including his present, twice harvested, donor scar.

This 28 year old actress with high hairline is shown before surgery ( first 4 pictures ) including 3 cm lower marking she wanted, 1 before and after paired view on the operating table from the top (surgeon's view), 4 paired pictures one day after with sutures in place, 1 front paired view at 8 days with hair styled, and the last three pictures showing result at 9 months .

This early, 5 month, result shows a 34 year old male after 2600 grafts (strip harvest). The first three photographs show the patient before surgery. The fourth is a top view immediately after surgery. The following three side by side photographs show the patient before surgery and 5 months after. He should expect much more coverage at the end of the year. This case will be updated to show his progress. He is considering a seccond session.

This patient presented with concern for the tall appearing forehead due to a high central frontal hairline. In one operation, the hairline was brought down 2.5cm so the center portion met the temporal hairline at a lower level.

The first picture shows a view from the top (surgeons viewpoint) immediately before and after the operation. The remaining paired pictures show the patient one day after surgery with the sutures in place. She was able to resume work on the 2nd day after surgery, with her hair combed forward in her usual hairstyle. This was the equivilent of moving 2700 to 3000 grafts. We expect a totally undetectable, invisable scar in 3-6 months and will post the result at that time.

Thanks for the kind comments. This is a great procedure for young to middle aged women who are not actively losing hair. There is an occasional male who can benefit from the procedure. The beauty of it is that it does not preclude having follicular unit grafting at a later date.

19-year-old female, Fashion consultant/Model concerned with high hairline. She has no history of hair loss. First two photos show the patient pre-operative. The next photo presents patient immediately before and immediately after her hairline was lowered approximately 2.5 cm ”equivalent of 3500-4000 follicular unit grafts" The last four comparisons present the patient before and six days post operative. She was able to go out in public 2 days after surgery.

As her surgeon, I agree she is equally as attractive before and after. However, like men who are losing hair, the high hairline cut into her self confidence. Many women with this situation recall being teased as teenagers about their "big forehead" and were distressed about being unable to wear certain hairstyles. These patients need to have a lower and much more dense hairline than what is acceptable for most men. These hairline lowering patients are so very happy and grateful for this type of result, especially with the quick outcome.

The next photo pair shows her front view from before and one week after surgery when the stitches were removed. The scar line is slightly detectable as the hairs from the trichophytic incision and closure need time to grow in. Her last three-paired views show her before and six months after surgery showing the permanent result with hair growing through and in front of the scar. The last view shows a close-up of the hair growing through and in front of the scar at the hairline after six months.

23 year old female without any history of hair loss, who has always had a very high hairline ( 10.5 cm above the midbrow level) and wore wigs to have a female a stylish female appearance. Note the discoloration of the forehead from the hairpiece elastic banding.

With one hairline lowering operation utilizing our continually advancing techniques, we were able to lower the hairline over 3 cm to make a dramatic change in her appearance by the next day. The patient is show before, and one day after surgery, with stitches in place and able to resume most nonstrenuous social and occupational activity.

It would take 3500 follicular unit grafts done in at least 2 sessions, and one year of growth, to get an equivilent result.

The patient is a 50 year old African-American female who admits to losing hair on the front from past hairstyling practices. She now wears her hair natural and this should stop further hair loss. She has always had somewhat of a hair hairline throughout her life. She had very good scalp flexibility. She had the hairline lowering procedure last week and is shown on a bird's eye view (from the top) before and immediately after 2.2cm of lowering of her hairline. Her hair density is approximately 60 follicular units per cm2. This hour and a half procedure moved the equivalent of over 2200 follicular units. The patient is shown in comparative views taken today after surgery, and when she returned home to have sutures removed in six days later by a local physician. She has been extremely happy with the result even at this early stage and with minimal hairstyling has achieved the look she wanted

One year ago, he had a 2nd session of 1500 grafts, with pictures four through eight (4-8) showing his present result at age 66.

28 year old female with a high hairline measuring almost 9 cm from a line between the brows. The photos show before and immediate after surgery from the top and after 24 hours (note stitches are in place).

In three months, hair should grow through and in front of the scar.

Over 45 square centimeters of forehead was immediately covered with the equivalent of over 4000 follicular unit grafts. This was only possible because the patient had an exceptionally flexible scalp. If there is an unsatisfactory scar, a small session of grafts could be done. Less than 10% of patients have grafts after this procedure.

50 year old male, 20 years post chemotherapy for lymphoma, wanted greater density within his existing hair pattern. His hair was fine in caliber and his donor density was only 60 follicular units per square centimeter. Strip harvesting was done and 1700 follicular unit grafts were placed carefully between existing hairs in the front, midscalp, and crown. He is extremely happy with the improvement seen after 1 year.

The first four picture are pre-operative. The paired pictures show before and 1 year after his procedure, and the last photos is of the donor scar, also at 1 year.

This patient had a facelift along with a hairline incision/browlift. A non-tricophytic approach was apparently used. She was concerned about the visable scar and thin coverage at the corners of the hairline.

She was treated with 1400 follicular unit grafts, taken from the back of the head, above and below the old facelift scars. Therefore, causing no further scarring in the back of the head.

She is shown one year after her hair restoration surgery.

This patient with male pattern baldness, had 3 graft sessions in 2004-2005 totalling over 4100 follicular unit grafts. He is shown 7 years later.

The donor site area shows minimal detectable scarring, even after 3 donor harvests.

61 year old male with gradual hairloss over the years, which also involved his formerly bushy eyebrows.

In March of 2012, the patient had 2321 follicular unit grafts transplanted to the frontal scalp with 180 of the grafts used in each eyebrow.

He is shown immediately after the placement of the grafts in the scalp and eyebrows, and is also shown again 7 months again with most of the grafts growing out.

The patient is considering a second session.

Myself and many other surgeons on this site show early results of hair restoration surgery on younger men. Most of these men will continue to lose hair as they age, and it is important that enough donor material is preserved for later work to maintain cosmetic hair coverage.

This updated case deals with multiple procedures done over a 9 year span and followed 3 years after the latest fill-in procedure.

The patient had his first 1800 graft procedure at age 48 (3rd picture shows the grafts 3 days after surgery) he had a second procedure one year later and is shown at age 51 with very satisfactory coverage. Over the next 6 years, gradual loss of native hair occured, and a third session was done.

The photographs illustrate results at different times. He is now 60 and three years after his last 1600 graft transplant and maintains good coverage with enough donor reserve for an estimated 2000 more grafts, if ever needed.

One should always plan for the future in hair transplantation.

This patient underwent a hairline lowering operation in 2001.

33 year old male with recession and thinning of the front hairline and crown. The patient has a family history of limited baldness that he is following. 1500 grafts (200 of which were multiunit grafts) were performed in November 2008. The patient is shown 3 1/2 years after transplantation. It is important to point out, he has continued to use Propecia faithfully.

This 21 year old female of East African decent is concerned about the very large appearance of her forehead. Her hairline is not only high, but the curvature of her forehead contributes to the masculine appearance. The hairline on women, should be in an area where the skull curves downwards.

In this case, it required almost 3 cm of advancement, which was possible due to the flexible qualities of this patients scalp.

The hairline was lowered by making a trichophytic incision at the hairline, bringing up the entire scalp, moving it forward, and cutting out the overlapping forehead skin. The trichophytic incision allows the hair to grow through the eventual scar. She is shown immediately after surgery from the top of the head and with comparative pre and post operative views. The post operative views were taken only one day after surgery.
