How do doctors know where to place new hair grafts?

Hey guys, not sure if this is covered elsewhere or something obvious I have missed. I was curious, how do transplant surgeons know where in your scalp to make incisions for new grafts? How do they know if your existing hair follicle there there might be completely gone and unlikely to grow hair vs follicles that are just dormant as part of normal shedding?

They just stab where the scalp is bald.

They only implant on areas with clear signs of miniaturisation or loss.

The odds of transecting a healthy, non miniaturised telogen follicle during incisions, is very small.
