Of course this is an out of date method, but one must admit that it does do an excellent job at thickening up a thin area as long as that area is well behind the hairline and there are at least SOME surrounding native hairs. This patient had the old style procedure years before I met him and I have no doubt it worked well for a few years. That is until he lost enough of his native hairs in the front to allow the bases of the plugs to become visible. So I simply sought to blend the plugs into some new hair to give him some density and coverage. So I danced in his donor area around a ton of scar tissue and was able to remove 2500 grafts. I implanted them in the hairline, always important, and between the old plugs in the mid-scalp. Here's how he looked 18 months later. Am I correct in assuming that this type of hairline restoration could be achieved on virgin scalp if the patient had similar anatomy? Research, research, research! If the patient characteristics were similar (hair caliber, curl, donor density, etc), this type of restoration could definitely be achieved. Cases like this are extremely satisfying as it finishes what the other doctor started and lends naturalness and thickness. We get patients like this all the time, and I mean every week. This case represents the striking contrast between the old world of plugs and mini grafts and the modern world of TRUE follicular units. I say TRUE because way too many clinics are STILL performing mini/micrografts and CALLING it follicular unit surgery when it clearly is not.
