Regular Member Wen-Yi Wu M.D. Posted December 22, 2010 Regular Member Share Posted December 22, 2010 (edited) Patient Profile 2 This is a 27 y/o Chinese patient who received Follicular Unit Transplantation on March 31, 2009. -one session of 3520 grafts were done -General thinness on top of the scalp, see through appearance mimicking female hairloss Ludwig type II to III -Donor density of 82 FU/cm2 at the mid-occiput area, 70 FU/cm2 at the lateral sides. -Black color, straight hair , hair caliber 78 microns. -on Finasteride long time before, but stopped for 2 years prior to transplantation -Does not want Finasteride anymore for personal reasons -Patient expectation is to have hair covering top part of the scalp in one session. -Willing to shave the hair completely during the procedure. Treatment Plan: Recipient Area: -A total of about 110 cm2 to be transplanted. 50 cm2 in the frontal zone and about 60 cm2 in the midscalp -About 1700 grafts are placed on the front part ( 2 sides and the frontal core ) -the rest of the grafts are placed evenly in the frontal zone and the midscalp. -the new hairline is about 7.5 cm from the mid glabella -a combination of sagittal and coronal incisions with gauge 21 hypodermic needle for the single hair in the frontal hair line ; custom made 1 mm blade for the 2-hair and 3-hair follicles. -dense packing in the frontal hairline with about 50 single hair/cm2 -Frontal core (tuft) received 40 to 45 grafts/cm2 -the remaining grafts are placed evenly in the rest of the frontal zone and the midscalp -existing hair is shaved prior to the procedure to avoid injury of the existing hair and to expedite the creation of recipent sites and the graft insertions Donor area: -Total number of grafts ( 3520) harvested: 1189 single hair 2221 double hair 110 triple hair -Donor strip 31 x 1.5 cm was excised, average donor density 75 grafts/cm2 -Donor laxiety 20% as calculated by scalp elasticity scale. -Donor wound closed in 2 layers: one layer with Nylon as retention suture; and the other layer superficial running suture with Vicryl -trichophytic closure with trimming of the inferior edge applied. Remarks: -Typical of Asian patients, the hair caliber of this patient was thick, about 78 microns. We are expecting good coverage in one session. -Donor laxiety is only 20%, a wide strip is not recommended due to possible high tension with difficult closure and wide scar postoperatively . Instead a longer strip is advised. - Due to thick caliber of the hair, only Follicular Unit Transplantation is recommended to produce completely natural look. Mini-micro grafts or DFU procedures are not recommended. -Post-op Finasteride is still recommended because the patient is still young. The purpose of Finasteride is to prevent further hair loss. We did not emphasize regrowth since he had tried the medicine before and the results were not encouraging. People are polite and are especially friendly to foreigners. Foods are great also. Come visit Taiwan and for sure you will have a wonderful and unforgettable experiences.
