PATIENT'S BACKGROUND - Age: 27 y.o. - NW: Not Classifiable - Cicatricial Alopecia - Medicaments: None RECOMMENDED PLAN & TREATMENT This 27 years old young man came to visit Dr. Lupanzula for the first time in September 2013. During this first visit, he was wearing a hairpiece but the Doctor immediately perceived from his look that this was not a case like the others. In fact, when he asked him to remove the wig the bitter truth showed itself: at 1 year old, the patient’s scalp burned to a 3rd degree because of a gas explosion. This resulted in a very extended scarring (cicatricial) alopecia, affecting the patient’s frontal area and mid-scalp. This very problematic situation was worsened again by the wig the patient had been wearing for years, making his already damaged and scarred skin gluey and not transpiring correctly. In this situation, Dr. Lupanzula decided to propose him an operative strategy divided in two separated sessions: the first procedure, consisting in approximately 2800-3200 FUs, with FUE technique, in order to repair the burnt tissue in the frontal area, while rebuilding the patient’s hairline and adding density in his front up to the mid-scalp. The second procedure, of about 800-1200 FUs, still with FUE technique, with the purpose of restoring the scarring, burnt tissue left in the right side of the patient’s mid-scalp. It is also important to note that the patient had previously undergone a first FUT hair transplant in another clinic, leaving him with a 1x10 cm scar in his donor area and with a pluggy hair and very poor result as shown in the pre-operative pictures. Eventually the patient underwent both proposed surgeries, for a total of 3765 FUs, with Manual FUE technique, and we are pleased to show below the details of the two surgeries and next the related pictures. At the moment of the pre-operative consultation Dr. Lupanzula suggested the patient to start a Finasteride treatment, but the patient refused for personal reasons. Consequently, he was informed that this choice could result in the need of additional surgeries in the future, given that the Doctor could already foresee a loss in the vertex area. The patient eventually accepted this approach, also considering that his donor capacity was assessed as approximately 7500 FUs, amount that guarantees the resources for a future coverage of the vertex and of the mid-scalp too if needed. He considered the usage of toppik but he actually prefers to keep a more natural appearance without the usage of additional products. Dear delancey, Thanks for your query. Definitely yes, wearing a hair system can negatively affect both native and transplanted hair; as you say, this is mostly due to the glues used, which don't allow the skin of the scalp to perspire, to "breathe" in a correct way. This could turn out in a poor regrowth after a hair transplant, or in a more significant hair loss in a normal situation. Sincerely,

PATIENT'S BACKGROUND - Age: 40 y.o. - NW: VI - Medicaments: Finasteride 1g, since 8 years - Donor area density: - 90 FUs/cm2 in the occipital region - 85 FUs/cm2 in the parietal region - 75 FUs/cm2 in the temporal region - Donor capacity: ~8000 FUs - Recipient area surface - Front/Mid: ~80 cm2 - Recipient area surface - Vertex: ~80 cm2 RECOMMENDED PLAN & TREATMENT When the patient met Dr. Lupanzula for the first time was wearing a hairpiece and wanted a similar density as the wig he was wearing (reason for which his operation was refused by another clinic). Dr. Lupanzula made him understand that his expectations were unrealistic and proposed him the following approach: Given the extended hair loss the patient suffered (NW VI), Dr. Lupanzula recommended a strategy consisting in two separated sessions. Unluckily we didn't have the opportunity at that time to check the regrowth percentage. His hair caliber was assessed as 65 micron - good diameter - and for the moment he's not planning any other procedure. Regards,

  19. Hi wibbles180, That scar tissue may have been originated by a wrong execution of the incisions in the recipient area. In this case, we hided that bad appearance/discoloration by implanting hair follicles all around and on the scar tissue itself. Another solution to this problem could have been a tricopigmentation of the affected tissue. Regards,

Coming to your questions: the patient's donor density, after his FUT surgery, was assessed at around 6000-6500 FUs. After this FUE procedure, his donor capacity is approximately 3500-4000 FUs. And, as soon as we retrieve his hair caliber data, we'll update you. Concerning the scar: before it was indeed visible with a buzz cut 2 or 3, while now you should not perceive it at all. We'll ask to the patient to send pictures of his donor if and when he will cut his hair very short, so hopefully we'll have the visual proof! Best,
