I may be wrong but I see a norwood 5 or 6 in your future. Diffuse thinning is the worst. I would get a full assessment B4 you do anything. Your gonna need a ton of grafts if you want to maintain a real good head of hair over your lifetime. I would expect you have two or more procedure in your future if you have the money, time and grafts.
Make sure you get a top notch doc or you may get graft failure. Getting a lot of grafts from a cheap clinic is not worth it if you don't get a decent yield. I would consider a large FUT procedure, followed by another FUT in two years, then A fue afterwards to finish off. Hair loss is progressive and it sucks. I didnt start losing a lot of hair until I hit 48 years old.
I appreciate your honesty; both sides of my grandfathers had Norwood 5/6 severity.
I’m making inquiries and will ask about a full assessment about how to prevent such a possibility.

Quite surprised that your transplanted hairs thinned, especially while on finasteride. I wonder what went wrong?
The Temples didn’t have a lot of work the first time and I do recall the crown wasn’t filled in massively because I was only 29 on the 1st Transplant and told that Crowns don’t stop expanding until your mid to late 30’s.
I’m somewhat also suspicious that I caught COVID-19 very early on in 2020 which has lead to hair loss or it could be due to stress during this period… I’m not sure but My father is completely bald on top, My Maternal Uncle and their respective fathers (My Grandfathers on both sides) were bald which has made me somewhat damned despite Finasteride

I contacted Dr Bicer a few months ago and have been recommended that 2nd HT to keep my existing hair and regrow some lost hair is the best course of action.
I guess now I’m starting to think I’ll need to plan a trip back to Istanbul in March 2023

Well you’ve already had your first hair transplant so that means some of the hair that’s been transplanted is permanent. The more natural hair you lose, the more you’ll be left with only the transplanted hair.
As for whether or not you can wait for a 2nd hair transplant is up to you. Hair loss is unpredictable so nobody can tell you how much additional hair you’re going to lose. It’s also difficult to tell from the pictures you posted of the results how much thinning you have in the crown and mid scalp area.
While the hairline can certainly be improved upon, lowered and made more youthful looking, how low we can take it will largely depends on hair needed to fill in the thinning areas both now and in the future. Therefore, future concerns of hair loss must be considered.
That said, you can always choose to get a hair transplant now or wait but it’s important to try to get your existing natural hair loss under control by using non-surgical treatments if possible.

Thank you Dr Rahal

You've got a decent head of hair, what are your goals? Are you on medication?
Hi Melvin, I have expanded on this a bit more.

Patience is always key when it comes to hair loss and it’s medical and surgical treatments. I would definitely weight until 2023. All the best!
Thanks, I feel like the pandemic has been a contributing factor to this as I felt a couple of years back my hair was relatively thick enough.
I’m curious now about what my budget will be from next year etc, I live in NI so need to plan ahead on where to consider.
I think europe generally is very expensive and Turkey generally has a very good selection of brilliant doctors who are not as expensive.

Hi All, apologies for the delay in response.
Yes, I can provide you with further information.
It was a 3200 Graft FUE and there wasn’t a lot placed in the temples or the crown compared to the scalp (I had been told that my age at the time was unwise, the crown expands until you reach late 30’s or 40 until it stops).
I have used Proscar Finesteride 5mg since March 2015 (1mg per day).
I have started to take a zipvit Saw Palmetto Plus/Pumpkin Seed and Zinc capsule daily.
I use Nizarol Shampoo twice a week every 3 or 4 days.
I have explored dermarolling and onion juice for the scalp also.
I’ll add close ups later on.
I had been told I was likely to need another transplant in the future or likely loose it all eventually.
My goal is really to avoid being completely bald and have a thick enough density hairline… (I’m aware it will never equate to native density but close enough would be fine).

Hi All,
I had my first transplant in March 2015 by AEK. I have attached pictures showing my journey from September 2015 to early this summer.
I turned 36 a few months back and remember being told that I would likely need another one before I turned 40. Since early last year before the pandemic started; I feel my hairline is shrinking and hair has continued to thin since…..
Just want to ask how severe this looks before sharing more detailed pictures up close.
I’m curious to know if I’ll need to consider a HT as soon as things get back under control next year or if I can hold out until 2023?

Do you have some piks?
Feel free to check my profile pics if you wish

Dr Karadeniz did my surgery earlier this year ( March). I would say that I'm very happy with the result that I've had so far after 9 months; he gave his recommendations on what areas are best left alone until a later stage. Particularly, if you are of a certain age bracket or likely to follow a particular pattern if your hair loss is inherited.

Hi Robleary11,
I think it's pretty common from my own queries that most online pharmacies won't deliver to Ireland. Do you have any friends who live in the North? Maybe you can order it to their address and get them to send it to you? The cheapest place I know of where to buy it would be in Turkey from a pharmacy as it can be bought without prescription

A Japanese company has made this announcement recently.... Already patented and getting ready for 2018
-
Ok, thank you for confirming this. I likely didn't explain things accurately in the OP.
I was merely curious whether the eventual cure for baldness will have possibly something to do with altering this enzyme 5AR, or blocking DHT entirely. It really sounded interesting though
-
I just saw this video about how the enzyme 5AR produces DHT which leads to hair loss and prostate cancers due to very high levels of testosterone.
There is the claim that a diet can help inhibit this, but my question is this.... Can we "switch off" this setting? There was recently the discovery that an enzyme which causes asthma has been identified and can be stopped in the next 5 years.

Thanks Guys, sorry for the late reply. I'll certainly take a look now

Hi guys,
Just a simple query.
I've recently had a HT FUE (3 weeks post op). I keep hearing about how coconut oil has great components for the scalp and for fat burning etc.
I currently live in the UK but plan moving back home to Ireland, can anyone recommend any brands of this?
Which are:
Good value for money
Edible for healthy reasons
Good for the scalp and skin etc
I've seen a lot of price variance and not sure what to do

Hi David,
Yes these were the instructions as i remembered and recorded. I had 2 sessions and had the final one two weeks ago on this Friday.
Day 1 don't wash the hair (Saturday)
Day 2 remove bandages (Sunday)
Day 3-8 (Monday) Wash hair (donor area and transplanted) by putting moisturiser cream on areas very gently. Leave for one hour and then use shampoo to mix and rinse
Air dry hair
Day 9-14 same process but with gentle finger movements and some light towel drying of transplanted and donor area.
Air dry remaining hair
Day 15 onward. wash and dry hair like normal.
I was going to wash like normal two weeks to the day of the final procedure to wash out the remaining scabs etc.... I'm just nervous of lots of hairs being washed out etc
-
Hi guys,
I'm 14 days post op and asking for some advice. I've been ultra cautious and been washing the hair very gently in the time since returning from Turkey...
Here's an image of the top scalp skin now.
Just wanted to ask the experienced veterans if now would be ok wash the hair normally and thoroughly? Judging from this picture
I'm just concerned that I might harm grafts etc, thankfully no bleeding has happened at any point and not many little hairs have fallen out when rinsing the dinor and recipient areas...
-
Great news! Emre will do you proud (see my profile write up, I'm now 7 months post op..!).
Keep us all informed & glad this New Year is going to be a special one for you!
Kindest Regards - Campbell.
I certainly hope so. I've been suffering for year with genetic pattern baldness and been prone to some dermatitis but really want this to work out.

Questions for hair transplant afterwards:
Would Alpecin shampoo be best for post operation?
What would you recommend for sleeping positions after the op?
If you occasionally smoke cigars, would this have to be permanently ceased after the operation?
Are there alternatives instead of finestraside etc to take afterwards?
Would it be best to leave activities for a minimum of ten days?
Despite Genetic Pattern baldness, I am also prone to dermetitis
Any other recommendations?

Hi,
I thought I’d share the history for full transparency dating back to before and after my 1st hair transplant to the present.
