  3. Patient HNZ / 2,178 grafts

Bernstein Medical - Center for Hair Restoration Patient HNZ has Diffuse Patterned Alopecia (DPA), a type of androgenetic alopecia that manifests as diffuse thinning in the front, top and crown, but with a stable permanent zone in the donor area. He has medium-fine, straight, dark brown hair with a donor density of 2.0 hairs/mm2. The patient had one surgical hair restoration session of 2,178 follicular unit grafts. His “after” photos were taken one year after his procedure. Note the light coverage due to the patient’s low density, but still a very natural look after one procedure.

  10. Dr. Bernstein Repair / Patient LHI / 3,466 grafts

Patient LHI is Norwood Class 4 with straight, medium weight hair, a donor density of 1.8 hairs/cm2 and a slightly tight scalp. He had two large-graft hair transplants in the 1980s that left him with a pluggy hairline and a thin appearance in the front part of his scalp. The first phase of the repair consisted of removing 82 of the largest grafts and re-implanting them as individual follicular units. In the second phase, 3,466 follicular unit grafts were harvested from the donor area and used to soften the frontal hairline and add fullness to the front part of the scalp over three sessions. Three smaller FUT sessions were performed (instead of one larger session) to insure maximum growth in the scar tissue.

  11. Dr. Bernstein / Patient RPI

Plugs placed within patient RPI’s old hairline were not noticeable – at least at first. With further hair loss, the poorly planned hair transplant became very unnatural. Fortunately for the patient, the old plugs were located high enough so that camouflage was achieved with just one session of Follicular Unit Hair Transplantation.

  12. Dr. Bernstein / Patient JHG / 1,732 grafts

Patient JHG had extensive scarring from a sew-on hairpiece and three hair transplant sessions using plugs, many of which didn’t grow well. Luckily, the patient had salt and pepper hair, ideal hair for a repair. Dramatic improvement was accomplished with just one hair transplant using 1,732 follicular unit grafts, although a second session is planned. Unfortunately, his very low hairline remained.

  13. Dr. Bernstein / Patient LKE

After an unsuccessful graft removal by an electrologist, we excised Patient LKE’s grafts, divided them into individual follicular units using the dissecting stereo-microscope, and re-implanted the hair just behind the frontal hairline. The repair was followed by three small sessions of follicular unit hair transplants using hair taken from the permanent zone in the back and sides of his scalp.

  17. Dr. Bernstein / Patient QLR / 1,945 grafts

Patient QLR is a young woman in her early 20s who had hair loss on the front and top of her scalp following radiotherapy and surgery near her left part. Because the area was totally devoid of hair, she was unable to cover it, since the hair that was combed over the area from the part side had nothing to hold on to.

After one hair transplant session of 1,945 follicular unit grafts she had enough coverage that complete camouflage of the area was possible with proper grooming. A second session will give her greater density. It is important when transplanting hair into scar tissue that the hair restoration is performed over several sessions to allow the blood supply to re-establish itself in the area.

  22. Dr. Bernstein / Patient QLZ / 3,155 grafts

Patient QLA is a Norwood Class 6 with gray/blond hair a low donor density (1.7 hairs/mm2). He had two hair transplant procedures of 1,861, and 1,294 spaced a year apart. This a good example of the cosmetic improvement an older person can get with light coverage if the hair quality is good – even in the face of low density.

  25. Patient UAR / 2,336 grafts

Patient UAR is a Norwood Class 6 with slightly wavy, black hair and a donor density of 1.5 hairs/mm2. He had one hair transplant consisting of 2,336 follicular unit grafts. Photos were taken before and then one year after the hair restoration.

The patient had 2,336 follicular unit grafts placed over an area of approximately 150cm2; therefore, the average density is just under 16 grafts per cm2. The reason why the patient’s transplant looks so full is first, because he has wonderful hair characteristics — i.e. dark, wavy, coarse hair on dark skin — and second, I forward weighted the hair transplant so that in the frontal forelock (an area of about 50cm) I placed 1,250 grafts giving a density of 25 follicular units/cm2. The other 1,086 grafts were placed in the remaining 100cm2. Another trick that I use to give the appearance of more fullness is to place the larger follicular units (those containing 3- and 4- hairs) in the central-forelock area. I first described these techniques over a decade ago in my paper “The Aesthetics of Follicular Transplantation”.
