Dr. Jean Devroye Posted September 7, 2021 Share Posted September 7, 2021 PATIENT'S DETAILS ✓ 53 years old, NW VII, losing hair since he was 38, with a hair loss history in his family (paternal grandfather NW VII). ✓ No medications ✓ Average caliber hair. ✓ Normal follicular units. ✓ Medium % of hair in telogen phase. RECOMMENDED TREATMENT This 53-year-old gentleman presented a very advanced hair loss, classifiable as a grade VII on the Norwood scale. Since he wanted to be able to wear his donor hair short after the surgery, we agreed upon performing an FUE restoration consisting of two separated sessions. The first surgery took place in October 2019 and aimed at rebuilding the patient’s hairline, anterior and midscalp areas. During this first session we transferred 3150 grafts (2811 coming from the traditional scalp donor, 339 extracted from the beard donor - the area under the chin). One week after the surgery his donor area was already quite immaculate, as you can appreciate in the photos published below. One year later, in October 2020, the patient underwent his second procedure with the objective of covering the crown area. During this second sitting we extracted and implanted 3344 grafts (2843 scalp grafts, 501 beard grafts). The results are truly remarkable if we consider his starting point and the patient is understandably ecstatic about his improvement, so much so that he has just booked his third procedure (to further improve the general density) for November 2021. I am glad to share below a comprehensive photo and video documentation, which will be updated following the patient’s next procedure. The main advantage is to reduce substantially the number of damaged and transected hair, thus to raise the quality of the procedure. Do you feel as if you would have been able to harvest even more total scalp grafts if you had combined FUT and FUE over the course of multiple procedures? Thanks! Yes, without a shadow of a doubt the combination of FUT and FUE is an excellent solution for increasing the number of grafts to be harvested. This increment can be applied in the context of one single session (4500-5000 grafts usually easily available in a two-consecutive-days session); or globally, over the course of multiple procedures, when referring to the number of grafts available in a lifetime (probably, approximately 10000). Dr. Jean Devroye Posted November 25, 2022 Author Share Posted November 25, 2022 (edited) As mentioned in the original post, the patient was scheduled to undergo his third procedure to further increase the general density, shortly after his last check-up consultation in November 2021, During this third sitting I have extracted and implanted 3236 grafts via FUE and BHT techniques (3057 scalp grafts and 179 beard grafts) – which gives a total of 9730 grafts (8711 scalp grafts and 1019 beard grafts) transferred in three separated sessions. You can find below the surgery details and the photos showing the patient’s evolution until nine months after his third procedure. The general density is now much more important, and the slightly frizzy texture of the newly implanted hair will further soften over time. If we compare the photos taken before the first surgery to the ones the patient recently submitted, the improvement is quite remarkable.
