4600 NW6 - Dr. Taleb Barghouthi- Vertex Hair Clinic

Dear community,

This is a 41 year old gentleman who had his FUE HT done 4 months ago (July 2020). He is a NW 6with a family history of the same. His donor area is good with good hair calibre. We decided to attempt full coverage of the Frontal, midscalp and crown areas initially with around 4500 grafts. He understands that he most likely will require another session to add density to the areas. 

Surgical details:

FUE 0.9 mm punch

Hairline 400 refined single hairs

unrefined singles: 156

4044 Doubles , triples and quadruples. 

Total: 4600 with an average of 2.76 hair/graft. 

Sites made using 0.8 mm and 1.3 mm Sapphire blades. Density of around 45-35 follicles/unit 

I'm happy with his fast growth so far. I am even happier with his donor area. I think should he need further coverage he could really end up with a fully restored NW 6.

Update of progress 6.5 months from surgery. He will send some better photos according to the angles we requested or pop in clinic hopefully soon. Very pleased with his progress so far.

Looking great for 6 months. Shame the lighting conditions don't match the original pre op photos, however it is evident this guys already experienced a pretty epic transformation! 

His donor and hair calibre looked very strong to begin as you mentioned in your original post, how many more grafts via FUE would you be comfortable to further extract from this gentleman?

Yes I agree that the photos are in different conditions and angles are really homemade. :)) He has a great donor left. I think I would be happy with extracting 3- 4 k more. I dont think he needs that much hopefully but it's a good option to have.

Incredible work @DrTBarghouthi!  Very natural direction of the crown whorl. This guy must be very close to having 100% yield with this surgery!  The first HT is always crucial and you set him up for both short-term and longterm success. Keep us posted with any updates with this patient.  🙌

For 6.5 months this is a complete change. He must be extremely pleased with much more time left to progress still. Even the change from 4 to 6.5 months is huge. This is a great result with the use of only 4600 grafts for full coverage.

Is the patient on any meds to prevent any further dipping of his crown lateral humps?

Yes I have asked him to use finasteride on alternate days. He’s been on it since surgery.

That should hopefully keep him stable and not even need another surgery. This is truly a great result.

Unfortunately the last pics were just sent by him to me but he is due a proper in clinic follow up in a week or two where I will take more photos including the crown. I expect it to be of slower growth but it seems on the right track.

Just saw this lovely gentleman for proper follow up. I will share some photos taken in clinic: He is now around 7 months post surgery. Crown obviously still lagging behind and I would expect more growth in it although we did not do the same density in it as in the frontal and Mid scalp zones. Pristine donor!

I’d be very tempted to hit this with a second pass after 18 months and completely knock it out the park with a donor like that. 

An average of 103 /cm2. Yes very tempting but hopefully with more growth he won't need it or feel the need for it.
