I had hair transplant before 21 days with 3000+ grafts extracted from back and peri auricular region. The back has grown well but the sides look patchy. Is it shock loss or over harvesting? The sides are still a bit sensitive 21days post OP.

It's too early to say, I would wait at least 6 months before making a decision. It may be shock loss, but only time will tell.

Let’s hope its shock loss.  I can’t imagine the doctor would extract so many grafts in such a confined area.  There’s no need to do that.  You will know for sure in 4-5 months.  Be patient.

I suffered the same after my HT, it's 2 and a half months now for myself, But I'm not sure if in my case it's shock loss or over harvesting. I confirm best bet is to wait and not panic.

You can check my pictures here, your loss site is almost same as mine. However, mine looks like more loss at donor site than your's.
