Was hoping to have better coverage by now but I read somewhere that the crown takes 18 months to get to 100% so I’m hoping I will get a lot more growth in the next months.

I have personally seen improvements up to 18 months everytime I have had my crown area done.

Is there any study or physiological explanation why the crown takes longer than the hairline?

The grafts are the same (from the same donor area) and typically even more robust ones (thick multies) are planted. Do this multies grow slower than finer hair? Or is it just more difficult to create the illusion due to the whirl etc.? Or is it the lower density than in the hairline which requires the hair in each graft need to reach its full diameter and all hairs of the graft to grow to achieve the illusion? 

Blood supply is often mentioned but it this explanation does not sound logical to me. If there is not sufficient supply the grafts just would not survive to begin with.

Good yield for 5 months Portugal, it will be better by time 😊

I would suggest you to go back to Pekiner in about a year for another round. Let him harvest all of your beard under your chin (probably 1000-1500) and another 1000-1200 from your donor and place it to the same area. This way, you will have at least 6000 grafts on second and third area.

I think the argument for blood supply being the cause for slow growth in the crown is based more on observation rather than anything approaching scientific fact sadly. 

Bloxham mentions it and gives an explanation of it in his "how hair transplants grow mature pop" topic if that helps at all (?)

Pekiner cherry picks the grafts and told me I had no more donor area in my scalp.

I`m pretty sure Dr. Bruno Ferreira can extract more grafts from my scalp donor but he`s not fond of beard grafts.

If I do go for a third surgery I will most likely go with @Dr. Bruno Ferreira but I would need to convince him to extract +1000 grafts from my beard. 

However, I`m hoping this surgery with Dr. Pekiner will be enough for a decent coverage.

As much as you love Dr. Ferreira, I would say you need to go to a BH specialist.

As much as you love Dr. Ferreira, I would say you need to go to a BH specialist.

BHT specialists are HLC, Pekiner and Eugenix (Dr.Arika or Dr.Sethi). 
Pekiner is hyper cautious and said I have no scalp donor left so that would leave me with HLC (not sure if they are less conservative with the donor area because Pekiner came from HLC) or Eugenix.

Dr. BF also has experience with BHT (he’s just not fond of it) and I know he can work better with a almost depleted donor area so if I have to go for a third round I will probably chose Dr. BF in order to get a few grafts from the scalp.

Fair point. IF you can afford, I would choose Dr. Lorenzo directly as well. I've seen quite a few repairs with depleted donor area being "repaired" as well as controlled depleting of donor area, where overall density is getting lower, but due to controlled extractions it doesn't look bad.

Since I don`t need a repair HT I think Dr. BF would be right choice to get a few more grafts out of my scalp but only if I don`t get enough density from this surgery (here`s hoping!).

Well good luck then! I'm sure it will be a major difference since it's already looks good!
It's interesting to see a patient of Dr. Pekiner with very close timeline as myself. I was considering him for my surgery as well until he refused to extract only scalp hairs.

Hi guys

pekiner refusing working on patients with minitarising hairs/grafts. 
 

How do I know if I have this / find out. 
big chance to take booking surgery if there is a risk he refuses me when I show up?

Will Perkiner not do Hair transplants will only hair grafts?

@Jijypuru I wasn’t aware Pekiner refuses surgeries because the patient doesn’t want beard grafts you should confirm this with Pekiner. 

I do know he aborts surgeries if he finds miniaturized hairs. He basically takes Zero risks.

That’s why I recommend Dr. Bruno Ferreira more often even though my results with Pekiner so far are very good.

I remember the was a debate for one patient that went to Dr. Pekiner and after 100 extractions he refused to proceed with surgery due to short grafts. Dr. Pekiner thought they are miniaturised. I'm so glad I went to Dr. Ferreira as turned out I have short grafts as well. 
I can also confirm Dr. Pekiner refuses to operate if patient doesn't want beard grafts. That's the main reason I kept looking. It was kind of weird, he said "that's what I think and I will not change my mind".

5 months is early, even for the hairline. It’s important to remember that every hair follicle is independent, and will enter the anagen (growth) phase at different times, that’s why we say 12 months for full results, because that’s the time when most of the hairs have entered the growth phase. Patience is easier said than done, but necessary with hair restoration. 

Im considering BHT myself, but I will likely go to Dr. Arika and Dr. Sethi, I’ll be following your thread closely.

My last hair surgeon, told me I had 3000-4000 left on my scalp to extract for my next procedure so i'm not planning to take grafts form my beard.  Would you think Dr bruno or Lorenzo would be the best choice in my case instead of Pekiner?  I checked out Dr Lorenzo too but it appears his techs do the extractions?  I need to see more results for Lorenzo.

 

@Rolandas Followed your story, results look very impressive so far! wish you continued progress with Dr Bruno. Both you and @Portugal25 are looking great, i'm going to reach out to both doctors to see what they say.

Excellent. massive difference thus far. I am glad to see a nw 7 transform and beat the odds...with everyone in industry claiming it cant be done on a nw7.

Lorenzo is using tech to extract grafts, but it's like in H&W, this tech been there for ever and knows his craft. Just check their results and especially their donor on YouTube. They consistently produce excellent results. Also transparency is a huge factor, you can find cases that are not "wow", either it doesn't look super thick or something didn't grow, so they analyze and do a touch up etc.
The best choice is the one you choose. Be careful and don't rush. You have to be 100% confident in your doctor.

@Rolandas   When would they do touch up?  Would they know right away during procedure or is this something they would touch up on another procedure 6-12 months down the road?  I just got my response from Lorenzo and he said that I have "repair case" with depleted donor area and that there is no possibility to fill my mid- and crown areas. Also my donor area is not safe.  First time I heard this from a HT surgeon. :(

I was told by my last surgeon that I have up to 3000-4000 healthy grafts still in my donor area for my next procedure. 

I reached out to Pekiner and Ferreira at the same time so I just forwarded them detailed photos waiting to hear back.  Pekiner asked to see my beard and chest hairs for more donor options i'm assuming. 

Thank you for your feedback.  I want to take time and be 100% confident if a 3rd procedure would even get the results i'm hoping for.

However, you can’t compare Demirsoy with Pekiner or HLC because it’s two totally different techniques. 

Demirsoy is able to do up to 4000 grafts per day because he does motorized extraction which is faster, then does pre-made slits/holes and has the technicians implant the grafts into the 4000 holes he opened.
Slits/holes immediately start to close after being opened so if you have opened holes for hours before being implanted with grafts it means the holes need to be wider and the recovery more painful.
I’m not a fan of pre-made slits or surgeries where the technicians are responsable for part of the surgery. 
For me It’s Doctors doing the whole with nurses or techs providing assistance. 

HLC (2.5€/graft) and Pekiner (2.25€/graft) do manual extraction that takes longer and then implant using stick&place which means the Doctor opens 1 hole/slit and immediately implant the graft.
Because it’s the Doctor doing the whole procedure with a more precise technique the maximum grafts per day are just 1500.

So Demirsoy (1.25€/graft) can charge half the price of Pekiner or HLC and still make more money per day because he can implant more than double the grafts per day. 

I only recommend two options in Turkey: HLC and Pekiner (only if you don’t have miniaturized hair because he aborts surgeries if he finds miniaturized hairs).

The best option right now is @Dr. Bruno Ferreira he does motorized extraction with a smaller punch than Pekiner or HLC (easier recovery) and implants with the most recent technique - DHI - meaning he places each graft inside a implator pen and this  pen will open open the slit/hole and implant the graft at the same time.
This technique is limited to 2000 grafts per day and Dr. BF doesn’t go beyond 2 day surgeries (Pekiner and HLC go up to 3 days but they do less grafts per day).
Dr. BF also operates on just 1 patient per day and has his clinic in Porto where he charges 2.5€/graft, he also works in Madrid with one of the best FUE Doctors in the world - Dr. Lorenzo. 

Final note: it’s the Doctor that matters when doing a surgery because a bad doctor using the best technique in the world will still get you a bad result.

@Vpac if you read recent threads on ASMED you will see that they are a high price Turkish Hairmill with technicians operating on 8 patients per day and with zero to little intervention from the doctor.
There are several ASMED patients on HRN with botched results stating they were either threatened with lawyers or simply ignored.
I never considered ASMED or any other Turkish hairmill because I would never gamble my HT results on my luck to get a good tech team doing my surgery. 
I prefered to have a Doctor and his staff solely focused on my surgery for 3 whole days. 
Techs in Turkey don’t need academic healthcare background so you can have a tech that was washing dishes 2 months ago now doing your surgery. No thanks!

@Chances101  I had a HT with Dr. Rahal to rebuild my front when I was a 25 year old kid.

Because I didn`t take Finasteride my hairloss progressed so I lost all the hair in my midscalp and crown by the time I was 32 years old.

Last year at age 37 I went for a second HT to rebuild my crown and midscalp.
