Great video Melvin, loving the YouTube content.

Perhaps the only one I can think of, is it a patient isn't a good candidate. 

E.g. maybe they have a donor that will thin, or something along those lines.

Other than that I can't think of anything, you covered the big 3

Yea, an honorable mention is when patients have hair transplants too young without stabilizing their hair loss. That’ kinda coincides with what you’re saying.

# 4 scalp issues with some patients 

#5 no diet, too much drugs, alcohol, no supplements, no care, wash, shampoo, oil massaging etc... nothing at all will make it fail eventually 

#6 grafts inserted manually too deep or too high above the surface, which are unable to connect to nerves inside our head from where they take blood supply to initiate their birth and growth eventually 

#7 extreme baldness, less donor supply - no point getting HT as it will almost look like a failure at the end of the day

and yes chain smoking ( extreme cases) i heard will result in thinning blood pipes towards the brain and it makes hair so thin again.... i think each and every HT patient should do themselves a favor ( myself included ) and quit smoking forever.
