Hey all, 

I'm a 33 yr old guy who has been losing hair for 10+ years.

I had an fut procedure 5 years ago which was a failure (mostly on my behalf as I did not do enough research and failed to look ahead to the obvious future of further hair loss based on my family history).

In any case, there is no point in dwelling on the past more than I already have, so I am now looking at solutions and what options would be best based on my particular situation. 

I would really appreciate your opinions/advice on what options you feel would be suitable. 

My # 1 goal would be to camouflage my strip scar via beard grafts and then perhaps look at SMP into this area and then shave my hair down and then move forward with my life!

My # 2 goal if possible would be the above and then look at jumping on finasteride + further fue procedures to provide maximum coverage so I can grow my hair out and style naturally

anything is possible man. just have realistic expectations.

you can set get smp and grafts put into the scar to ease the contrast. look at Dr. Umar

 

sedond part would be holding onto what you have on top and fix up the frontal third/hairline with the remaining grafts via FUE

I agree hairlossPA.

It is always hard to have realistic expectations because you see some home runs out there and then envision this will be the case with yours, but yes, based on my previous result I am much more realistic in terms of what is possible, especially with my donor supply which I think is below average. I would 100% need to incorporate beard/body hair to get the kind of density which would make this worthwhile. 

Based on this I was looking at Dr Bhatti/Bisanga/Umar. 

I feel that this case is quite similar to mine and if I could achieve a similar result then I would be very satisfied. I could then have a much better camouflaged scar and would be able to fill any gaps with SMP if needed or even shave down in the future and complete with SMP

SBSinNYC, pics taken last week. Thicker looking top is #3 guard, thinner is #2 guard. Only other differences are lighting.

Just had a Skype consult with Dr Bhatti. He suggested SMP into scar and then 2000-2200 from scalp and 500 from beard to fill in front + mid scalp followed by another procedure in 6+ months where he would look to extract a further 1500 scalp + 500 beard for further density into front/mid/crown

What do you guys think of this result? I would be pretty content with this, especially as only 1600 grafts were used.

Pre-op the guy looked to be in a similar situation to me.

Dr Umar is a lot more expensive than Dr Bhatti though.

You already have a FUT scar, so you should look into the possibility of having another FUT by one of the better Drs on this site. They may be able do a FUT along the same scar line and make the scar thinner while also getting a decent number of grafts. I would at least look at that possibility with some clinics before doing SMP or body grafts into the scar. You can always do SMP and/or body grafts later if you still need it.

Thanks for the advice BeHappy. This is not something I had considered so I will definitely do more research and look into into this option.

Would you look at someone like Dr Konior for this kind of option?

I've done more research and have finished consulting with Dr Konior and Dr Bhatti. I still have consults coming up with a rep from Shapiro & Dr Basinga 

In a perfect world, I would like to do the following:

1. Get on 0.33 proscar every 3 days to maintain/regrow hair in the crown area

2. FUE or FUT with Dr Konior to add between 1750-2000 grafts to the front-mid scalp

3. 12 months later look at an FUE with Dr Bhatti to add beard grafts to scar

4. If required down the track, a further FUE with Dr Konior or if hair donor is not available, FUE with body grafts with Dr Bhatti or Dr Umar  

 

Dr Konior is extremely expensive though so I am not sure what to do. Perhaps look at Dr Nadimi? Any advice would be appreciated, especially from experienced contributors like @spex and @Spanker

Don't think about the money in this case as it will likely lead you down the wrong path and ultimately cause you even more grief down the line. Konoir has a very long wait list and should leave you ample time to save up, if I were in your seat I would only go FUT with him as you simply can't risk losing more grafts due to incompetence, also FUE now seems like a bad idea as you need to maximize the number of grafts you have left and you already have the linear scar.

I really do believe a larger FUT with Konoir or H&W would leave you very happy in the end.

If I had the choice where money was not an issue then I would definitely go with Dr Konior, but financially that option may not be possible. I'll see what I can do to try and hit this goal. The reason for going with Dr Konior are exactly as you mentioned, I do not have much donor left so it is imperative that i go with a top notch surgeon so I can maximise what I have. 

Is it standard practice to do FUT when one has had a previous FUT procedure? Are there any factors you know of which would prevent one from having a second FUT procedure?

Also, how would you compare Dr Gabel with Dr Konior in your opinion?

The most important thing is going with an ethical surgeon who will look at your case in the long term. For your case FUT is the smartest choice for sure, since you will require a larger session and graft yields and survivability will be better. The only thing I could think of is if your scalp laxity isn't enough for a second procedure, you could always email your case to dr Konoir and ask him yourself.

Thanks Payam, I never considered this option. I'll shoot him an email now and see what he thinks of this option. Would be much cheaper too I'm assuming.

go with a top notch doctor to do an FUT complete frontal zone repair and restoration (3300-3800 grafts), the hairline does not look too low so I assume they'll just be looking to add density and further enhance it, if I were you, don't touch the crown at this point and get on meds and minoxidil, once the frontal zone is repaired and you can grow out your hair, the strip scar is essentially camouflaged and I think you'll be very happy, otherwise just do SMP into the strip scar, dont waste any grafts trying to conceal it even body/beard - I believe that's your best most efficient option - your case should not be that difficult in the hands of a good surgeon, get multiple opinions thru consults - I'd suggest the following: Rahal, H&W, Cooley, Shapiro, Gabel, and of course Konior if you can wait.

What are the main differences with Dr Konior and Dr Gabel, not regarding waiting time.

I agree 100% with what Home said. That would be your best strategy. If you go to a good surgeon, you may even end up with a better scar than what you have now. I would definitely get on Fin. Based on my own experience as well as from pics I’ve seen, I would recommend Konior, Gabel, Cooley, Alexander just to name a few.

If the scar is in the right location and your scalp is still loose then I'd go with the FUT option.  If it was made too low or the scalp is tight it might not be possible to improve it.

konior, gabel, nadimi, shapiro, cooley all great choices.

Yeah I think I would prefer to go with the FUT option, especially as Dr K specialises in this. I was quoted $17K for 2000 grafts...does this seem right??

I think 17k is a lot but then I am in Europe and it is much cheaper here. I got 4387 FUT for less than that at an elite clinic in Europe. As you were willing to travel to India for this, you could try looking in Europe too. Hattingen (my clinic) and Dr Feriduni are great FUT clinics although Dr Feriduni was pushing me into FUE. Glad I went with Hattingen in the end.

Also, even if the scar is too low for another FUT over the top of that one, you could still have an FUT this time and then have the 1st scar taken out to reduce it in the future. Then you just put some FUE grafts into it/them so they are better hidden
