Dr. Jean Devroye Posted February 5, 2018 Posted February 5, 2018 (edited) PATIENT'S DETAILS ✓ 30 years old, NW V-VI,with a hair loss history in his family (father, same situation). ✓ In 2011, he had a previous procedure done in another clinic, 1000/1500 hairs transplanted (micrografts via strip). ✓ No medications. ✓ Medium hair. ✓ Normal follicular units. ✓ Normal % of hair in telogen phase. RECOMMENDED TREATMENT This gentleman came to my office to find a solution for his situation. In fact, in 2011 he had undergone a first procedure in another clinic that gave him very sparse and unnatural results, as well as a scar in his donor area, as you can see in the preoperative pictures. In this case, I felt that a COMBO (FUT + FUE) would have been the best way to proceed in order to maximise his donor resources. By means of the combination of the two techniques, I would have been able to: 1) partially correct the existing FUT scar, taking it as a reference while extracting the new strip, and 2) enhance the amount of available grafts via the supplementary FUE. The patient has undergone the surgery in January 2016, when he received 4689 grafts in two consecutive days. I have used the single grafts to rebuild the very first hairline, while the other grafts to considerably boost his frontal, midscalp and crown density. The patient has just sent in two different set of pictures showing his results 2 years after the procedure and is very satisfied about his current situation. One set shows his hair a bit longer, the other after having get a haircut. Please find them below. SURGERY DETAILS ✓ 4689 grafts ✓ Technique: COMBO FUT (3840 FUs) + FUE (849 FUs) ✓ FUE scoring and extractions executed with my WAW system, now widely used in the sphere of FUE. It consists of a very precise pedal that enables to control the movement of the punch, that I also designed myself. The main advantage is to reduce substantially the number of damaged and transected hair, thus to raise the quality of the procedure. The system is now used worldwide and several dozens of surgeons or centres are already using it. ✓ Incisions: 40/45 cm2 ✓ Grafts Breakdown: - 514 Single FUT + 96 Single FUE = 610 Single grafts - 1935 Double FUT + 341 Double FUE grafts = 2276 Double grafts - 1161 Triple FUT + 308 Triple FUE grafts = 1469 Triple grafts - 230 Quad. FUT + 104 Quad. Thanks for your comments and appreciation Andy and Spanker. I'm glad you liked the presentation. Indeed, the consumption of Finasteride would help him a lot in his situation. However, he has always been quite reluctant to take it and so far he hasn't changed his mind.
