This 58 year old gentleman was a class 4v, but really heading towards a class 5. He had a very small hair transplant procedure that consisted of a few hundred grafts over 25 years ago. After establishing his medical history and understanding the progression of his hair loss, I diagnosed his condition as Androgenetic Alopecia (male pattern baldness), ruling out any other possible dermatological or medical conditions which can cause hair loss.

This patient’s objective was simple; he did not want to have a large procedure but he wanted to achieve a very conservative hairline and look younger. That said, he also wanted a FUE procedure. As an executive in a business environment, he felt that his hair loss was starting to impact other’s perceptions of him and he was in an industry where perceptions were important. His hair was making him appear older than he actually was. His feelings were understandable and his goal achievable. That said, I explained that there was a possibility that he could lose more of his own hair. If this was to happen the transplanted hair would be able to stand alone and still look natural, but he would need another procedure to build it back up.

To plan the surgery we need to take his age, present degree of hair loss and possible further hair loss into consideration. We agreed that it would be best to keep some temple recession so that his result would look good for him now, at 58, but also look good as he continues to age. We had planned to utilize 1,500 FUE grafts, arranging them into the frontal 1/3 but we actually ended up doing over 1,700 grafts.

We transplanted a total of 1,749 grafts. The break down was as follows: 793 single hair, 689 two hair grafts and 267 three hair grafts. I used a significant amount of single hair follicular units on his part side/right side. The hair on this side needs to work hard and will shingle over the middle and left side of the scalp. This action will allow him to comb/style his hair without anyone detecting that he has had anything done.

These are his results 10 months after his procedure. I think his results go a long way in showing the hair loss community that you can get great results with a small number of grafts when the procedure is done correctly. Remember that his goal was to look younger; this goal was definitely achieved. I understand that when he tilts his head all the way back, he looks very thin in the crown, but people do not walk around holding that position. You have to keep in mind that this was an FUE case, yet the yield this patient achieved was as good as if it had been a FUT procedure. Needless to say, the patient is extremely happy with his results and is considering another procedure to add some density.
