This is a 37 year old patient who presented with a history of slowly eroding density along his entire frontal hairline and within the central tuft/midscalp region.His goal was to create a natural, high density appearance throughout the entire thinning zone.His restoration consisted of a 3000 graft session using highly refined grafts to dense pack the frontal hairline and central tuft/midscalp regions.Presented are one year postop views.These include a part view directly through the bald central tuft to demonstrate the density gain achieved.

did the patient have alopecia areata in the mid-scalp region btw or was that just a unique pattern of MPB?

Is this FUE?

 

The result looks great but the pictures don't show a case requiring 3000 grafts IMO unless all of the existing hair infront of the bald patch was transplanted over, which would seem like overkill to me. An immediate post op pic would be very informative.

I would disagree about the overkill. I believe that it is strip. If you consider that the the hairline was done (close to 2000 grafts) plus the spot that large that is completely bald, 3000 grafts does not surprise me at all. An immediate post op would tell more of the story, I agree, but going by the pre-op plan and the unique loss that needs a total rebuild, and the rebuilding of the entire hairline, 3000 seems right to me. You can always plant less grafts, but that is going to sacrifice density.

Agreed. This is a somewhat unique case, and that bald patch is a lot to fill with enough density to match the surrounding areas. Plus, he's 37 and his loss looks pretty stable, so this isn't some 20-something using an undue volume of grafts.

 

And the result obviously looks killer.

Nice and soft result.

 

I would guess the frontal area was shaved down for this one.

hsrp10 - Excellent observation regarding the possibility of the patch being related to alopecia areata. However, this was a slowly progressive thinning process which was consistent with an atypical presentation of male pattern baldness. The remainder of his examination confirmed that.

 

Rootz - Spanker was spot on correct with his comment. Not only was the hairline reestablished with dense-packing, the central hollow was filled with higher density grafting and the surrounding areas which showed evidence of miniaturization were reinforced with additional grafts.

 

1978matt - As you suggested, the graft zone was shaved for this case. He had a very haphazard follicular unit distribution. I felt that this along with the goal for localized dense-packing warranted a shaved approach.
