Advice for Hair Loss Sufferers Considering Hair Transplant Surgery

  1. First let me say that I am a semi-public figure, so unfortunately I will be unable to share pictures, but if I could, you would be as impressed as I am. I spent 3 years researching and interviewing doctors for my hair transplant. I probably met with a half a dozen doctors. I am thinning in front and, especially because I give live speeches and appear on TV, my work (and ego) required a more youthful look. I am fortunate indeed that I finally found Dr. Steven Gable. Dr. Gable is an artist and a scientist. He was easily the most passionate doctor I met about my hair transplant, and the most knowledgable, and his expertise was evident in all aspects of his center and his crackerjack staff -- from their professionalism to their state-of-the-art materials and work to their pre and post surgery processes and on to the actual, fantastic, hair transplant. 7 months ago I had 2,300 grafts placed. Mostly FUT but some others as well. Just yesterday I was getting my hair cut and I told the barber. She was dumbfounded. Had no idea and she was staring at my scalp. My hair and hairline is organic, natural, full, and great. One other thing to note: The morning that I went in for the surgery, my blood pressure reading was high, too high. Dr Gable put safety first and told me I had to get it back down or the procedure would put me at risk. Disappointed, we cancelled the procedure and I left. Later I saw my cardiologist and we changed my medication. Thereafter I rescheduled and had my surgery a month later. It is no small thing to cancel a patient on the day of a surgery. Space and nurses had been allocated. But my health was clearly more important than anything else to Dr. Gable. I cannot give him a higher recommendation. He is The Best.
  2. This patient is a 31 year old male who had 2084 grafts harvested via the FUE donor harvest method. This was a combined case with both Dr. Gabel and Dr. Konior at Dr. Gabel's Portland Hair Restoration Center. The patient is now 8 months after surgery and is thrilled with the results thus far. He continues to send in photographs from home, and we will continue to post them with his permission.
  3. This patient is a 33-year-old gentleman who desired tricopigmentation to the scalp to give him a full, shaved appearance to his hair. At this point, he is not interested in surgery, and likes to keep his hair very short. The photos show before and after 2 sessions of tricopigmentation. The dots are very fresh, and the color will deepen over the next couple days. He will return in about 6 weeks for a touch up. The video shows the actual procedure being performed on the second day. Video Link: Dr. Gabel performing tricopigmentation*
  4. This patient desired restoration of the frontal hairline and the frontotemporal angles. He had been slowly developing alopecia of these areas. He is currently under medial treatment to help slow the progression of alopecia. 2511 grafts were placed into the frontal region of the scalp. This patient demonstrates the steps leading up to the 4 month mark. The patient is planning to return in 2 months to also follow the progression of growth. At this point, the transplants are growing well, but they are still thin compared to the native donor hair. We expect over the next few months continued growth and thickening of the transplanted hairs, which will be nice to demonstrate in this forum.
  5. This patient is a 57 year old gentleman who desired to increase the density of the frontal scalp. He was not interested in lowering the hairline; he only wanted to increase the frontal density without causing a major change in his appearance. The patient is presented 19 months after surgery showing a much improved frontal scalp density. The hair was not trimmed or cut to perform the procedure. We utilized a combination of precut slits, and it was further refined with the stick and place method. The patient is extremely happy with the results and it has certainly met his expectations.
  6. Hi everyone. I'm looking for a good Dr. around the Seattle Area. I heard Dr. Gabel in Oregon is awesome. Any suggestions?? I thought H&W were my guys, but I realized they only do strip which I want to save for later when my hairloss gets more serious. Plus the linear scarring is a turnoff. I'm trying to fill a small bald spot on my hairline where I used to part it too much when younger and maybe add a little density to my hairline so the operation will be less than 1000 grafts. Maybe even like 300-500 if it's just filling my bald spot. Hope there are doctors out there that are willing to do small jobs like that... I'm looking for a good Dr. around the Seattle Area. I heard Dr. Gabel in Oregon is awesome. Any suggestions??
  7. This patient was interested in an eyebrow transplant to increase the density of his existing sparce eyebrows. He stated that he never had very thick eyebrows so his goal was to define the eyebrows more especially in the outer half of each one. He did not want them too thick. Approximately 230 single haired grafts were densely placed in each eyebrow. The hair direction is extremely important in eyebrows as the hairs must lay flat against the skin and in the correct orientation in specific portions of the eyebrow. The middle portion of each eyebrow is oriented up and laterally. The middle portion is oriented laterally, and the outer portion of each eyebrow is oriented laterally and inferiorly. If the hairs are not oriented properly, then the eyebrow does not look natural. The before photos are presented with the outline desired by the patient. The post op photographs are 6 months out. As seen, they are growing well and will mature over the next 18 months.
  8. This female patient presented with only a few, scant hairs in each eyebrow. When she was a teenager, she thinned them out by plucking; unfortunately, after a few years of thinning them out too much, they never grew back in. Her goal was to restore her eyebrows so they provided a nice balance of her facial features and definition to her eyes. To achieve her goals, about 250 single-haired grafts were transplanted into the eyebrow as shown in the design photos. I always have the patient draw their ideal pattern and adjust it for symmetry and naturalness. From a technical point of view, it is extremely important that the hairs lay as flat as possible on the skin, and that the natural curvature of the hairs are oriented correctly or else the patient will have "wild hairs" that are coming out in the wrong direction. The medial portion of the eyebrows are oriented superior/laterally, and as the eyebrow sweeps away from the central area, the hairs are oriented laterally with the curvature pointed toward the central portion of the eyebrow. This type of design and hair placement produces a very natural appearing eyebrow as seen in the "After" photographs.
  9. For several years, this patient has noticed a gradual thinning and decline of her frontal hairline. She felt that it was thinning to the point where she was unable to style it to her liking and always had to brush it forward to hide her hairline. It became very frustrating to her and ultimately she was self conscious about it. We designed a female pattern hairline by following her current hairline and lowering it to make a very natural female hairline. 2234 grafts were placed into the frontal hairline and immediately behind it to connect to her native hairline. Over 750 single haired grafts were placed just in the immediate 1 - 2 centimeters and the rest of the grafts were used to connect the new hairline to her native hairline. Close up photographs show the density of the frontal hairline around 60 FU/sq. cm. Once the grafts were placed, I surveyed the entire transplanted region, especially the frontal hairline, and additional grafts were placed in any spaces that could accommodate the hair. Photographs were submitted with similar orientations showing the timeline for the restoration process: immediately after surgery, 1 week, 3 months, and 1 year following the hair transplant procedure. A video demonstrates the 1 year result at approximately 85% growth; we expect the transplanted hairs to mature and thicken up over the next year of growth.
  10. This 58 year old male was concerned about the progressive alopecia of the frontal area and crown regions of his scalp. He described it as a slow loss over the past 2 decades. He noticed that along with the alopecia, that his hair was gradually thinning as well. When he presented at the clinic, he had significant alopecia of the frontal scalp and some alopecia of the crown. His hair was blond mixed with grey, but very thin. Additionally, the density of his donor hair was on the low side. We discussed his options in detail. He had been on a regimen of minoxidil and shampoos. Although we discussed the role of finasteride, he was not interested. In terms of a surgical transplant procedure, we discussed the potential outcomes given his lower density of donor hair and and the fine texture of the hair. On the plus side, his hair color/skin contrast difference would give him a more favorable outcome as they were similar. If his hair would be black, the result would be much thinner; however, since his hair color is blond, it was to his advantage in this situation. Given these facts, he understood that the end result would improve his appearance, but he would always have a thinner look given the characteristics of his donor hair. The photographs presented are 2 years following surgery which show the transplanted hairs filling the frontal and crown areas very well with the rest of the hair on his scalp. Although the density of his donor hair was low, the 2195 grafts were able to fill in the frontal and crown areas sufficiently to give him a natural, fuller appearance which he ultimately desired.
  11. This patient desired to have a thicker/fuller beard specifically within the goatee pattern. He is able to grow a beard at the cheek area, but always felt that the area around his chin was sparse. He also wanted to thicken up the mustache area. On examination, the patient’s scalp hair was thinner then his native beard hairs, but pretty good caliber. I discussed with him preoperatively that although I would be able to transplant into that area, the hairs would not be as thick and he may require more then one transplant to thicken the area up to meet his goals. The patient had 1573 grafts placed into the beard region centered in the chin area in the goatee pattern; several grafts were also placed in the superior portion of the mustache. The photographs are 8 months after the procedure where the hairs have been growing for a couple months. I expect it to certainly thicken up over the next year to fill in the area. At that time, we will reevaluate the results to determine if he would need additional grafts.
  12. This 41-year-old gentleman was concerned about his receding hairline and desired to have it recreated to a more youthful appearance. Although we discussed his thinning mid-scalp, that did not bother him as much as the hairline, which he wanted us to concentrate our efforts on. 2642 grafts were placed in recreating the frontal hairline and frontotemporal angles. Understanding that hair loss is progressive, he also elected to be more diligent with medical therapy by taking minoxidil and finasteride, which he had been using on and off in the past. He feels that since starting medical therapy, his hair loss has slowed down. The results are shown 2 years following the hair transplantation. Just before the last visit, he cut his hair short, which he feels gives him a nice, clean appearance with his new hairstyle, which he is very happy with. Shortly, I will add a video showing his two year results.
  13. This 28-year-old gentleman desired greater density in the frontal and mid-scalp. He had been losing his
hair for a few years, but felt it had stabilized. He was started on medical therapy, consisting of finasteride
and Rogaine, to further stabilize his hair loss.
3,033 grafts were placed into the frontal hairline and frontal mid-scalp in one session with a high-density graft placement technique with slits oriented in the coronal plane. The photographs,
taken six months post operatively, show tremendous growth and increased fullness in the transplanted
areas. Since he is only 6 months out, we expect continued growth and maturation of the transplanted hairs. Additionally, some growth can be seen in the crown area, which was treated solely with medical therapy (this is illustrated in the final photo). We will report his one-year results in the future.
  14. Ken is now 9 months out after transplanting 2569 grafts to the frontal scalp. The before and after photos show how well a transplant can change the appearance of someone. Ken looks younger, he feels younger, and it has had a tremendous positive influence on his life and career. Hair style is also very important; again, his curly hair is a bonus as that adds a layering effect. We have been documenting his progress monthly. Here are his previous links to show the monthly progress of the growth of the transplanted area. Looking at all the previous months that he was posted, the progression is fantastic and life changing. Here are the links to show the prior posts showing his monthly progress: Link to Week 1 Photos Link to Month 1 Photos Link to Month 2 Photos Link to Month 3 Photos Link to Month 4.5 Photos Link to Month 6 Photos Link to Month 7 Photos Link to Month 8 Photos 9 Month Before & After Photo Results
  15. Ken is now 7 months out after transplanting 2569 grafts to the frontal scalp. These comparative photos show his progress. His hair continues to fill out very nicely, is growing very fast now, and starting to add some density to it giving a fuller appearance. We have been documenting his progress monthly. Here are his previous links to show the monthly progress of the growth of the transplanted area. Again, I want to thank Ken for taking the time to come into the office every month to demonstrate the monthly progress of a hair transplant. 16. This patient is a 57-year-old gentleman who came to me after a failed hair transplant elsewhere in the Northwest. He stated that 2 years prior to my initial evaluation, he had a transplant, which did not achieve the desired result. In fact, he had problems with an infection throughout his scalp and an area in the left mid-scalp turned black. It took almost a year to fill in with scar, which is shown on the close up. On initial evaluation, he had many 4-mm areas of “white scars” which represented plugs that failed and had no hairs. There was an area in the left mid-scalp which was white and thin where the skin died (necrosis), and a white scar eventually replaced it. There were several areas of plugs that were very unnatural appearing. (See photograph with diagrams). The donor scar was wide and unsightly. We were able to perform 2 procedures on this patient with a total of 3570 grafts, with repair of the donor scar. Our goal was to repair the prior transplant by placing grafts where he lacked them, and thicken the entire scalp region with new transplants. We also wanted to make it look natural. I was not sure if the grafts would grow in the prior necrotic area, so these were placed very conservatively in that area to make sure there was adequate blood supply. The postoperative photographs are 3 year after the procedures. The entire scalp has been filled in and he has a very natural appearance to the hairline and scalp.

  18. This is a 59-year-old woman who had naturally deep frontal temporal angles. She consulted with us to fill in the area and to give a more feminine hairline. She desired to be able to wear her hair back; however, because of the lack of hair in the frontal temporal angles, she always wore her hair down to conceal the area. In one session, 2,111 grafts were placed bilaterally into the frontal temporal angles. Photographs show the patient preoperatively, six months after surgery, and one year later. In the six month photographs you can see all the transplanted hairs as they are shorter than the rest of the patient’s hair. When she was in for her one year visit, she said that the transplanted hairs are now long enough that she can tuck them behind her ears and she is happy that she can now pull her hair back and does not always have to wear her bangs forward. A video will follow that shows the one year result. Graft Count Single hair grafts: 521 Two hair grafts: 1,137 Three hair grafts: 440 Four hair grafts: 13 Total grafts: 2,111 Total Hairs Transplanted: 4,167
