Advice for Hair Loss Sufferers Considering Hair Transplant Surgery

My Story

  1. Many patients have inquired about the current status of development for the piloscopic surgical technique. Therefore, Dr. Wesley wanted to share a link from his recent presentation delivered at the 2015 American Academy of Cosmetic Surgery Meeting that provides both an overview and an update. We hope that you find this helpful.
  2. A 33-year-old male received a total of 4534 grafts with Dr. Carlos K. Wesley in order to fill in not only the frontal half of his scalp, but also to add density to his mid scalp. Below, he can be seen before and three (3) years after his FUT session. For his procedure (as with all of our patients), platelet rich plasma (PRP) and ACell were used as storage solutions.
  3. This 27-year-old man had undergone a follicular unit extraction (FUE) procedure at another practice one year ago. He grew frustrated that a significant number of the grafts remained as stubble even a full year after the procedure. He, therefore, had a platelet-rich plasma (PRP) series at our office. The results can be seen a few months later both in magnified form as well as globally.
  4. A 40-year-old man had worn a hair piece for years. He became fed up with the routine and wanted to liberate himself from it. Using a combination of scalp follicular unit transplantation (FUT) and BEARD follicular unit extraction (FUE), Dr. Wesley was able to create a frontal hair pattern that mimicked a conservative version of the hair piece so that his transition to free himself from it was possible.
  5. Patients are often pleasantly surprised to learn during their initial consultation that our practice does not require patients to shave their head for a follicular unit transplantation (FUT) procedure. As Dr. Wesley operates on only one patient per day, it is much easier to take the time to carefully navigate around the pre-existing hairs during recipient site creation. The benefit to patients occurs not only in the immediate short term (7-10 days) postoperatively when the redness can be better camouflaged by the longer native hairs, but also during the first few months of early growth when the transplanted hairs are just beginning to grow and provide coverage and density. Here is a recent example of a patient who underwent a session of over 2700 grafts to the caudal region of his scalp. You can see that by being able to comb his longer, pre-existing hairs over the incoming base of new hair, he is able to effectively camouflage the new growth and continue to lead a normal lifestyle.
  6. Patients are not required to shave their head during a hair restoration surgery procedure. When operating slowly and carefully, navigating between pre-existing hairs at their natural length is achievable. This allows patients to more comfortably and discretely return to work and/or social activities within a week to 10 days after the procedure. This video depicts a 28-year-old male who underwent a 2100-graft follicular unit transplantation (FUT) procedure with Carlos K. Wesley, MD in NYC. He can be seen before, during, and a few weeks and months after the procedure that was coupled with PRP treatment. All the while, his pre-existing hair is kept at its natural length. To critically view this patient's transformation, a sliding feature can be used to compare the superimposed images from before his procedure and his early results only five (5) months later. Click on the image below to access this feature.
  7. I am writing this heartfelt testimonial with great pleasure, gratitude, and relief. It is my hope that my great fortune in finding Dr. Carlos Wesley will also become yours. I am a fifty-nine year old woman who feels twenty-nine, so I wasn't going down easy! In Sept of 2014, I noticed my widow's peak was disappearing and my hair line receding. This was a huge dilemma because I have always worn my hair pulled back, a classic Audrey Hepburn look. Now, the me I always saw in the mirror, was vanishing. In October of 2014, I went to a NYC dermatologist where they performed a full work-up which comprised of a biopsy, and blood work which included thyroid, auto immune, and ferritin level. Everything came back normal. I followed the protocol, Avodart, biotin, and lots of vitamins. Nothing seemed to stop the nightmare. When I looked into the mirror, I could see that my hairline continued to recede, thin out, and finally, I saw an empty spot. A HOLE!!! Fast forward, it's now Sept. 2015, a year later. I started researching and reading every Hair Forum website that I could. I began seeing the names of the same few doctors over and over again, Dr. Carlos Wesley being one of them. One of the testimonials I read on a Hair Forum site inspired me to seek out Dr. Wesley and make that phone call. I called for an appointment and Barbara, the office manager, answered. I told her that I was hanging by a thread, was ready to have a nervous breakdown, and needed an appointment immediately. She told me that unfortunately I would have to wait four weeks for the next available appointment. OMG! Barbara promised me that she would call me if she had any cancellations. Two days later I got the call and was in the office the following week. Barbara was a life-saver because she really UNDERSTOOD. She was as excited for me as I was to get the appointment. We bonded immediately. Barbara GOT IT! November 6, 2015 was the end of the nightmare. I met with the brilliant, humble, and let's not forget to mention an accomplished surgeon and Yale graduate. He was soft-spoken, kind, sincere, compassionate, and also had a sense of humor. He made me feel that it was his pleasure to meet me and not me him. My consultation with Dr. Wesley was a miracle to me. As he explained my options, he showed me photos of his clients who had similar hair loss and the positive outcomes that they had. I knew I didn't have to suffer and live with this problem anymore. He told me that he was going to put the smile back on my face and restore my hairline. At that moment, I knew I was in the right place. The day of the surgery was actually relaxing. The nurses were professional and very sweet, making me feel comfortable every second that I was there. I can tell you that I felt no pain whatsoever. No anxiety. This doctor was an answer to my dreams! I write this so if you have any doubt, DON'T! The best advice I can give you is not to wait until your hairline or hair pattern is too far gone because Dr. Wesley will recreate the pattern and restore all the hair that you lost. FYI, 40% of Dr. Wesley's patients are women. I don't consider myself a great writer, but because I am so fortunate to find this gem of a human being and brilliant doctor, I felt compelled to write, sooooooooo DON'T WAIT! Debbie, New Jersey
  8. Much is still being discovered regarding the effect of platelet-rich plasma (PRP) on human hair follicles. In our practice, we have observed that, when used as a holding solution, PRP can help increase the survival rate and caliber of transplanted grafts. We've also seen trends towards earlier transplanted hair growth when PRP is used as a graft holding solution. An additional finding that has also been observed by other practices throughout the world is that PRP can help return some of the original hair color to follicles. PRP appears to stimulate melanocytes in hair follicles which, in turn, may help bring back some of the color to hairs that have become almost transparent over time. Below, an example of a patient who underwent PRP mesotherapy in our office and a few of the hairs in this image are colorless prior to his treatments and then have more of their original brown coloration about 6 months later.
  9. Carlos K. Wesley, M.D. was invited to author a chapter entitled http://www.amazon.com/Aesthetic-Series-Transplantation-Marc-Avram/dp/9351529320'>http://www.amazon.com/Aesthetic-Series-Transplantation-Marc-Avram/dp/9351529320'>http://www.amazon.com/Aesthetic-Series-Transplantation-Marc-Avram/dp/9351529320'>http://www.amazon.com/Aesthetic-Series-Transplantation-Marc-Avram/dp/9351529320'>http://www.amazon.com/Aesthetic-Series-Transplantation-Marc-Avram/dp/9351529320'>http://www.amazon.com/Aesthetic-Series-Transplantation-Marc-Avram/dp/9351529320'>http://www.amazon.com/Aesthetic-Series-Transplantation-Marc-Avram/dp/9351529320 in the recently-published textbook entitled http://www.amazon.com/Aesthetic-Series-Transplantation-Marc-Avram/dp/9351529320 edited by Drs. Rogers, Singh, Watkins & Avram. Among the topics featured within his chapter, Dr. Wesley highlights the efficacy and mechanism of Low-Level Light Therapy (laser cap & comb), scalp mesotherapy, platelet-rich plasma http://drcarloswesley.com/platelet-rich-plasma-prp/ (PRP), http://www.drcarloswesley.com/innovations/, Robotic Technology (e.g. ARTAS System), http://www.drcarloswesley.com/scarless-surgery/, and hair cell therapy (i.e. hair cloning). The first page of this chapter can be viewed here. http://www.amazon.com/Aesthetic-Series-Transplantation-Marc-Avram/dp/9351529320 explains hair transplant surgery in eleven concise, illustrated chapters. This book covers the various surgical techniques intended to achieve maximum hair growth for the patient. This book begins with a chapter on consultation and preoperative evaluation, including donor supply, hair calibre and control of hair loss. Further chapters cover instrumentation and anaesthesia, female hair transplantation, medication in hair transplantation, and a final chapter on future trends in hair transplant surgery. Aesthetic Series - Hair Transplantation also includes practical guidance on establishing the height and shape of the hairline, equipment needed for hairline design, holding solutions and graft creation. Enhanced by 124 full colour images and illustrations, Aesthetic Series - Hair Transplantation is an essential, up-to-date guide for dermatosurgeons and dermatologists.
  10. This 37-year-old female wanted to fill in her receding hairline and slightly advance her hairline. Early hair growth from a 1527-graft follicular unit transplantation (FUT) session with Dr. Carlos K. Wesley (NYC) can be seen even at 4.5 months. The graft breakdown is also included below. The early growth seen by many of the patients in our practice may stem from the use of platelet-rich plasma (PRP) and ACell Matristem as a graft holding solution. Full growth is typically appreciated about one year post transplantation.
  11. For those interested in a broad overview of surgical hair restoration, Dr. Wesley has recently co-authored an in-depth entry on Medscape. This is the largest online physician reference site and can be accessed by all for their research needs in familiarizing themselves with the process. The article is entitled "Hair Transplantation" and (in collaboration with Dr. Epstein) covers all aspects (pre-op, post-op) of hair restoration surgery. Hair Transplantation topics covered include: Background Anatomy Pathophysiology Etiology Epidemiology Prognosis Patient Education

  13. Over 500 cosmetic surgery professionals convened in New Orleans, Jan. 14-17 for the American Academy of Cosmetic Surgery’s (AACS) 31st Annual Scientific Meeting. Attendees participated in four days of in-depth, cutting-edge programming, and networking events. Dr. Carlos K. Wesley (NYC) was featured on the first day of the conference in the Technology Series entitled, "Technology: Advancements in Hair Restoration". Just as with his 2013 ISHRS Conference presentation in San Francisco, Dr. Wesley plans to make his entire presentation available to the general public via a video posting in the near future.
  14. Dr. Wesley has been invited as a featured speaker at the American Academy of Cosmetic Surgery Conference. This 31st Annual Scientific Meeting entitled "Technological Advancements in Cosmetic Surgery" will showcase some of the world's top surgeons with sub-specialties ranging from Rhinoplasty to Liposuction. The presentation will take place on January 14th, 2015 in New Orleans, LA and a preliminary schedule of presentations can viewed [https://c.ymcdn.com/sites/cosmeticsurgery.site-ym.com/resource/resmgr/Annual_Meeting_2015/AACS_Program.pdf]. Dr. Wesley has been invited to present on the Technology panel, "Advancements in Hair Restoration." Specifically, the presentation is to focus on the novel sub dermal approach to intact hair follicle harvesting (piloscopy []Recent Advancements in field of Hair Restoration Surgery) that has demonstrated a 45% improved survival rate over traditional follicular unit extraction (FUE) []Follicular Unit Extraction (FUE) in NYC - By Dr. Carlos K. Wesley, MD. These findings are statistically significant. As the Founder and CEO of Pilofocus [http://www.pilofocus.com/company-profile.html], a surgical device company dedicated to the development of this hair follicle harvesting technology, Dr. Wesley has presented the original findings from piloscopy []Pilofocus | Scarless Hair Transplantation Surgery - By Dr. Carlos K. Wesley, MD at the International Society of Hair Restoration [http://www.ishrs.org] (ISHRS) Conference in 2013. This presentation, however, will include updated information regarding not only findings in the donor and recipient areas, but also the experiences of patients who have undergone this surgical procedure.
  15. This 49-year-old gentleman wanted to finally cover the balding crown that had been bothering him for many years. A 2284-graft session by Dr. Wesley allowed for coverage over this large surface area. ACell and Platelet-Rich Plasma (PRP) were used as storage media. The patient returned 10 months after his follicular unit transplantation procedure.
  16. A 47-year-old male was bothered by the progression of his receding hairline. He underwent follicular unit transplantation (FUT) with Dr. Carlos K. Wesley (NYC). He returned to the office 15 months after his 2700-graft session for evaluation. The graft storage solution was a combination of platelet-rich plasma (PRP) and ACell.
  17. A common observation amongst patients during the early phase of hair growth following surgical hair restoration (either FUT or FUE) is ingrown hair follicles. The "pimples" that some (but not all) patients experience generally represent focal inflammation (folliculitis) where a newly-ingrowing hairs are beginning to surface. Patients can often feel reassured knowing that these generally precede more apparent hair growth over the following weeks and months. They can be seen in both the recipient area (where transplanted hairs are coming in) or donor region (especially in FUE where any residual hair fragments may be coming in). They may be more commonly observed in zones where the angle of hair implantation is the sharpest angle (e.g. the vertex or crown as seen in the attached image) In general, a simple cotton ball soaked in rubbing alcohol can be used to rub on each pimple. The microabrasion allows the nearly-ingrowing hair to surface and resume growth. It's relatively rare that an antibiotic is required, but even a one-week course of antibiotic can have a positive impact. The ultimate appearance is not impacted by the presence absence of a few "pimples" and patients should simply be aware that they generally represent an early phase of growth.
  18. A 42-year-old male wanted to advance his receding hairline. The most important thing to him was a natural appearance after the procedure. In the accompanying video, he can be seen before and 17 months after his follicular unit transplantation (FUT) procedure with Dr. Wesley (NYC)
  19. Hi, I'm Derek and am new here. I'm 31 years old and have been losing my hair slowly since I was 19. I have been on Propecia and have been on Avodart for the past few years. For the most part it has worked. I am not balding on my crown that bad, but my hairline keeps on receding ever so slowly and it's gotten to the point where I need to do something about it. Luckily I have pretty long hair so I am able to somewhat hide the fact that it is receding. Who is the absolute best person to go to for transplant work in New York? I have been told I have terrific donor areas and would only need my hairline filled in and a bit on crown. Dr. David Orentreich has been my family dermatologist for years, and has kept me with a good amount of hair, but I can't find many reviews on him. I know his father Norman created the hair transplant, but is he good? I have been doing research and it also sounds like Carlos Wesley is very good as is Dr. Bernstein . I feel like Berstein might just have the best advertising though! There is also Dr. Feller.. Can anyone lead me to the absolute best? Luckily, I make a good enough living that I am willing to pay to have the best results. I've attached some pics of my "situation" Any advise would be so so appreciated.
  20. This past weekend's "Advances in Facial Reconstruction and Cosmetic Surgery Symposium" provided audience members in New York's Mount Sinai (Icahn) School of Medicine an opportunity to learn about insights regarding the future of hair restoration (both surgical and otherwise). Dr. Wesley lectured to colleagues ranging from resident physicians to attending physicians in fields such as dermatology, ENT surgery, and plastic surgery about topics including hair follicle cell therapy, laser hair treatments, and scarless surgical hair follicle harvesting. Building on this lecture, Dr. Wesley has been invited to author a chapter on "Future Trends in Hair Restoration" in the upcoming "Hair Transplantation" textbook edited by Drs. Marc Avram and Nicole Rogers (Cambridge University Press).
  21. This 34-year-old doctor wanted to add density to his thinning mid scalp. While he was bothered by the thinning on top of his head, the crown region (in back) didn't bother his as much. A 2579 FU session of FUT with Dr. Wesley focused on his mid scalp. By creating a feathered "arc" along the back border of the pattern of transplantation and leaving a portion of his crown untreated, young patients continue to have a natural appearance even when the thinning portion of their crown expands over time. The patient returned 12 months later for follow-up images and video of his new hairstyle.
  22. This 22-year-old male was bothered by the progressive thinning of his hairline and the frontal portion of his scalp. His surgical hair restoration session with Dr. Wesley included included 2507 grafts incubated in platelet-rich plasma (PRP). In addition to his surgical session, the portion of his mid scalp that did not receive grafts was treated with PRP injection and stimulation with a 0.5mm micro roller. The patient returned to Dr. Wesley's office at 10 months. Early growth from his transplant can be seen and this patient's response to subcutaneous PRP injection is included in a larger investigation currently being conducted by Dr. Wesley's office.
  23. Dr. Carlos K. Wesley (NYC) recently authored the chapter "Androgenetic alopecia" in one of the leading physician reference textbooks in the field of dermatology: Treatment of Skin Disease, 4th Edition (Elsevier). His chapter not only provides an overview of the most common form of hair loss plaguing men and women, but also cites the best and most influential peer-reviewed articles that inform others about diagnosis and treatment of androgenetic alopecia. This textbook is unique in its format: equipping physicians with not only standard treatment strategies, but also second-and-thrid-line therapies for instances when other alternatives fail. Dr. Wesley covers the efficacy of current medical treatments as well as the various scenarios in which surgical hair restoration may be the most appropriate form of treatment.
  24. Here is a 42-year-old man who wanted the fullness of his hairline back. The results of a 2326-graft follicular unit transplantation (FUT) session with Dr. Wesley can be seen at 11 months after his surgery. The patient is ecstatic.
