  1. I have not heard good things about Giannato. Check out DR. Lindsey - he is in the same area as Giannato. At least get another opinion from another Doc, since you have not had great results from your current.

  3. Richie, But this isn't true and you know it. Just because you disagreed with HOW Dr. Farjo could help you, doesn't mean that he couldn't help you. You disagreed with his approach so you chose to have surgery elsewhere which was certainly your preogative. But to state that he couldn't help you is a lie. Nowhere did Dr. Farjo or Mick say that they couldn't or wouldn't help you nor did they say that you ONLY need 500 grafts in the crown overall. Dr. Farjo's approach was made clear above. In summary, he wanted to wait to perform any surgery on your crown to see 1) what the medication would do on its own 2) to see how much more your hair loss would progress in the crown. Had you insisted, he was willing to perform a small 500 graft session on the crown in order to give you a small improvement while monitoring the 2 items I described above. This would have left you plenty of donor hair for future work. The issue isn't that Dr. Farjo believed that you required less grafts overall. He simply felt that waiting to transplant a LOT of hair in the crown was the best approach. Clearly there are differences as to what is the best approach here. And you adapted to the one that you liked best. Many other surgeons will agree with Dr. Farjo on this approach including Dr. Beehner who wrote an article on Why Waiting to Transplant Hair in the Crown Might be a Good Idea. Regards, Bill P.S.   4. I also agree with Dr. Gabel and would add 2 comments on swimming/summer time activities. First, I swim a couple of miles a week and I have used various masks/goggles and I think just about all would apply undo stress to the donor area, possibly causing a wound separation. Additionally, the turning of the head from side to side will DEFINITELY stretch the donor area probably causing scar widening. Having personally performed 2700 facelifts in addition to hair transplants I can absolutely say that wound stretching in the first few weeks after surgery is key to hypertrophic scar formation. I authored a paper back in 1995 on the treatment of hypertrophic scars and keloids in the Archives of Otolaryngology or the Archives of Facial Plastic Surgery discussing this further if anyone has interest. The second issue is sun exposure on recently operated upon skin can permanently stain the scar line, or cause the recipient area to become hyperpigmented, requiring bleaching agents or chemical peels as treatments. So to conclude, I echo Dr. Gabel. If you have had a transplant, ease off stretching of the scalp and direct sun exposure for 4-6 weeks. William H Lindsey MD RESTON VA www.lindseymedical.com

  7. William, Congratulations on your recent hair transplant surgery with Dr. Bernstein. Remember that patience is a virtue. Honestly, the wait is the hardest part in my opinion. So my "words of wisdom" would include staying away from mirrors for awhile, taking monthly photos to measure your progress, and focus your thoughts on other aspects of your life.   8. Thanks for the comments Dr. Gabel. You are correct on several issues. First, we did need a lot of grafts packed DENSE to cover in this repair procedure, and using our custom made 0.7-0.8mm blades, we can do just that with minimal surrounding trauma. Second, by having a good patient-physician relationship we maximized preop planning and informed consent. And finally, you are absolutely correct in that I have been highly influenced by Dr. Feller over the past 8 years. I look forward to sharing more cases with you all over the next few months.   9. Dr. Lindsey Welcome to HTN and the forums. I think your planning on this patient was very creative for his specific situation. Clearly, as you stated, this was a repair case which requires a lot of grafts to adequately cover; but the donor was depleted so you had to really customize your strategy for this patient. At first look, it certainly looks unique, however, I'm sure there was a lot of dialogue between you and your patient to come up with this plan based on the way he parts his hair and his main concern to cover up the old plugs. it will be very interesting to see his results at one and two years when he grows out. What is definitely evident is your technique with refined follicular units, small slit size for the frontal scalp, and high density graft placement. Dr. Feller has had a positive impact on your hair restoration technique!! I look forward to seeing more of your work. It is important to remember that each case needs to be customized for the patient. That is where the doctor-patient relationship is so important so that the patient is educated in the pros and cons of various strategies to accomplish their goals; at some point, it is ultimately up to the patient to decide which direction to go. It is up to the physician to make sure that the planned strategy is of sound judgement to achieve the desires of the patient. Again, I look forward to seeing his results down the line.   10. Bill, Well said. I still agree with everything you wrote. Felix, I understand your disappointment, but NO doctor can tell how well an FUE will work out PRIOR to actually attempting it. Sure, there are doctors who CLAIM to, but NONE of them will allow a knowledgable witness into the room while they perform the procedure. The reason is pretty obvious. I have asked every doctor making this claim to allow me into their OR while performing FUE and NONE have invited me. In fact, the only doctors who have given me an honest invitation to watch their FUE are: Bernstein, Rahl, Lindsey, Alexander, Gabel,Jones, and Wong(when he was experimenting with them). Is it a coincidence that NONE of these well known doctors promise to hit a target FUE count? Of course not, because telling an FUE patient that they simply might not be a candidate BEFORE hand is simply the right thing to do. If the skin allows for it, we will max the session out. If it doesn't, other options have already been explored and agreed upon. Janna, I perform 3 FUE procedures per week. Most of the time they are of the smaller "lunchtime" variety which I have a hard time calling a "case" because they're so tiny compared to my strip cases. I call them lunchtime cases because I traditionally start them around lunchtime in my office (12:30p). Here are links to an FUE case I did just yesterday afternoon. This patient had an alopectic area from forcepts delivery during his birth. It always bothered him, but he didn't want to have a strip surgery to fill the area, so he opted for FUE. FUE is identical to learning a musical instrument. At first it seems near impossible to do (try picking up a guitar for the first time and playing the opening to "stairway to heaven". It isn't going to happen. But over time it becomes second nature. It goes from frustrating to sastifying if the practitioner keeps tenaciously trying and practicing and never giving up. Once confidence is gained that the procedure is do-able AND the doctor realizes he can actually do it, it gets easier and faster. My first FUE attempts in 2002 literally almost made me rip my own hair out. I made training myself in FUE a "self imposed residency". I must have quit and said I would never try it again a dozen times. But I was always compelled to return to the challenge time and time again. When I realized it may not be ALL me, I started to focus on the instrumentation and decided to design my own. This got us around some problems, but not all of them. FUE is a blind procedure that relies on "feel". The ONLY way to develop "the touch" is to just keep doing them. The bigger the case becomes, the more you lose your feeling of touch and the quality begins to drop off. In strip surgery I found this occurs for the average experienced tech at about the point they've already cut around 400 grafts. In FUE it is about 400-500 FUE grafts for an experienced doctor. You should encourage Dr. S to get into FUE. My door is always open to him if he's interested.

Dr. Lindsey, Very precise work...even immediately after the procedure, it almost (ignoring the Purple Ink Trail many of us know so well...and the blood spots, of course) looks as if his hair were merely buzzed down; no trace of the "pitting" or "cobblestoning" that seems a common concern.   13. I was going to add that the zoomed in photo is somewhat deceiving as Dr. Lindsey stated. With the native hairs shaved it does indeed look a little sparse. When looking at the photos that are not zoomed in you can more easily appreciate the density. Should be a good result.

Your 100% correct, but I highly doubt Dr. Feller is willing to put his reputation alongside someone who is either a.) unworthy or b.)going to tarnish Dr. Feller's reputation. Doctors do not give recommendations lightly (at least in the online community) I don't know Dr. Lindsey at all, have never spoken to him, never seen any of his patients in person-- however, I can say with utmost certainty that I would feel more comfortable recommending Dr. Lindsey then other docs who I have seen 3-4 before and afters pics. Why? Because of Dr. Feller. In fact, I think someone should start a thread to add Dr. Lindsey to our Recommended Docs list (if he desires)-- I think that would be a good thing. Take Care, Jason

  20. Hi all, and a slightly belated Happy New Year! Yesterday, I went to see Dr. Feller for a follow-up consultation. As many of you know, as part of his new joining of forces with Dr. Lindsey, Dr. Feller was doing consultations this past week at his office in (at the time) snowy Reston, VA. Heavy snow notwithstanding, it was certainly convenient, given that I live 10 minutes away! Dr. Feller and Dr. Lindsey were both pleased with the results, and combed through my artfully reconstructed mop with keen interest as Dr. Feller took photos and a video. Dr. Lindsey was notably impressed -- he being an experienced plastic surgeon -- with my scar, which was barely detectable. I am particularly interested to see the photos, particularly alongside those that Dr. Feller took the day of my procedure over a year and a half ago. Dr. Feller pointed out that the top-angle shots are the most telling, and indeed it was that shot, from June 16, 2006, that showed me just how much balding had happened! Long story short -- I'm going in again! Having been as happy as I've been with the first phase, I am going to finish the job with, based on Dr. Feller's estimate, another 3500 or more grafts to a) essentially restore the hairline to its original, and high, state, getting rid of the "receding" altogether, and b) add density to the thin areas on top, extending upward from the temples...if there is a more medically suitable term for that region of the head, I don't know what it is. An interesting side to Dr. Feller's plan was to create an even hairline, but to pack the left side (hairline and top), where my part has been since...yikes, 1984...more heavily than the right. This would, based on my head size and the coverage I have now, focus the grafts in the area where they will offer the most bulk, while still keeping a visibly consistent look. I am interested to hear from anyone who has seen this idea implemented; to me it sounds totally logical, and I do not doubt Dr. Feller's ability to pull it off (hmm...bad choice of words) with gusto. One impression, or re-impression, from talking to Dr. Feller again was the same "vibe" I had when I first met him: a genuine, likeable, sincere guy who offers his patients realism and compassion, and is committed to their best interest. And now that I've seen and experienced what the man can do, I am that much more eager to put my large head in his hands again, and to recommend him to anyone. I also liked Dr. Lindsey; although Dr. Feller did the "formal" consultation, Dr. Lindsey was present and interested in all details, and came across to me as not only a nice guy, but engaged -- someone who would take his patients' needs seriously, and who was committed to his craft. And so, I am as of now scheduled for late March...! The catch, so to speak, is growth in enough time to look presentable for my WEDDING in mid-August...and will trade a case of Scotch for an earlier date, with any Feller patient willing to swap ! Dr. Feller should be posting yesterday's photos and video soon, and I myself will add the large backlog of photos from this past year as soon as possible... Benjamin
  21. Hi there, Today is my day after surgery. I finally did it! Long time ago, when I was sporting long locks, my buddy noticed that I had a spot the size of a one-pound coin. What was once too small went on to become my nightmare. I have nice hair in the front & back - its the crown that betrayed me After 2 years on Propecia & a few visits to various doctors, I finally went under the knife. Literally, as it was a Strip Transplant. 1000 grafts & few hours later, I'm done. I have 2 teams to thank- 1. All the members of this forum. But for your posts, I would've got my head (& hair) butchered by Bosley. I consulted 3 of their docs, with each coming up with totally different estimates, when I had a minor spot on my crown. 2. Dr Feller, Dr.Lindsey & their teams I had a rough idea that its a surgery & not like visiting a salon as shown in the infommercials. This being my first Surgey ever (as I'm not a C-section baby), I was a bit nervous. They made me totally comfortable, playing some Fleetwood Mac (as much as I love Stevie Nicks, I got to say that I'm only 27 & she is not as hot as she used to be). Growing up under the sun, I got a thick skin The docs & the Technicians had a difficult time as the grafts kept popping up. They showed great patience (& a lot of compassion) in getting me the best results. Hats off to them! I'm still in the infancy stage of transplant.I'll post some pics once Amazon delivers my camera. Again, thanks to everyone (esp. for introducing me to Dr.Feller). I've waited a long time to get it done. Now I got to wait for another 8 months to see my head full of hair! Cheers!!

  25. Gentlemen, Thanks for your kind words. YES! I had a double-team workin' on my head!! I believe that Dr.Feller performed most part of the surgery & Dr.Lindsey assisted him. I'll post the pictures once I get my new camera delievered. Unfortunately, the one I have now is the first generation Olympus Digital camera I got when I was still in diapers Quick question(s) - 1. From when can I start using the concealers like Toppix/Dermmatch? I got to meet my clients on Tuesday - which is the 6th day from HT. As my recepient spot was shaved for HT, it is very prominent 2.
