  1. In this short video we show another 23 year old male who came in with his parents, and a class 5 or even 6 hair loss pattern. We did 2500 or so grafts and unfortunately he did AWFUL cleaning which likely autoextracted some of his grafts. Nevertheless he has a nice result, is happy and now married.   2. In 1989 I was a college freshman with thin hair that was starting to fall out. Graduated in 1993 and had considerable hair loss. By 1994 I was almost done hair-wise, very traumatic for a young man who is just starting his life, I freaked. That winter, watching a late night informercial on HT's, I though I had the answer. After two consultations with the 'Dr.', I fell for the micro/mini graft, very small scar, full head of hair, etc from this Long Island Medical practice. No internet at the time and I didn’t even know plugs were a thing and as been repeated as I have recently read up on numerous message boards, “You trust a doctor and never would you think they could be so unethical.” This is much different thn a snake oil salesman selling a hairpiece. This is a doctor doing surgery that lasts a lifetime. As a young man I should have understood the supply and demand model and I still kick myself in the ass falling for the full head of hair BS. 3 Procedures winter and spring of 94. By fall I was obviously very unsatisfied. Unnatural small plug look with a few large ones, bad distribution, horrible donor strip scarring, etc, etc. My hair is kind of fine/thin so I feel it was not as bad as some I saw as I noticed more people with plugs (never really noticed them until after my ordeal started) but now I was stuck and stupidly went for one more ‘free’ procedure as I was told they just need to make it more dense.The next 4 years I got by and since my hair is fine/thin texture, under certain lighting, dark, sun, etc it just looked like a young man getting pretty bald. Under many other situations, angles, etc you could see the plugs, cobble-stoning, unnatural hairline, etc. I got by, met a nice girl (now my wife) who never really noticed until I opened up to her, etc, etc. Some days, and even to this day, people don’t notice. Other times in a conversation I could see eyes look at my hairline. Sometime when I am out I see, or just maybe imagine, people checking out my hair/hairline. Myself, as I wrote, I got by but was, and still, always self conscience.In 1998 I visited Dr. Bernstein (at the time he was with NHI in Ft Lee NJ) and felt he was very honest though understandably I lost all faith in the medical community, especially cosmetic medicine. He explained what he and Dr Rassman were doing (which now made more sense to me) and how he could try to camouflage. I can say that I think Dr. Bernstein used some of the best technique that 1998 technology provided and he tried to camouflage my pluggy look. I forgot how many grafts I received and the ones he added were the size he stated but my coverage, and at that point I was very realistic, was still less thn desirable. This is in no way a bash of Dr Bernstein as he never up-sold me, never promised a full head of hair, etc. He was ethical and honest with his graft size/hair per graft, no bumps from his grafts, and it was definitely an improvement though not what I thought it would be. I understood a full head of hair would never be in the cards for me but thought I would get more camouflage in my hairline and coverage. I’ve lived the most normal life I can since thn though I am always self conscious. So here I am today, 25+ years in this journey and things are changing. I have lost even more hair recently (crown area getting even larger) and I am graying. The contrast between my skin, gray hair and dark have given me a more pluggy look. Also, I don’t mind being bald but so is the case of a 48 year old compared to 23 year old staring out life. I actually think I pull off the very thin/very balding look ok and I do not even mind my gray hair. That said, I’d like to look more natural (or at least less pluggy) and maybe even fill in more spots to even out the thinness. My donor area seems kinda depleted so I am not sure how possible any of this is. I am looking at a few possibilities to just move forward being less self conscious as my life moves on. I have no problem with my scalp showing and just want a more natural appearance. I could care less if the plugs in my crown are removed/re-distributed to other areas as my crown will never be covered anyhow. Some things I’d like to accomplish if even possible or recommended. 1) Remove larger plugs in my crown area slice them into smaller grafts and redistribute them to areas of need to try to maybe balance out my thin appearance 2)Remove larger plugs in my hairline and a lil behind hairline dissect and re-distribute them to that same area or others depending what doc says. (Does plug excursion and re-distribution grow at a decent rate or is the return rate poor? What about scarring? Blood supply/flow to the head if plugs are removed and redistributed.)3) See how that goes and if I have any hair left to use in my donor area, just use that to even out some coverage for a thin or more natural appearance in areas of need. With FUE has the donor hair expanded to your sides?4) Fixing my donor scars if even possibleI do not know what is possible but I do know technology and techniques have changed since 1998. I also know, though many docs are very reputable I am very nervous having someone touch my scalp again. I have started reading message boards on HT repairs, articles, researching Dr’s etc. For you young bucks and older gents out there thinking of getting HT’s, you have great resources and you truly need to do your HW. I am shocked that there are still ‘doctors’ who will do bad work and destroy peoples lives. It is all really sad and sickening on so many levels. I live outside of Princeton NJ. Some of the Dr’s in my area that have popped on my radar in my short research are Dr. Bernstein, True and Dorin, Dr Carlos Wesley, Feller/Bloxham, . Anyone have any experience with their plug excursion and re-distribution? What other Dr’s should I put on my radar as I start my research? I am not against travel and have just started reading up on Dr. Cooley in NC, Dr. Lindsey in Va (against excursion), etc. Thn again, I have no clue and do not want to fall for some sales pitch again. Cost is also a factor though I will cross that bridge when it comes and make my decision. That said, who else should I look into? I’m sure I will have more questions but here we go with my first post. Thank you for reading and I look forward to answers and more discussion on this in this thread.   3. This poor fellow went to a doctor who dabbled in hair. That started him on a path for multiple strip surgeries placed all over the back of his head, virtually zero resulting hair, and countless thousands of dollars lasering, smp-ing, re-lasering, re-tattooing those scars. He shopped with me for a long time and frankly I told him I'm not sure he has any good options. After months of discussions we agreed to excise 2 of the scars and coalesce them into one scar, which we may repeat in a year or so...to at least decrease the number of scars he has. We had in person, on phone, via email, and back in person BLUNT discussions on this and at least at 3 months or so he is happy to have less scarring. We'll see if he opts to do this again. He has fine laxity despite all of his surgery, so repeating this shouldn't be a big risk.   4. In this brief video we FUE 350 or so grafts into a crown scar, already previously attempted to be revised by a plastic surgeon. We've done 20 or so of these hair into scar cases in the last couple of years and presented our results internationally. Nevertheless....using planting analogies...we're taking relatively fragile plants from the donor region...stressing them with FUE extraction, and planting them in hard clay(scar tissue). By leaving a little extra tissue on the root balls and making the recipient slits a little larger, we've had good success.   6. It’s funny how things turn around. Before the black and decker type punches ruined donor areas. I wonder if in the future there will be some sort of strip type procedure with minimal scarring. I believe Dr. Bloxham and Dr. Lindsey were doing something called modified FUE. MFUE was essentially, circular punches about the size of the old plugs that were sutured and dissected. They may be on to something, but I haven’t seen many examples.

  8. In this short video we discuss a 40ish year old with fine hair, now 20 months out from surgery with a short haircut and virtually imperceptible scar. It can be tricky with the combo of fine hairs, and short haircuts--which limits layering and styling. I am not talking about a combover...but styling definitely adds to transplant results. But due to contemporary hair styles and lots of military patients...we see this alot. He looks very natural and that is the goal...an unremarkable hairline.   9. In this short update, our patient has turned the corner and really starting to grow from his case last winter. We've put up several videos of this guy, who runs a gym a few hours from our office. Interestingly we did have a bit of trouble with donor site irritation and finally figured out that he spent lots of time on weight benches...and I mean LOTS of time... and the sweat irriated his scarline. Once we determined this was the cause, all it took was putting a clean towel down on the bench to rest his head on, to solve the problem. He's since signed up for his second case and we'll surely have updates over the next 18months. The only real question is if he's going to propose to his girlfriend or not. The society has been on a roll lately, trying to combat black market clinics, surgery being performed without a doctor, and FUE slaughterhouses.   14. One day I woke up and a huge patch of my hair was missing from the top of my head. To this day I still have no idea how this happened. This completely devastated and destroyed my confidence being a 27 year old male with a huge bald spot in the front of his head. Instead of doing extensive research I just wanted to find the first quick fix as possible and came across Dr. Richard Giannotto website and this was the worst decision I made in my life. I went in for a consultation and was told strip surgery was the best option then i asked about my hairline and beard and was told that could be filled in as well. Signed up for Care Credit dropped the 5k for the procedure and It should of been a red flag when the doctor wasn't even doing the surgery it was 2 assistants who looked like they just graduated out of high school but I still went through with it anyway and now i have this horrible scar in the back of my head for the rest of my life and the surgery was a complete failure. I drive back and forth from MD to VA 2x a week for 2 months to sit under a light for 15 minutes because I was told it would help the growing process. A year after I went back and after seeing no improvement the doctor advised me since i was an African American it may take a little longer for my hair to start growing in and at this time I knew this was complete bullshit and I just got taken and i look even worse than i did before. After waiting another 4 months and seeing no improvement I found the hairrestorationnetwork.com website and start reading other testimonials about Giannotto and realized i had been duped. I found Dr. Lindsay and met with him and decided we were going to do 202 graphs to cover the bald spot in the front of my head and this was a success then we could go from there. Dr. Lindsay actually performed the surgery along with help from his staff and everyone was so nice and it was basically painless and after a week I shaved my head and no one could tell I had surgery. Dr Lindsay took some pictures of the before and after surgery and the 7 days later which I will post and will also post pictures at the 6,9,and 12 month point.

Common stories from many of our repair patients are: 1. Slick ads and an even slicker consultant with no doctor meeting preop 2. New doctors to hair offering "deals" with their new robot/automated FUE systems 3. Price Unfortunately not only is the donor area now somewhat...or more...depleted, but the patient now has lost all trust that the surgery will work as described, making everyone's job a little tougher. In a recent hair journal for the ISHRS, Joe Tillman--a pretty well known doctor rep/patient educator, did a bunch of spoof ads trying to warn patients against making stupid hair transplant decisions. Wendy and I discuss a couple of our favorites. I have no relation to Mr. Tillman, have never met him, and other than maybe an email or 2 on a forum discussion years ago, I have again no relation to him. However, I think he is spot on with these ads. And I applaud his efforts to educate the consumer against foolish decision making when it comes to hair surgery. In this video we show you a younger girl with a very high hairline and discuss the options of putting hair in front of the current hairline vs doing essentially a reverse browlift, pulling the hairline toward the nose, rather than the brows up...to lower the hairline. We're now doing these in the office in about an hour and TYPICALLY the scar is really minimal and concealed by hairs in front of the scarline itself. However, more than half of these patients ultimately decide to add a few hairs anyway...partly due to chasing that "perfection is the enemy of good" line I always talk about, but partly to address the upper temple/sideburn hairs.   18. I've posted a few of these in the past and this patient was kind enough to do a short interview. We discuss the pros and cons of hairline advancement vs hair transplantataion, and frankly a finicky lady like this patient ultimately will probably end up doing both. We brought her hairline down about an inch in less than an hour and at a month...other than expected redness of the scarline..she looks as expected with no pull back of the hairline. We did a nice 2800 or so graft case and, like many guys, at 2 months out...he thought he'd made a mistake. That's normal...all the hair falls out, your bank account is smaller, the scar is a little redder.....I understand the frustration. He stopped by at 7 months out and did this brief video, and hopefully will look twice as good at 14 months.   20. Promotional??? Cosmo I'm simply reviewing the lead article in the hair forum. Like all trends in medicine and politics, the pendulum swings too far and then comes back...probably too far..and then so forth. No doubt FUE is here to stay, and in patients with good donor hair and healthy roots with good connective tissue around them...it works probably as well as strip. Unfortunately many people have fine hair roots and they get shredded, yes shredded...during the most gentle extraction (like skinny little plants in this video ((from way back before my beard )) AND, given the volume of no yield or low yield FUE repairs I do, and the few other hair docs here in the US that I interact with do as well, I'm pretty sure a lot of people are getting butchered. Usually by a clinic just starting out in hair that added hair as a new profit center to an ENT/derm/plastic surgery office. And by the time we see them, the story is usually the same, they got "sold" by a slick ad and didn't do any research. Til it was too late. I've seen doctors do exactly the same thing. Lastly, I have now heard from several doctors, a bunch of techs wanting jobs, and a few patients...that with one of the robot systems this is occurring: Since the company supposedly gets a dollar every time the machine "fires"...the doctor puts on a 1.5 mm punch so that he/she will get at least one hair for transplantation. And if they get 5 or 6 hairs, supposedly a cutter will sliver that like we did minigrafts and micrografts in the early 90's. Now I haven't seen this personally but after hearing the same story from maybe 10 people, where there is smoke there is usually fire. I suspect that FUE and strip butchery is here to stay, and only by educational sites, such as this forum, can you the patient get educated and make the best choice for you. But get educated first, see a real doctor at your consultation, look at some results of that doctor, see a surgery going on if possible, and then decide what is best. I knew this post would be controversial and frankly MOST people on this forum are well educated. I'm not trying to sway you guys, you've already made your mind up and most of you know the facts. Its the virgin head that has seen a sales person who has tailored a surgery to fit that customer's budget and downtime with no discussion of the big issues, that I'm trying to reach--before they impulse buy a surgery. He makes it challenging in that with that short haircut....there is no layering to give extra hair "body" in the front, and that scar has to be excellent.   22. Michael was kind enough to do a couple of interviews. We did a frontal case on him a year or so ago and he returned to finish off the crown. Virtually always, I push patients to let me place a few hundred single hair grafts along the hairline. NO MATTER HOW TIGHTLY we pack hairs at the hairline...a few more always are beneficial. And particularly when we're back in the crown on that second case...those singles would be of less value than at the hairline. He lives nearby and I expect a final visit and video next summer.
