Advice for Hair Loss Sufferers Considering Hair Transplant Surgery

My Story

  1. Patient returned for his one year follow-up appointment, s/p 1870 FU grafts to recreate his hairline, which had always bothered him since childhood, and which had become more apparent with the effects of androgenic alopecia (male pattern hairloss). He is very happy with the results, as he has been able to go about all his activities "without even thinking about" his hairline/thinning look. His crown has been stabilized on finasteride. Patient is a 25 YO male with a congenitally high hairline/large forehead. In addition, he experiences Androgenic Alopecia, i.e., thinning predominantly affecting crown, hairline, FF, posterior FF, and bilateral temples. Plan: 1600 total grafts, to recreate hairline and temples. Approx 40 cm2 total area. Goals: Reinforce hairline and FF and round bilateral temple recessions. Actual yield at time of surgery: 1870 FU grafts: 700 1's; 1060 2's; 110 3's. Ellipse 18.2 cm x 1.0 cm. Acceptable closure tension. Tricophytic.
  2. 33 yo male 2100 FU Grafts. After an in depth consultation discussing the ramifications of recreating a more aggressive hairline at his age, this gentleman elected to go with the hairline shown, understanding the concepts of limited donor supply and the progressive nature of MPB. There is a familial history of very "self limited" crown loss in males on his fathers side, the loss in the crown being limited to thining (not complete hair loss down to bare skin) in a 4 x 4 cm area, and he himself does show miniaturization in a area 5 x 5 cm in the crown. At 7 months post op he shows about 60% of his grafts in the growing phase, and is quite pleased with the results so far.
  3. Dear Forum Members, Since Dr. Timothy Carman was approved for recommendation in August of 2008, he has and continues to impress this community with examples of his ultra refined results and his passion and dedication to patient education. Coalition member Dr. William Reed who works with him has also become one of his biggest fans. Dr. Carman's expertise, wit, and likeable personality also shine brightly in answering dozens of hair loss questions monthly on our forum and popular Hair Loss Q&A Blog. As the Old Russian Proverb declares 'trust but verify', we wanted solid evidence that Dr. Carman met the demanding criteria and standards set by this community before considering him for the Coalition of Independent Hair Restoration Physicians. To view the Coalition standards, click here. Thus, in addition to the impressive results Dr. Carman has posted on this community, Pat visited with and observed Dr. Carman in live surgery. He was very impressed by his passion and commitment to performing exclusive ultra refined hair transplantation. To see the highlights of Pat's visit, view 'Visit to Dr. Timothy Carman in La Jolla, CA'. Dr. Carman has the experience, skill, and staff necessary to perform over 3000 follicular units in a single procedure and dense packing when appropriate for the patient. Given Dr. Carman's experience and proven track record of producing ultra refined results, Pat and I firmly believe he should be considered for the Coalition. To learn more about Dr Carman's experience, technique and philosophy, visit his Hair Transplant Network recommendation profile. We welcome all input and comments regarding his potential inclusion. 4. This is a 52 yo male who had a previous procedure 5-6 years ago and was looking to add density to his frontal forelock and crown. His previous procedure left him with a 2-3mm scar, and his genetics rendered him a rather tight scalp as compared to the norm. As a result of these factors, donor yield from the 24cm x 1.0 cm strip was 1907 total; 304 1's; 1499 2's; and 163 3's. Our goal was to create a subtle increase in the frontal forelock density, add a little more definition to the temporal hairlines and add density posterior to those lines, and to add to the crown in a subtle manner to increase density there as well. The crown area is about 50 cm2, which we transplanted at an average density of 30 FU/cm2, in deference to the pre-existing hair. Total FF and temporal areas are about 16 cm2, which we transplanted at an average density of 30 FU/cm2, in deference to the pre-existing hair. I apologize for the presence of the flash in these shots- it was what we were using back a year ago- we have since updated our photo taking procedure so as not to include flash pictures. As there is no intent to "deceive"- the post op shots were also taken with a flash in our new setup.
  5. Dr. Carman Great result. There is always some uncertainty when grafting into scar tissue. The fact that you were able to do high density grafting into a scar with this result is excellent. Really nice job. Did you to a few test grafts first to see if they would grow prior to implanting the full scar? Steve Gabel, MD
  6. I am a Norwood 5A and considering a procedure soon. My choice is most likely going to be Dr. Tim Carman of La Jolla. While I was pretty impressed by him, I have not seen that many post op photos on this site. Can patients of his please post feedback and especially 6 months and up photos? This will help tremendously in my decision. Thank you.
  7. Patient is a 35 year old with bilateral temporal recessions (approximately 57 cm2) and thinning in the frontal and posterior frontal forelock area. We placed a total of 2300 grafts, which included combination grafting of the frontal and posterior frontal forelock areas utilizing 65 DFU's intermixed with 2,3 and 4 hair FU grafts to increase overall hair mass in that area. He presents here at his 7 month post op visit. Although he is aware that more growth can be expected, he is very pleased with the result to date. He is very happy about being able to style his hair in a shorter cut, something he previously avoided, keeping it long in the front to cover his bilateral temporal recessions.
  8. folliclehero- thank you for your opinion. Are you going to put any pictures up to document your procedure with Dr. Carman? i clicked on your profile and you have no documentation. Just curious, would love to see the Dr.'s work.
  9. This post is utter nonsense. CA has absolutely top-notch doctors. Don't listen to this guy's opinion. I had work done with Dr. Carman and the guy is a master surgeon, I'm sure there are others as well.
  10. Dr. Carman, The post-operative results look extremely natural and age-appropriate. Nicely done! Look forward to watching this case mature.
  11. Should be a subtle, but rewarding improvement!! Nice work Dr. Carman. Thanks for sharing the results. Looking forward to seeing photo updates as the transplanted grafts mature!!
  12. I had my HT done by Dr. Wong on 11/25 and got my staples removed on 12/4. Since I was back in CA, Dr. Carman was kind enough to see me and remove the staples for me. He spent time making sure that everything was healing well and even took pics to send to Dr. Wong. I have met Dr. Reed before and got a chance to meet Dr. Carman this time around. Both docs are amazing and I can see why they are recommended on this forum. It was really nice of Dr. Carman to make some time for me and remove the staples. Pam (his secretary) was sweet enough to schedule this appointment, even though I wasn't their patient. Thank you Dr. Carman!!! =)
  13. In November of 2006, when I visited Dr. Bill Reed's office, Dr. Tim Carman was nearing the completion of his one year fellowship in hair transplantation (to view the highlights visit http://www.hairrestorationnetwork.com/eve/showthread.php?t=146822 . Dr. Carman had an impressive background and had the good fortune to work with and learn from one of the very best hair transplant physicians in the world, Dr. Bill Reed. Given Dr. Carman's strong background in plastic and reconstructive surgery and his artistic talents he was a natural at hair restoration and has since joined Dr. Reed's practice, La Jolla Hair Restoration. Dr. Reed also entrusted Dr. Carman with performing his last hair restoration procedure. Dr. Carman is a native of La Jolla. Dr. Tim Carman shares Dr. Reeds philosophy of putting the patient's best interest above technical dogma and as well as his intense compassion and commitment. In addition to a medical background that includes experience in plastic and reconstructive surgery, Dr. Carman has a strong interest in painting, sculpting and music composition. He enjoys the artistic aspects of hair transplantation and his careful attention to detail was very evident when I watched him perform surgery. On the day of my visit, Dr. Carman performed a crown restoration surgery in which he carefully recreated the crown whirl. He, like Dr. Bill Reed, used very small needles to create incisions as small as 0.7mm. Dr. Carman took a great deal of time and care in removing a long and narrow donor strip, which measured 28 cm long by 1 cm wide. The donor density was lower than average and yielded 2,066 follicular unit grafts - consisting of 398 ones, 1,459 twos and 209 three and four hair grafts. The experienced staff then placed these grafts into the crown area. Dr. Carman performed a trychophytic closure using the beveled ledge technique advocated by Dr. Paul Rose. The closure and surgical pattern of the grafts was excellent in my opinion. Overall I'm impressed with Dr. Carman's passion and commitment to hair transplantation. I also appreciate his active sharing of his expertise on our discussion forum. I look forward to seeing many examples of his impressive results in the months and years to come.
  14. LosAngelesHair, Welcome to our community. Just so you know, the moderators of this community include Pat (the Publisher of this community), Dave, Blake and myself. Joe and Spex are highly esteemed members and are representatives of hair transplant clinics. I just wanted to clarify so you know who's who :-). While this may not be the popular opinion, I feel that there are many excellent physicians who regularly produce results on par with surgeons the select few surgeons you mentioned. While surgeons like Dr. Feller, Hasson and Wong and Dr. Shapiro produce excellent results and receive great press and regular positive public reviews, there are several less active physicians who also produce excellent work. Some of these excellent physicians do reside in California and even in the Los Angeles area. For instance, I highly suggest researching and consulting with Coalition members Dr. William Reed and Dr. Timothy Carman, who both serve the Los Angeles area. Coalition member Dr. William Rassman is also excellent and worth considering. To see other surgeons recommended on the Hair Transplant Network in Los Angeles and the state of California, click here. Remember, popularity is great, but it's not the only good sign of high quality. Take your time, research this community, consult with doctors, speak to and meet with their patients, etc. If there's anything else I can do to help, please let me know. Best wishes, Bill
  15. jes45468, Thanks for taking the time to share your thoughts and experiences with Coalition member Dr. Carman.
  16. Jes, Thanks for sharing a detailed account of your hair transplant experience with Dr. Carman. In my opinion, you've selected an outstanding physician and your results certainly show it. I've taken the liberty of creating a topic on our discussion forum promoting your experience, blog and photos. You can view the topoic at ""Member Jes45468 Hair Transplant Experience and Photos with Dr. Carman". I encourage you to post on the topic as well as you garner the support and encouragement of dozens of other patients just like you. Congratulations and best wishes, Bill
  17. Dr. Carman, Thank you for sharing this patient's recent procedure. Please continue to update us on his progress.
  18. Hey bud.... The big names around here are Dr. Rahal, Hasson & Wong, The Shapiro Bros and a few others. Those seem to be the names that come up the most.....HOWEVER, all of the recommended doctors on here are great. I went with Dr. Carman out of La Jolla and am very happy with my results, I expected just the corners to fill in but the whole frontal area of my hair is now thicker and more dense. I'm guessing the results would have been just as good with most of the recommended doctors. What you should do is type in some doctors names in the search box and check out some of their posted work samples, and read some threads about them.
  19. Dear Forum Members, As many of you know, we gather as much information as possible about a physician, their staff, technique and track record before they are even considered for recommendation on the Hair Transplant Network. Part of our review process involves getting input from patients and the members of this forum community. To learn more about this review process, click here. At the request of this community on this thread, I agree that it is now time to formally present Dr. Timothy Carman of LaJolla, CA for recommendation on the Hair Transplant Network. Then in time I expect that he will ultimately be invited into the ranks of the Coalition of Independent Hair Restoration Physicians. We would appreciate your input regarding his potential recommendation. As some of you know, Dr. Carman has a strong background in plastic surgery and trained intensively with Coalition member Dr. Bill Reed, finishing his one year fellowship in hair transplantation in 2006. He now works with Dr. Reed and his excellent staff doing ultra refined follicular unit hair transplantation. I know that Dr. Reed is very pleased with Dr. Carman's ethics, work ethic and final results. Dr. Carman's passion is demonstrated by his active involvement in our forum community. In the last several months, he has answered almost one hundred hair loss and hair transplant related questions, and has presented evidence of his ultra refined technique with great reviews from our patient and physician members. I also recently visited with and observed Dr. Carman in live surgery and was very impressed by his passion and commitment to performing exclusive hair transplantation. See my Visit to Dr. Carman's Clinic in La Jolla, CA Dr. Carman shares the same experienced staff with Bill Reed averaging over 5 years of experience, and has the ability to perform over 3000 all follicular unit grafts in a single session when appropriate for the patient. Dr. Carman shares Dr. Reeds philosophy of putting the patient's best interest above technical dogma and as well as his intense compassion and commitment. Using cutting instruments as small as a 20 gauge needle, Dr. Carman believes creating a combination of sagital (parallel) and coronal (perpendicular) incisions depending on the area of baldness and position of the native hair, best mimics the natural angulation and direction of the natural hair. He believes that in using this approach, he can easily work around existing "native" (natural) hair without requiring shaving. Dr. Carman uses a #10 single blade scalpel to harvest the donor strip to minimize transection. All follicular units are dissected and trimmed under microscopes and carefully placed into the recipient sites by his experienced staff to maximize hair growth yield. Dr. Carman uses the trichophytic closure technique for all hair transplants he performs and utilizes a single running suture so that the tension is on the underlying fascia rather than the skin. He believes this minimizes and uniforms tension on the wound for maximum healing and minimal scarring. Given Dr. Carman's impressive credentials and commitment to ultra refined follicular unit hair transplantation, I believe that he should be considered for recommendation. See our standards for recommendation. Patient and surgical examples can be found below: 2015 Grafts with Dr. Carman 2176 Grafts with Dr. Carman 2207 Grafts Post-Op with Dr. Carman 2300 Grafts Post-Op with Dr. Carman 1929 Grafts Post-Op with Dr. Carman 1821 Grafts Post-Op with Dr. Carman 3515 Grafts Post-Op with Dr. Carman 2893 Grafts Post-Op with Dr. Carman 2820 Grafts Post-Op with Dr. Carman
  20. Pt presented with bilateral temporal recessions and decreased density of frontal forelock area. On finasteride 2.5 yrs, rogaine BID 6 yrs. Pt had donor density of about 90 FU/cm2, with average to below average laxity. Strip of 27cm x 1.1 cm yielded 2,820 total FU grafts: 694 1's; 1566 2's; 560 3's.
  21. Dennis, I'm sorry that you are having a frustrating time contacting physicians recommended by our community. I can assure you that our database of recommended physicians is up to date and that each physician has been prescreened by our entire community. To see how we recommend physicians, click here. We are currently in the process of investigating the Dr. Ross issue. I have left a message for him on his personal cell phone and intend on following up tomorrow. However, I have not heard back anything yet. As for Dr. Reed and Dr. Carman, they are definitely still practicing and are doing outstanding work. Feel free to use the "search" feature on our discussion forum to see what their patients and our members are saying. I'm sorry you had a difficult time getting a hold of any surgeons recommended by our community today. But if you've left a message, I trust that someone will get back to you. You can also visit their profiles (click on their names above) to email them and/or do a free virtual consult. As for Dr. DeYarman, he is not currently recommended by our community. However, you are free to use our forum to research and ask your questions about Dr. DeYarman and/or any and all doctors you are considering. Is their any reason why they were not listed on your referal notice??? Its seems that everyone on your list i called either doesnt pick up or has closed up shop, whats the deal man with your website??? So far this coalition website has been pretty disapointing and thus is leaving me to just not do the procedure all together. Not trying to put your website down, just need you to stand behind what you preach on it with correct and accurate facts and information. I am very busy and dont want to be wasting my time. If in fact that is the case with Ross and the non response im getting from Reed and Carman, i will end up going to the great service and support i've received at Dr. Deyardmans office. Could someone help me with this situation?? ---------------------------------------------------------------------------- Sender info: IP: 70.0.161.238 <http://ws.arin.net/whois/?queryinput=70.0.161.238> Browser/OS: Mozilla/4.0 (compatible; MSIE 8.0; Windows NT 5.1; Trident/4.0; .NET CLR 1.1.4322; .NET CLR 2.0.50727; .NET CLR 3.0.04506.30; .NET CLR 3.0.04506.648; .NET CLR 3.0.4506.2152; .NET CLR 3.5.30729; BRI/2; BO1IE8_v1;ENUS) ----------------------------------------------------------------------------

  24. I'm back. It's been a few years and I'm still trying to fix the scars and bad transplant from my first 2 operations with Chajchir. The expense has been high and amount of work has been absolutely exhausting. Frankly, I wish I had just decided to shave my head and buy gold all those years ago. Having found myself in Argentina again, I decided to check out the cost and now numerous centers of hair transplantation including Alvi Armani's. All costs were about the same and only the 2 main centers offered FUE; Alvi Armani and Medical Hair. There was one more major center called Hair Recovery that wanted a $100 consultancy fee and even though $100 is not much in light of total costs, just the idea that they want me to pay $100 in order to potentially pay them several thousand dollars frankly pissed me off. As I explained to them to no avail that I don't remember Bernstein charging me any consultancy fee and he's the absolute best in the world at this procedure. I know there's alot of Armani haters here as I've just been reading all the posts. Sorry to tell you all but I decided to go with them and had a 1500 graft h/t yesterday. I've had alot of experience with horrible doctors like Chajchir and excellent doctors like Bernstein in NY who did a very small procedure simply to fill in the hellish donor scar created by Chajchir (re: the movie "Saw") and another team of excellent doctors in La Jolla, CA (Carman and Reed) who put 2K grafts into the front after I had to laser off the front 1.5-2.0" of Chajchir's work. Here's the thing...I was and remain deeply impressed by the professionalism, caring, office/facility and just about everything but the horrible soggy lunch sandwich that was provided to me afterwards at Armani's center. The price was reasonable, the hairline was not overly packed and not lowered from what Dr. Carman did. In fact, this was about creating more density in zones 1, 2 to camouflage the larger mini/micro 80's-style plugs done by Chajchir (who also put everything in at a 90 degree angle to my scalp - f*cking idiot) and filling in the previous donor scars. The doctor that performed my FUE procedure there, Dr. Juan Jauregui, was and has been incredibly kind and professional and in fact has insisted that I come back today and tomorrow so that he can personally wash and inspect the implanted hair and donor site. While the technicians were putting the hairs into the sites he had cut, he personally participated in, supervised and directed the work. Considering all the terrible posts about Alvi Armani's work, I have to say that I'm sorry to let you all down but I'm deeply impressed with the caring and professionalism I have been shown at Alvi Armani Buenos Aires. Ok, that's not to say I'm not as freaked out as anyone that the grafts won't thrive...but I think they will. PS. I'm not a shill...I don't even speak Yiddish. Cheers, JH
