Thank you guys for all the feedback! So I've been researching clinics, and the top 2 I feel most confident in for now are Dr. Bicer in Istanbul and Dr. Carman in San Diego. I'm about to start residency this summer so it'll probably be awhile until I can actually get enough time off to actually get this procedure done, so we'll see where the medications take me til then! Haha thank you! Guess avoiding the sun by spending my entire life indoors does lol. I'm not sure if the nape thinning is from some electrolysis I had done to remove hair on the nape of my neck to clean up the area, or from actual retrograde alopecia. So when I personally look at myself in the mirror, I see some receding hair line and also some diffuse thinning that allows the scalp to come through. Some of my friends are definitely taller than me, or I go to the gym and bend in certain positions which I feel like exposes my scalp to a lot of people, and that makes it somewhat embarrassing, which is one of the reasons as to why I was trying to fill in the crown/scalp. My initial concern with the front was what happens if I transplant hair there, and then the hairline recedes further and now it looks like there's a big gap between what remains of the old hair and the new transplanted hair. That very well could be a misunderstanding of what happens, but I'll try some in person consultations in the near future and go from there. Alright I'll try to focus more on the front and mid-scalp. Is there any reason not to touch the crown? Is that just because of limited donor ability vs area to cover?

How many grafts would I need?

I talked to Marc Dauer in Los Angeles. He stated 1300 to 1400 grafts at $13,000. I was immediately turned off. I sent my pic to Dr Umar as well. He stated 1800-2500 grafts @ 6.50 each. I will be meeting Dr Carman next week. I will also be speaking to Dr. Pak also. -

Thank you for the quick response. Yes I am coming to the same understanding. While in LA, I understand that Dr. Carman, Dr. Mohebi and Dr. Diep are the ones to visit? Thank you very much for your response and for informing me of the possibility of DUPA. I will definitely get this checked. I have already returned to the medical regimen though it would be helpful to meet some good Doctors to understand where my hair is headed towards, especially for the DUPA you pointed out. I look forward to engaging and learning more on this forum. -

Update: Dr. Bisanga - his rep reached out to me saying that they would need an in-person consultation to know for sure that I am a good candidate. Unfortunately, till COVID subsides he won’t be traveling to the US - so bummed that this possibility is basically eliminated. Dr. Panine - rep reached out to me as well, I am going to have a video consultation with the Dr. soon. I asked if the doc would need an in person consult and the assistant said no. Makes me feel a bit wary that two well reputed Dr’s have different opinions (guess that happens?) Eugenix - had a very illuminating call with Dr. Sethi …. Very wise man! Again due to COVID I think traveling to India is not advisable… I am bummed I did not know them 2 yes ago when I was there. Dr Ahmet/ Long Hair Center - I think I am going to rule them out based on what I have heard about them here. Dr. Carman - trying to schedule sometime this week. -

In LA there’s Mohebi, not sure about OC to be honest. In San Diego theres Dr. Carman, who’s highly rated. In SF area there’s Dr. Diep, who was my surgeon.

I can't believe it's been almost 10 years since my first FUT. I was 28, panicking and depressed at what was happening. I went into Dr Meshkin and received 1500 grafts to the front and mid section. Couple of years later at the age of 30, I went back for 1800 more grafts, this time I brought my cousin with me who also had 1800 grafts done. I again added 1500 grafts to the front and mid section and 300 to the crown area. I am now 37 and have been thinking of going back in for a 3rd and final procedure to replace the native hair I've lost since those two procedures and thicken my hairline. This time around I'm going for the FUE as I don't like the scaring the first two procedures left. I've researched Dr Carman in San Diego and may go with him. Anyone have any recommendations for Dr's in Socal? Oh and a funny story. Ive been a pilot since the age of 19. I now fly as a Captain for a major airline in the USA. About 6 years ago, after we landed and parked at the gate, I opened the cockpit door to say goodbye to the passengers as I usually do. As we waited for the gate agent to pull the jetbridge forward, my flight attendant greeted me and said she'd like to introduce me to Dr. Bosley who was sitting in the first row of first class. She said "he is the leading Hair Transplant Dr in the nation". By that point, I had already had my first or second procedure(I can't remember). I shook his hand and my first thought is he looks nothing like the guy on TV. Then he looks up at my head and says out loud, "You'd be a great candidate for a hair transplant". Immediately after, every passenger in the first 10 rows of the airplane looked at my head. Fun Times! :cool:

This patient is a 32 YO male who experienced loss of definition of the frontal hairline which is consistent with his fathers pattern. There is no family history nor clinical evidence in this patient of crown thinning. Two procedures of 1601 FU grafts and 1609 FU grafts were performed a year apart, to obtain the final hairline shape, location and density. Graft design pattern and robust long term growth were achieved from conservative donor strip harvest, leaving the cosmetic quality in the donor virtually unchanged.

Patient is a 26 yo male with MPHL manifested as a loss of frontal hairline, frontal forelock, crown and midscalp density. The hairline was recreated in a conservative design, frontal forelock and midscalp density were addressed, and the crown underwent a conservative reconstruction. Total FU grafts used: 3092. Donor and graft quality preserved using "FUT" excision.

This patient is a 62 YO male who presented with loss of frontal hair line and frontal forelock density. His donor had lower than usual density, about 60 FU/cm2 (vs average of 90 FU/cm2). This resulted in a donor yield of approx 1900 FU grafts. Patient returns at about 15 months post op very happy with the improvement. Note the last photo which demonstrates the typical donor scar from a strip excision when conservative, plastic surgical techniques are employed as standard procedure.

Patient is a 42 yo male who presents with an advanced degree of MPHL. Patient had thinning of the HL/FF/Midscalp and crown early in life with stabilization over last ten years without medications. Total 3757 FU total.

Patient is a male in his late twenties who has been slowly thinning due to Androgenic Alopecia affecting his hairline, frontal forelock, midscalp and bilateral temporal points in a Norwood 5 pattern since his early twenties. He has mild crown involvement as well. Understanding he will continue to have further losses, he understands the need to manage his limited donor in such a fashion that initial transplantation occurs in a limited area which can be addressed in the future without fear that grafts are spread out over too large an area, which would result in a "grafty" appearance later on in life. At one year post-op, the qualitative change in the appearance of his hair in terms of hair shaft caliber, distribution, shine and coverage are greatly improved over the pre-op baseline. The conservative hairline will work well into his 50's and 60's, and precious donor graft reserves have not been squandered on an unnecessarily dense frontal hairline, freeing those reserves for the midscalp and anterior crown as needed as he ages. The last photo illustrates the 1 mm donor scar which should be the normal expected result using conservative, state of the art surgical technique. 2200 Total FU grafts.

Patient is a 33 yo male with very mild recession of the hairline and crown area. His family history is similar on his fathers side, with no hair loss history on his mothers side. Patient has had this condition which has stabilized since about 26 years of age. Our goal was to recreate a very subtle change in his frontal hairline, as well as address the minor thinning in the crown which was a concern to him as well. A total of 1617 FU grafts were obtained via strip excision; 730 one's; 779 two's; and 108 three's. A photo of the donor incision is included to illustrate the type of donor scar which should be most anticipated in the great majority of patients, given the current state of the art techniques we as Hair Transplant Surgeons utilize when performing conservative surgical methods.

Patient is a 37 YO asian male who presents with loss of frontal hair line, frontal forelock, and midscalp/anterior crown thinning. The cornerstone of long term success in hair transplant surgery is understanding the supply/demand inequity which is a factor in over 90% of candidates: there is a limited/finite supply of follicular units available in the patients lifetime, so the artwork design and subsequent placement must respect this overriding concept. This generally translates into higher, more receded/diffuse hairlines and less aggressive crown reconstructions in order to constrict the total area to be transplanted in a pattern that mimics those naturally occurring thinning patterns in nature. Her we see a diffuse, receded hairline created along with good density realized in the midscalp and anterior crown area. This allows for future loss to be successfully addressed as the patient experiences any further progression of his hair loss. Total: 2184; Ones: 849; Two's: 1082; Three's: 253.
