PATIENT'S DETAILS ✓ 36 years old (in 2017), losing hair since the age of 25, with a hair loss history in his family (uncle, father, brother). ✓ No medications. ✓ Thin caliber hair. ✓ Normal follicular units. ✓ High % of hair in telogen phase. RECOMMENDED TREATMENT Here we present yet another interesting case of a complete hair restoration by means of FUE technique. Also in this case, the patient wished to restore all of his balding areas in one single session. However, considering the important surface that needed to be treated, I recommended him to take a two-steps approach in order to ensure the best management of his donor area. The patient has eventually undergone two FUE surgeries : the 1st session (November 2017), consisting of 3654 FUE grafts, aiming at restoring the patient’s hairline, frontal and midscalp sections; the 2nd session (March 2020), 2132 FUE grafts, with the objective of covering the vertex area. Below are shown the results of the first surgery and the immediate pre and post-operative photos of the second session. As usual, we will share the progress and outcome of this second procedure as soon as new material will be available. PATIENT'S DETAILS ✓ 37 years old, losing hair since the age of 18. ✓ Taking Minoxidil 5% (intermittently) since the age of 27. ✓ Medium caliber hair. ✓ Normal follicular units. ✓ High % of hair in telogen phase. RECOMMENDED TREATMENT This 37-year-old gentleman wished to restore the whole of his balding areas in a single session. Given the extended nature of his hair loss, which affected the anterior as well as the posterior area, I suggested him to follow a two-steps approach : a first procedure aiming at harvesting as much grafts as possible in order to restore the anterior and midscalp areas, and a second session focusing on the vertex area. In April 2018, we performed a first FUE procedure consisting of 4261 grafts, including few hundred beard grafts used to increase the midscalp density. The pictures taken at the clinic 6 and 8 months after the surgery already show a drastic change in his look. In January 2019, we carried out the second FUE session - 2065 scalp grafts, in order to cover the vertex. The improvement is apparent also in this case; a total restoration has therefore been performed, as it can be appreciated in the pictures sent afterwards by the patient. Regarding the donor area management, it is interesting to underline the extraction pattern followed during the second FUE session. As it can be seen in the relevant photos, I ended the extractions in a progressive way, getting closer to the recipient area as much as possible. I tend to adopt this approach when patients are undergoing a second (or further) FUE surgery; in fact, this method allows me to avoid the so-called "fringe", a section of scalp between the donor and recipient areas which otherwise would maintain an original density, resembling a halo all around the skull and being often very ungraceful, especially when the patient wears his hair very short. PATIENT'S DETAILS ✓ 52 years old, losing hair since the age of 20, with a family hair loss history (father, brother and paternal grandfather NW V). ✓ Taking Minoxidil and Propecia since several years (even though his hair loss has continued to progress). ✓ Thin hair. ✓ Normal follicular units. ✓ High % of hair in telogen phase. RECOMMENDED TREATMENT This 52-year-old patient underwent a FUT procedure in September 2018, when he received 3428 FUT grafts, with the aim of restoring his frontal and midscalp areas. The pictures taken at the clinic during his six-months and one-year follow-up show very nice and natural results. The patient is currently considering a second procedure in order to cover his vertex, next year (most likely an FUE consisting of approximately 2500 grafts). PATIENT'S DETAILS ✓ 33 years old, Diffuse hair loss pattern (Ludwig II), losing hair since the age of 23, with a hair loss history in his family (father NW VI). ✓ Started taking Minoxidil 5% since 1 month before the surgery. ✓ Medium caliber hair. ✓ Normal follicular units. ✓ Normal % of hair in telogen phase. RECOMMENDED TREATMENT This 33-year-old patient presented a diffuse thinning affecting the whole of his scalp. Despite that, he is a very good candidate for a hair transplant procedure: his hair is slightly curly and his donor area is excellent. Moreover, he had started taking Minoxidil 5% since 3 months before the surgery - even though the medication turned out to be not very effective for the patient. We agreed upon performing an FUT procedure and the patient accepted to shave the whole recipient area, which allowed for a perfect work. In this first session, we decided to focus on the frontal area and midscalp restoration, leaving the vertex untouched at this stage. The procedure was carried out in June 2017, when he received 3556 FUT grafts. As you can see in the pictures below, the patient has experienced a quite fast regrowth, since at the 7th post-op month mark the change in his look was already apparent. The patient will come back for his second session next month of April. His second procedure will consist in a combo FUT + FUE during two consecutive days with the aim of covering the vertex area. RECOMMENDED TREATMENT Here is a nice example showing a frontal area and vertex restoration, done in a single session, consisting of 3545 FUE grafts. While I am usually averse to the idea of grafting the anterior and vertex areas at the same time, while leaving a section of the midscalp untouched, in this specific case I agreed (following a fairly conservative approach). I have given my consent based on several factors : the patient's family hair loss history (just his maternal uncle reached the NW IV stage), the fact that he was under medications to maintain his native hair and that his hair loss was stable since a couple of years already. Moreover, his donor area is very rich and can still provide an important amount of grafts (approximately 3000-3500, not taking into account the potential BHT resources) so that it will be possible to cover the new balding areas in the future, if his hair loss progresses. Below are shown the pre and intra-operative pictures taken at the clinic and the post-operative photos sent in by the patient (1, 3, 6 and 12 months after the procedure).   6. PATIENT'S DETAILS ✓ 37-year-old female, suffering from Androgenic Alopecia since the age of 27. Hair loss stabilized. Her mother and aunt suffer from the same condition. ✓ No medications (side effects from Minoxidil). ✓ Medium hair. ✓ Normal follicular units. RECOMMENDED TREATMENT This 37-year-old female patient came to visit me to discuss about a possible solution for her hairline and temples thinning, caused by Androgenic Alopecia. She had suffered from that condition since the age of 27. She had tried to use Minoxidil in order to improve the situation, but unfortunately the medication caused side effects (hair growing on undesired areas). The loss had nevertheless stabilized since few years already. During the consultation, we therefore agreed upon performing a FUT procedure complemented by a small FUE in order to cherry-pick one hundred softer hairs from the nape area with the aim of restoring the very first hairline and temples. Eventually, the patient received a total of 2136 grafts. The post-operative pictures were sent in by the patient. One year after the surgery, she is very satisfied with her new look : the situation has actually remarkably improved, the yield seems excellent and the results are very natural.   7. PATIENT'S DETAILS ✓ 31 years old, NW IVa, losing hair since the age of 24. Hair loss stabilized since two years. No available data about his family hair loss history. ✓ Started taking Finasteride 1mg after the surgery. Treatment currently suspended. ✓ Medium caliber hair. ✓ Normal follicular units. ✓ High % of hair in telogen phase. RECOMMENDED TREATMENT This 31-year-old patient presented with a NW IVa hair loss degree: an almost completely bald anterior area, a thinning mid-scalp and obvious signals of loss in the vertex area. We agreed upon performing an FUT procedure of approximately 3500 grafts in order to rebuild his hairline and cover his frontal and midscalp areas - leaving the vertex untouched for a potential, second procedure. The proposed hairline, slightly conservative and drawn following the patient's original one, will guarantee a suitable look also in the long-term. In January 2018, the patient eventually received 3757 FUT grafts. The 3rd and 5th post-op month pictures show an early regrowth and already remarkable improvements. During the next 7 moths we expect the results to further improve, especially regarding the hair density and texture. As usual, we'll not hesitate to update this case as soon as new photographic material will be available. Hair loss quite stable since several years. ✓ Taking Minoxidil. ✓ Medium caliber hair. ✓ Normal follicular units. ✓ Low % of hair in telogen phase. RECOMMENDED TREATMENT This 56-year-old patient wished to cover his crown. He underwent his FUT procedure in April 2018, when he received 2515 FUT grafts. As usual during a vertex restoration, I have grafted the adjacent areas as well, so that the results will still be natural if and when the hair loss progresses. The pictures sent by the patient and taken at the clinic during his one-year follow-up show remarkable improvements and a very good result.   9. PATIENT'S DETAILS ✓ 31 years old (in 2016, at the time of his first procedure), with a hair loss history in his family (father and brothers). ✓ Taking Finasteride and Minoxidil. RECOMMENDED TREATMENT This 31-year-old patient wished to restore his beard - which presented a lack of density on the cheeks area - and his hairline, showing a slight recession on the temple corners. In February 2016, we performed a first FUE procedure (2011 scalp and beard grafts) in order to restore his temple corners and frame the outer and lower part of the cheeks. Following the excellent results, the patient decided to further improve his beard by completely covering the cheeks area. Therefore, in December 2017, we performed a second FUE surgery (1232 beard grafts). The patient was again enthusiastic about the results obtained. In April 2019, he decided to undergo a third FUE procedure - consisting of 2889 scalp and pubis grafts, with the aim of further enhancing his hairline/temple corners, restoring the temple points and perfecting the beard density. PATIENT'S DETAILS ✓ 34 years old, NW V, losing hair since the age of 20, with a hair loss history in his family (father NW VII). ✓ Taking Finasteride 1mg/day since the age of 25. ✓ Medium caliber hair. ✓ Normal follicular units. ✓ High % of hair in telogen phase. RECOMMENDED TREATMENT Here we present a nice example of an FUE restoration for a 34-year-old patient with a quite extended hair loss, classifiable as grade V on the Norwood scale. The patient wished to cover the whole of the thinning areas in one single pass, however I suggested to focus on rebuilding the frontal and midscalp areas as a first step, in order to achieve an optimal density on these areas and ensure the best donor zone management in view of future procedures. We therefore proceeded with an FUE surgery (since he didn’t like the idea of having a linear scar), over two consecutive days, during which 3878 grafts have been extracted and implanted. The patient has recently reached the 14th post-operative month and is extremely happy about his results. He’s planning to come back at the end of the current year to complete the restoration by covering the crown area. Since he wished to cover all the thinning areas - other than correct his hairline and anterior area as a priority - also for this patient I felt that a combo FUT + FUE was the best way to proceed in order to maximize his donor resources. In fact, thanks to the combination of the two techniques, I would have been able to correct the existing FUT scars - by including them in the new strip - and enhance the amount of available grafts via the complementary FUE. RECOMMENDED TREATMENT One of the most frequent questions I get asked from people interested in a beard transplant is : "How my beard donor will look after the procedure? How long will it take to recover?" I recently had the opportunity to closely follow-up one of my patients after his beard transplant. I'm glad to share below his photographic documentation as it can be instructive and clarifying in regards to what one can expect both in terms of donor management and actual results. Since always, the patient was bothered from the lack of beard hair, noticeable especially on his cheeks. We therefore decided to perform a FUE procedure, using his same beard as donor area (particularly the area under the chin and the neck). The patient received 1011 single FUE grafts. As can be seen in the photos below, the beard donor healing process was already advanced just 3 days post-op, while 2 weeks after the surgery the signs of the extractions were no longer visible. Concerning the results, the patient is extremely satisfied since we reached his goals just 5 months after the procedure. Generally, the beard transplant presents a main peculiarity : the beard follicle is always constituted by a single thick hair (rarely two). On the contrary, hair follicles can grow naturally in groups of 2, 3, 4 or even more hair. This is one of the main reasons why I always prefer to use beard hair when performing a beard transplant procedure (when possible) : same hair structure, no need for grafts dissection. In fact, the follicles resulting from a strip (FUT) or a FUE (from the scalp donor) must be subdivided in order to obtain one-hair grafts, because only single hair should be used in order to ensure natural results (with the exception of some double grafts that can be used to increase the density throughout the central areas). The beard present also an advantage compared to the hair: the beard hair density is less strong than the density of the scalp. So, it relatively easy to reach an original density in a single session. PATIENT'S DETAILS ✓ 27 years old at the time of the first surgery (Test 2015) ✓ In 2012, he had a previous procedure done in another clinic, 2300 FUE grafts. ✓ Treatments : to be confirmed. ✓ Medium caliber hair. ✓ Fragile follicular units. ✓ Normal % of hair in telogen phase. RECOMMENDED TREATMENT This patient presented for the first time to my office in October 2013, one year and a half after undergoing a 2300 FUE grafts procedure in another clinic. This previous procedure ended up with poor results and, worse still, two weeks after the surgery the patient started to develop a severe shedding due to psoriasis and dermatitis. This condition (never experienced before) affected him to a great extent both physically and emotionally, so much so that he didn't dare to go out without wearing a cap anymore. During this first consultation, I suggested him to start a treatment based on Nizoral and corticosteroids in order to mitigate the shedding. In March 2014, the patient showed important improvements in regards to the skin diseases. However, I suggested him to wait further before performing a hair transplant procedure, since the dermatitis was still present at that time. In February 2015, the patient informed that the symptoms were reduced, nevertheless still evident. Despite that, we agreed upon performing a small test (+/- 100 FUE grafts) in order to check the regrowth rate and if the psoriasis/dermatitis would develop again after the transplant (hence to verify the viability of a more important procedure). In July 2015, the test was performed - 78 FUE grafts implanted in the vertex area. The test proved to be successful. Nevertheless, I suggested him to wait again for a complete healing before performing an important procedure: analysing his evolution, the symptoms were likely to develop quite rapidly and unforeseeably. In December 2015, the patient announced that his skin lesions had completely disappeared. So, in February 2016, we were finally able to perform a first procedure to restore his hairline, frontal area and a part of the midscalp - using 2903 FUE grafts. I also suggested him to continue to wear his hair very short after the procedure, since I found that UV rays had a positive impact on the skin disease. The patient was extremely pleased with the results obtained and decided then to perform a second surgery in order to restore the vertex area. In August 2017, we managed to transplant 1437 FUE grafts (because of his limited donor resources). The last pictures sent by the patient show the current situation, nine months after the second surgery. RECOMMENDED TREATMENT This patient started to lose his eyebrows hair since his childhood as a result of unfortunate family circumstances that provoked very severe stress periods. The loss continued until stabilizing during his adolescence. Unfortunately, by time, the eyebrow hair hasn’t grown back. The patient has therefore found himself in the unpleasant situation visible in the pictures below until 36 years of age, when he gathered the courage to contact me to explain and attempt to correct his difficult situation. We therefore decided to perform an FUE procedure of approximately 500 grafts (taking his scalp hair as donor area) with the aim of rebuilding the eyebrows original shape with a good density. The final grafts count states 510, exclusively single (+/- 250 for each eyebrow). The regrowth of transplanted grafts has been relatively fast and the patient is fully satisfied with the results obtained. In general, the eyebrow transplant uses the same technique as for a classic hair transplant : we graft hair or, when it’s possible, body hair (coming from armpits or the pubis for example). It is mandatory to use exclusively single grafts (containing just one hair) in order to guarantee natural results.   24. This patient is 42 years old. I performed a surgery on July 2014 with the FUT technique. He received 3708 grafts for 7593 hair. Here are the details: 724 FUT1, 2200 FUT2, 667 FUT3 and 117 FUT4. It is a very nice example of a maximal cover after only one intervention. Here are the reasons why : a high number of grafts transferred, curly hair and an important hair diameter. Only a small part of the crown has not been covered.
