Advice for Hair Loss Sufferers Considering Hair Transplant Surgery


My Story

  1. So i probably havent posted here in years. Ive had 2 "small HT's" both around 900- first was like 12 years ago and the other maybe 3 years ago. one in Phoenix, AZ and the other in Philadelphia,PA. Sadly neither doctor told me its imperative i get and stay on a Fin type medicine and I have used Minox off/on for 8 years and tried Oral fin a few years back but with sides i didnt like. Anyways, fast forward to now at 53 yo, and on not only TRT but basically a pretty pharma based hormonals system due to a syndrome called dysautonomia. The short of it is the HT's def helped, but if I dont use Toppik it dont looks so great -and thats according to a wide consensus. My first Ht was the cutting kind off of the left side of head and the 2nd was FUE from the back. I really never felt like they were packing the density of the many successful looking HT's although i most assuredly admit i also couldnt afford a large grafting session at once. So moved to toronto 3 years ago and contacted Dr Rahal's office as they are known as :"The King of Hairline" and I really just want mine ever so slightly evened (less widow peak) and slightly refortified behind my other HT hairs. The Asst i called said it had been too soon from my last HT (bout a year or so) and so i waited another 6 months. After coming in, the guy just gave me nothing but bad news. First showing how I was growing my hair kinda funny to "hide" my temples (trust me and a gust of wind -im well aware!), then saying the donor area looks to sparse and i really dont look like a candidate. Now Im a pragmatist and can handle blunt truth - but this guy had no positive ideas or steps for me to think about and anything i mentioned he poo-poo'ed. Man was i sinking. I then brought up last case resort of SMP and he siad "Yeah...maybe that could " -but the point is he was just really dissuading me from taking any action and just wanting me to basically resign to the situation. Man i left feeling like sh8T. I still knew that with toppik i could get by and even sometimes look pretty good, but surely there had to be SOMETHING to grasp onto! then I looked around reddit and found a Hair Loss group -Boom! the big 3 Minox, Fin/Duta and Micro Needling. YES! Im all in -ive been doing that the past year and half and no its not HT finish but damned if it didnt restore a certain pretty decent looking shape again. Why on earth didnt that guy recommend Anything!? He saw i was desperate. So i emailed him and he said, that Dr Rahal had seen my pics and agreed i wasnt a candidate but if i wanted i could pay $75 to have an in person re-assessment...Ok. Gotcha. This aint about the money to me... heres a question? If i had an FUT from 1 side of my head, then surely there is "some" available from the other...? Not even addressed. I just had to share this for anyone else whos been totally deflated by some of these HT docs or offices -its NOT always the end or final or even a good answer. I wish you all the best.   2. Apologies if this topic has been brought up a lot. I've reached out to several of the top rated surgeons recommended on here (Dr. Gabel, Dr. Rahal and Dr. Konior specifically), and most of their feedback has been relatively consistent. I'm currently at the intermediate phase of what will most likely transition into an advanced hair loss pattern (my dad is completely bald on the scalp, I'd estimate he's a Norwood 6 or even 7) and my mom's father was completely bald on the scalp as well (I never met him, just saw pictures). I've been told my donor availability is limited and that I may not be able to have a second surgery, so I should focus on the frontal and midscalp regions and be prepared to possibly never target the crown. Dr. Konior recommended strip harvesting which I'm not sure I'm comfortable with - I generally wear my hair short, so I don't want a scar in the back of my head. Dr. Rahal actually recommended that I wait 6-12 months while using Minoxidil and topical Finasteride. His idea was that he would like to see stabilization before I make a final decision. But I don't love waiting because I want to get into the dating world and don't love the idea of going through this transition for another 2 years (waiting a year for surgery and then another year to see results). Dr. Gabel indicated that I will have a lot more hair than I have now, but that I'll still be able to see my scalp through my hair. I'm feeling overwhelmed with which surgeon to go with, and whether there are additional surgeons I should be reaching out to or considering for my hair loss.
  3. NW6/7 diffuse 45 yrs patient. I am a patient with a failed ht in the past (in 2018 - a dr not recommended here and anywhere), I also did smp in 2019 after that first failed procedure mentioned back in 2018. When i had the ht back in 2018 i was naive to this, the unhetical Dr back then told me that i did not needed to use medications anymore because my hairloss was “stable” (i used finasteride since i was like 22 years old). This unhetical dr told me he used 3000 graft for my hairline and some on the top but francly i think he only used half of that number of grafts. I did not like the results and went on to an excellent SMP clinic. The first 2-3 years the smp looked great…but since a wasnt in medications i continue to loose hair to the point the smp started to look odd. That was the time i started to worry again about my looks and thankfully found this forum. Sorry for not uploading this thread before…iam very private person…and was afraid…but i own it to the forum. One main reason going to @Dr. Patrick Mwamba was his BHT skills. I knew my donor area was below average…and it was shown during my in-person consultation with Dr Mwamba in Atlanta in march 2022. Also I am 45 so a mature hairline was suggested. The procedure aimed for a density of 60fu/cm2 in hairline and 50fu/cm2 in top and crown including my native hairs + the implanted follicles. Today the crown and top needs more work to do. We decided to do hairline and remaining grafts allover the top/crown and then come back in a second procedure for density in top and crown …(instead of 2 procedures covering only half of top of head and 1 year later the other half). Went to a local trichologist and told me that the density was achieved. Let me add that the communication with the clinic and Dr Mwamba always was and is transparent and concise. The procedure was scheduled to be in New Jersey on september 2022. Because of situations out of our control the Dr himself called me asking if i could have the procedure in Brussels. I already got my plane tickets to NJ….because of this the clinic took care of the new plane tickets and accomodation for 7 days in Brussels. Went to Belgium…the procedure was 4 days…First two days they harvest from scalp, last two days from beard and chest. Have to mention Dr Ali and the two lady technicians where awesome and have been working with DrM like forever (maybe 15-20yrs). I didint even feel the anesthetics. Mwamba made all the incicions and implanted the hairline and temples and mostly all the delicate areas…also he made extractions, punches and was present or available in all the procedure… the exact time each person participate i dont recall well. I just scheduled with clinic going back to Dr Mwamba on March 2024 now in Atlanta for final touch up of crown and top (2000-3000 grafts). Following are the photos…before, during and after. I am at oral finasteride 1mg 5 times a week, the other two days i take oral dutasteride 0.5mg, and oral minoxidil 5mg everyday. I dont comb my hair backwards as most of the photos posted here will be, i comb it to the side as my hair naturally goes to the front… taking photos with hair backward and with harsh lights we can see areas where i need a touchup. I will try to do my best posting this, bear with me. Any feedback and questions will be welcome. Being part of the forum and learning about this condition helped me a lot. Selecting DrM and the results after 12+ months was a great choice for me.   4. I'm here to share my experience, hoping it provides useful information to some. I won't focus on how wonderful everyone is at Eugenix or how well organized everything was, as many have already shared such information. While these aspects are valued and appreciated, at the end of the day, we truly care about the results. I plan on sharing the pre-planning, planning, procedure, and results. If anyone has questions related to traveling from Canada or other non-surgical questions, please feel free to message me; I'm more than happy to help. A little about me: I live in Toronto, Canada, and had two FUT surgeries. I would have classified myself as a Norwood VI. My first FUT was at the age of 35 (11 years ago), and my second FUT at 36 (10 years ago); I'm now 46. I don't think I've lost too much hair in the last 10 years, or if I have, it's been a slow process. My first FUT was, in my opinion, not the best choice and result because I rushed into the procedure without educating myself or spending time on forums like this one. I was told it was 3200 grafts, but they did not provide a graft breakdown, and I did end up with doubles in my hairline, but my scar was okay. I was not on Finasteride at the time as I started it before my second HT. The clinic was called Sure Hair International, and the doctor was Dr. Nelson Ferrara. My second FUT procedure was with Dr. Rahal in Ottawa, Canada, and I received approximately 3500 grafts. This was a major improvement in my opinion. The growth was good, and I could style it to give the appearance of a full head of hair with concealers. This changed my life for the better. My scar is good, except for a few areas where it is wider, but it can still be hidden with a fairly short cut and the right barber. I've come a long way from where I started and am happy, but I would love to make additional improvements and rely less on cosmetics such as Dermmatch and Toppik, so I'm going for it. I am done with FUT, and for my FUE procedure next week, I am going with Dr. Priyadarshini Das from Eugenix. My goals or wish list is to add density to the mid-scalp, address the crown, temple points, and if possible, add grafts into my FUT scar. The scar is a TBD as Dr. Das believes I may have hypertrophic scarring, so she wants to examine the scar before committing to doing anything, which makes sense to me. I've attached pictures of my pre-HT, after HT 1, after HT 2/now, and my scar. I'm excited, yet nervous, and a bit scared. It's a long way from home to a place where you know no one and nothing, but I'm doing it.
  5. Yeah, I would not recommend the Rahal clinic. I had an awful experience with one of the assistants - and their recommendation would have completely ruined my head. I went and saw Dr. Wong from Hasson & Wong in Vancouver and it basically saved me lol Can I ask where you do your micro-needling?
  6. Yea, my hair is fine and I just had a procedure with Dr. Munib three weeks ago. However, my procedure was for the crown (not the hairline), and surprisingly he only used 1750 grafts for a pretty sizeable area (whereas all quotes from other docs I consulted with were in the 2400-4500 range as I am a NW5). It looks great so far in terms of coverage. The guy has great artist skills. I wish I had my procedure with Munib for my hairline though. It's not that my first HT with Rahal was bad. It looks fine, but I think it could have been better. In terms of who I think the best HT docs are...I would say Munib and Zarev are a step above the rest.

  8. Hello Everyone . I'm 38 years old and NW4 , I may need 3500-4000 grafts based on some consultation I had before for my first surgery to get reasonable recovery. I am not able to leave the US , I know DR.Shapiro work is solid and very natural , However his hairline is somehow conservative for my taste , I personally like DR.Rahal hairline more as my reference . I will have in person consultation with Dr.Ron Shapiro next month and here are the questions I have if anyone can help : 1- Is it possible to ask Doctor to give you more aggressive hairline ( If donor is not an issue ) compare to his routine hairline ?! In Another words is it a good idea to have surgery with doctor who have solid work but try to negotiate about hairline/density design ? 2- Between DR.Shapiro and DR.Cooley which doctor have the best hairline/density outcome ( I know they are par and very close but I want to know more details ) Thank You in advance.

He does not use any non-surgical hair loss treatments. His family history includes nominal hair loss on his maternal side and advanced balding patterns on his father's side with higher Norwood classifications. The patient is very happy with his results.   11. Hey @Cyzkm - that's me ^ and you're right, I did get a great result! Better than I even thought I could expect. Seems like I'll be mostly echoing others' advice at this point but here's my piece: 1) I don't have experience with the other doctors - I consulted with Bloxham and Rahal. Dr. Bloxham was a very kind individual and obviously you can read my thread for details on my experience. I would definitely recommend him IF you were dedicated to going FUT (I'm not sure I would go to him for FUE as I just haven't seen enough from him - really hardly any). I chose FUT because I am relatively young, have a strong history of family hairloss, and was already headed into NW4 before age 30. I felt this was my best way to maximize my long term options. 2) I feel, looking at your pictures, that you are not in the same situation as me. You are much better of than I was when I originally consulted Rahal which was back in '16 and almost went through with it but ended up waiting for 7 more years and I'm glad that I did. You're not at a stage yet where it's clear where your hairloss is going or how fast, so I'd strongly recommend to wait a while. If I wasn't looking at multiple large procedures over a lfietime, I'd have gone FUE, so I'd strongly encourage you to wait and see where everything goes before you decide on a procedure type and therefore on a doctor who is great at that procedure. 3) You can find bad results for basically every doctor out there. It's a reality of the procedure (as well as most cosmetic procedures as best I can tell). When you're ready to get the procedure, I'd recommend looking for a doctor who seems to be having a trend of positive results over the last year or two. When I initially talked with Rahal, all his results were outstanding, and then 7 years later when I got back into looking, I was finding a lot of less than stellar results. I think right now Bloxham has mostly positive results, but in a few years who knows? The most important thing is to do your research, look at how their current performance is, accept the potential for a not great outcome but do everything to minimize it, and then make the decision that makes you feel most comfortable.

Over the years it progressed and I started to always wear a hat and became a heavy user of topikk to the point I applied it on my skin where I don’t have hair to have a “lower” hairline. Sometimes it looked decent and sometimes it looked ridiculous but I was so self-conscience about my baldness I preferred to rather look ridiculous than to be seen bald. I felt embareced and even humiliated if someone would see my baldness so I made sure it will never happen. It really affected my confidence and self-worth that’s for sure. Medications I believe that surgery is the last resort so I tried medication and non-surgical treatments both to keep my native hair and slow my hairloss progression as much as possible. Finasteride (oral)- I started using branded finasteride since I was 22 (now I’m 37). It may have slowed my progression but for sure didn’t stop it. I was Norwood 2 when I started taking it and now I’m a Norwood 4. Minoxidil (topical)- I tried topical Minoxidil over the years and stopped numerous times since it was very itchy (especially when I wore a hat at all times) and honestly, I didn’t see much improvement. Minoxidil (Oral)- Tried that for 6 months. Didn’t see much improvement and experienced sides effects such as dizziness (which I never had beforehand), chest pain so I discontinued use. Dutasteride (oral)- Been using it for the last 2 years. Taking it twice weekly based on the advice of Dr. Bisanga. Didn’t see much improvement but I think it slowed further hairloss progression. PRP + Dutosteride injections- tried that 3 times over a course of 3-4 months. Didn’t see much improvement. *Haven’t tried micro-needling. Research- which clinic shall I choose / Why I chose Eugenix - Dr. Sethi I started my research since 2010. I was reading and learning a lot from this forum (as a silent viewer) as well as Spencer kobren “The Bald Truth” talk-show and his association ISHRS, as well as JT and Spex Youtube channels, articles and direct communication with all of them. As I knew more about hairtransplents and botched cases I was more worried and wanted to choose smartly and not out of desperation. I know that even in the best hands there might be failures and doctors are not god. When I looked at results of many of this forum’s doctors, I always found stunning results yet from those same doctors I also found some results I wouldn’t consider great. So that kind of held me back in addition to the financial demand associated with it. Over the years I had online consultations with Hasson & Wong (back when Joe Tilman was working there- yes. That long ago! And also, in the recent year), Dr. Rahal, Dr. Diep, Dr. Ron Shapiro, Dr. Bloxham, Dr. Lorenzo, Dr. Konior and Dr. C*** either with a consultant or the doctor himself. In addition, I personally visited and was physically assessed by the following doctors: Dr. Charles Glen, Dr. Hasson, Dr. Bisanga, Dr. Nusbaum Sr & Dr. Paul Rose and Dr. Sethi. Yea. I did a lot of travelling. For me it was important to know in advance which hairline they are going to offer me before I commit to a surgery. I didn’t want to put myself in a stressful situation where I book a surgery, meet the doc and have him draw a hairline I don’t like. That was a lot of work, time and money to spend but I’m very meticulous and I care too much of how I look and I know anxiety will take place in such an incident so it was all worth it. It’s not the same to have an online consultation and a face-to-face consultation. Some doctors told me contradicting things online and in person. Also, when I compared one doctor recommendation to another – there were also contradictions that can sometimes confuse. Eventually, I decided to choose Dr. Sethi as my surgeon. First, I liked the hairline he suggested the most out of all the doctors I personally met. Second, I didn’t see any other clinic with so many successful Norwood 6/7 cases so me as a Norwood 4 should be an easy case for him. In addition, he introduced me with additional of his patients when I was in his clinic in Mumbai. I saw their results in my eyes. They spoke for themselves. Everyone was so happy and highly recommend of him. In addition, I was told all the celebrities, businessman and politicians in India choose Eugenix. Not to mention the amazing hospitality. I’ll be honest. After my consultation I wanted to go with Dr. Sethi but had doubts. Although I really liked his suggested hairline, he suggested 2500-3500 for my entire scalp (he said my head is small and the designated transplant areas where small) while other doctors I visited suggested 5000. I found it to be good and bad. Good because I get to preserve my donor for the future and it tells me the doc doesn’t want to over quote me and make more money which shows a lot. Bad because I was concerned I would have a low density. After thinking about it, as well as communicating with Joe Tilman I got to the following conclusion- Dr. Sethi is an excellent doctor. This surgery will significantly enhance my look and worst comes worst I can always have additional surgery to increase my density. Also, I get to save and smartly manage my donor area. In addition, I saw some cases where the doc used low number of grafts on a relatively high Norwood cases and the results were amazing. Moreover, the doc showed me other patient in his clinic with a large head and showed me his balding areas which were larger surface compared to mine. So, after clearing my doubts, I was convinced to proceed with Dr. Sethi. In general, I think that one should be critical of any doctor and ask as many questions as possible and understand the rational of their suggestions + cross it with the knowledge you gained in this forum or other sources and that’s the best advise I can provide. Never go to a clinic on a “desperate mode”. Be patient and do your research. I know I was crazy with my research but I was paranoid to have a botched results so it took me a longgggg time and trust me baldness controlled my life. It is a disease of the spirit. At least for me. Consultation with Dr. Sethi I will mention I initially had an online consultation with Dr. Sethi – twice. Then I flew to Mumbai and met him personally. I was thinking it will last max 30-40 min but I ended up staying few hours since I was talking with him, met and talked with several of his happy patients and stuff including technicians, he even showed me the operation room, tools, microscopes and ordered me a launch. The doc and the entire stuff were super nice and the hospitality was so great and welcoming. Most importantly, he drew a hairline I liked with ease. He mentioned he’s not sure he’ll touch my temporal points and that will be decided on the day of the surgery, which I was fine with. Surgery I arrived 2 days before my surgery. Eugenix were kind enough to pay for my hotel for those extra 2 days. I stayed at the Hilton Doubletree Baani Square and was picked up by their driver from the airport. When I arrived, I decided to get a local sim card which costed me 3$ (for 28 days, local calls + 1.5GB internet daily) as I stayed in India 8 days post-surgery (total of 2 weeks) so that I could wear a hat when I fly home as I wouldn’t feel comfortable to fly with a “Karate kid” band on my head and be stared at all the time. No thanks. As for India, you will get to know Indian culture this way or another. The stuff in the hotel and the clinic were super nice. They always want you to feel good and comfortable. Also, I got to experience their food for the first time in my life. I highly recommend Poha and Idli lol. Thank me later… Also, they have a food delivery app called “Zomato” where you can find meals for 10$. I even had noodles meal for 3$ shipping included! Since my payment wasn’t received on the day of my surgery, it was postponed in one day. I got my blood pressure checked, did anesthesia tests and signed some forms. Oh and they have a really nice tea with ginger in their clinic. I highly recommend! 😊 I had a 2 days procedure. Day 1 We started at 4PM and finished around 10PM. Dr. Sethi drew my hairline and decided to address the temporal points as well (which will require additional 1000 grafts). I expressed my desires and brought some pictures when I was a teenage to show him the shape of my hairline but he didn’t need it and I couldn’t ask for a better hairline. Pictures were taken from all angels, later on I was given anesthesia. In some areas it was more painful (it stings) and in other less. The team made sure I’m not in pain and were super nice the whole time. I was given PRP injections and upon that Dr. Sethi entered the room and made the slits. When we finished, I was invited to his house where I had dinner and had some chat with his son (intelligent boy I must say) and the doc. Later I was taken back to my hotel. I didn’t feel any pain whatsoever. Day 2 We started at 7am and finished almost at midnight. I changed my clothes, given anesthesia and the harvesting from my donor (scalp hair only) begun. I was constantly asked if I feel pain and was massaged from time to time by the technicians. On bathroom breaks I was always accompanied by someone. Nice music was played, some nice mashups/remixes and I almost found myself dancing while lying down on the operation chair lol. The hardest part for me was to lay down in the same position for long. Blood flow becomes limited and you start get uncomfortable but I didn’t want to disturb the techs work. In the evening / night time I felt I’m losing my patience and it became harder for me to lay down in certain positions. Also, they changed the anesthesia substance and I felt intense sting sensation at certain points. It was unpleasant to say the least (no pain no gain) but it was short and after that I didn’t feel pain. Graft count was written on a white board and all seemed to be very organized. When the surgery was done, I was told I was a great patient and made the techs work really easy as I was very cooperative. Also, I was told my grafts were very easy to remove from my donor which was music to my ears (no trauma = higher survival rate). On the following day I got the graft count which made me even happier. I have an average of 2.2 hairs per follicule which is great. More density for me. Also, I was told my temporal points should look really good as about 500 grafts were put on each side and it is rare for Dr. Sethi to use that number of grafts in that area. The max he usually does is 300 grafts per each side. At the end of the day, I was given dinner, got the post op instructions and was driven back to my hotel. Post op I decided to stay in India 8 days following my surgery so I’ll be able to wear a hat when I fly back. I absolutely made the right decision as I was able to get my first head wash in the clinic and was able to properly take care of myself as per the clinic post op instructions and address any issues that might occur post op directly with the clinic. On the first and second day post op, I had mild stomach ache from the antibiotics so I visited the clinic and they changed it to a different kind (which solved the problem) and also cleaned my donor area and answered some additional questions I had. Following my surgery, I stayed 2 additional days in my hotel and later booked for another one. After my visit in the clinic post op I should have gone back to the new hotel I booked. However, Dr. Sethi was kind to invite me to stay at Eugenix farm house for the rest of my stay (1 week). It is a house surrounded by enormous pastorale view which was super relaxing and beautiful. I was served with delicious meals 3 times a day and really felt like a king. I wasn’t wearing a hat obviously and it was quite liberating to be hatless in front of people (house crew) and not having the worry to hide it. When I walked outside the house there was a light wind and I haven’t experienced the feeling of a wind on my scalp for more than a decade so that was fun. Also, I didn’t have to worry and put my head down so my topikk fibers won’t come off… One of the workers in the farm house used to work as a technician in Eugenix for 5 years. He told me that his training took 1 year and that Dr. Sethi was very strict and meticulous which I liked to hear as this is how it should be! Post op I didn’t feel any pain. I had some swelling in the following 4-5 days. At certain point I looked like the character from Avatar movie (no joking). Overall, I had a great experience. I’m looking forward for my hair to grow. Ugly duckling phase is ahead of me… Enclosed, you will find my pictures immediately post op and the ones with the marked hairline is from my consultation which took place about 2 months earlier.

  19. I went to Dr. Rahal last year and I'm very happy with my experience and results. I felt perfectly comfortable at the clinic in Toronto. They've taken excellent care of me and their post-op care and responsiveness is outstanding, so I never felt alone or concerned during recovery. 1,100 grafts seems a bit low to me, but you can always decide on more that day, or even down the road if necessary. Achieving similar results with fewer grafts is ideal, certainly, but I was more concerned with needing a second procedure later and wanted to avoid that if at all possible. There's no way to guarantee this, but I personally felt better going with a higher graft count. I can't speak for Dr. Couto, but he seems very popular on these forums and his work generally looks good to me. I'm sure you'd be fine going with either surgeon, but to reach a decision, it may be time to weigh other factors such as cost, convenience (travel, etc.), and your comfort level with each surgeon after speaking with them. I personally try to post one example each week...in fact, i will be presenting another one today. I've been working formally with Dr. Rahal for just over 3 years now and before that, over the last 15 years of being in the hair restoration profession. Dr. Rahal has always impressed me and his nickname "King of Hairlines" is well deserved, in my opinion. That said, I'm always pleased to see one of his patients posting on the forum.   22. I don’t think that Dr. Rahal is underrated on the forum but he’s not talked about as much as he used to be. About 10 years ago (and before) there were many patient posted cases here and then they slowly stopped posting. Rahal is still recommended by the forum and I wish there were more patient posted cases. Most of the cases posted were absolutely amazing!
  23. I'm ~6 months post-op after a 3,000-graft FUE procedure with Dr. Rahal and I'm very happy with my result at this point. I know there is more to come, but I stopped checking for new growth over a month ago. I feel as though I got exactly what I was expecting, if not more. I wondered about this a lot before scheduling my procedure. In my opinion, Dr. Rahal is simply underrated on this forum. Like many other highly-skilled surgeons, he just doesn't have the enthusiastic following that a select few surgeons do. This means he isn't recommended quite as often as I'd personally expect, at least not on this forum. I have to assume that most of his patients come from places other than HRN, and so we don't hear about them here as often. This can also give the sense that a higher percentage of his cases are "disappointments", since unhappy patients are more likely to discover this forum while searching for feedback about their concerns, compared to happy patients (again, amongst those who did not discover Dr. Rahal through the forum to begin with). That's my theory, at least. I don't currently plan to share my full experience or photos publicly on this forum, but if you have any questions or would like to see my progress so far, feel free to message me in private. Note that it's not Dr. Rahal himself, but a representative, who posts here.
