Advice for Hair Loss Sufferers Considering Hair Transplant Surgery

My Story

  1. I am posting this as a review of my experience with Dr. Glenn Charles after I had a FUT transplant with him today (November 2018.) I will be updating my progress throughout the process. In addition, I want to give a brief summary of my hair loss experience and a (kind of) brief review of hair transplants from my perspective as an MD. 1. My history: 28 year old male who started balding in his early twenties. Baldness from both copies of my chromosomes, but my dad's side of the family really have it bad. It really accelerated around age 25 and has been in full force over the past year. I hadn't been on any medications until October, when I started Propecia. I decided a hair transplant was the right decision for me several months ago after over a year of consideration. 2. Hair transplants: I have some experience with the medical field as I finished medical school just over a year ago. Hair transplants weren't a big part of my education, to say the least. Luckily, I have more resources at my disposal than most (peer-reviewed journal articles, chapters from textbooks, etc.) I say this because I am concerned that most people don't have these resources or background to make a truly informed decision. I am by no means an expert, but I believe the following are true for hair transplants and hair transplant surgeons: a) Hair transplants have varying levels of success from mild improvement to moderate improvement (my opinion). Most people with severe hair loss will require several HTs over their lifetime if they have the wrong genetics (my case). The etiology of this variability is multifactorial and beyond the scope of my post. One factor you can control is your choice of surgeon. b) Many cases of dramatic hair loss improvement from a single surgery (i.e. nearly completely bald to a full head of hair) are the result of deceptive omission of information. Some pictures will show before and after pictures in different lighting, different angles, or with the use of thickener. Most importantly, I think that many cases of dramatic improvement are the result of the patient starting Propecia. 5% of patients taking propecia develop dramatic increase in hair density. Nearly one third show moderate increase in hair density and another third show mild increase in hair density. For these reasons, I think before and after photos are vital to your choice of surgeon, but not an end-all. Another factor is technique. c) FUT is an ideal procedure for patients with severe hair loss. Hair transplantation by follicular unit (FUE or FUT) is the gold standard in the industry from many recent journals I have read. FUE seems like a decent option for people who want to have really short haircuts and only have minimal-to-mild hair loss (my opinion). Use of multi-follicular unit grafts was something I had considered. It was the procedure used after "plug grafts," and before the advent of follicular unit transplants in the 90's. Some clinics are using them near the crown, saving single follicular unit grafts for the frontal region. There are sparse reports of this being a better option in a few subsets of patients (blonde haired Caucasians), but nothing that convinced me. Truly there isn't much data on this. My concern, which was shared by Dr. Charles, was that over time these grafts would look "pluggy" and unnatural as the native hair on the crown and frontal region fell out. d) Hair transplantation is a cosmetic plastic surgery procedure. This is not to say that only a plastic surgeon should perform it. It isn't as simple as a Botox injection, but it doesn't require such extensive training and education as a rhinoplasty, breast reduction, etc. This procedure can be done by a skilled physician from a variety of backgrounds (by law). In my opinion, I would like my physician (whether or not they are trained plastic surgeons) to have plenty of experience performing the procedure, preferably years. In addition, I would prefer that hair transplant be the only major procedure the physician performs. Especially (and certainly not exclusively) in the cases in which the physician is not a plastic surgeon, I would hope the physician shows that he has an interest in staying up to date on the latest research in the field. The surgeon should join societies like ISHRS, the ABHRS, or the AHLA in my opinion. 3. My choice: I chose Dr. Charles in part because he fulfills all of these criteria and then some. The guy was president of the American Board of Hair Restoration Surgery. He publishes research articles (though I haven't read them). His photos are all standardized. He is all over these forums posting results and actively engaging members of this community and others. He responds to patients even when they have a suboptimal result. He takes on complex repair cases and in my opinion has pretty decent results. He is fair and conservative in his promises. I am a physician and can tell you that this guy is a true doctor as the profession was meant to be. He calls patients and keeps them totally informed. It isn't a job to him, it is part of who he is. He must love what he does. He could have made plenty of money being a radiologist (with a better lifestyle), but chose this instead. I don't really care if my provider has a stunning personality (Dr. Charles seems thoughtful, compassionate, kind. I wouldn't be surprised if some might think he has a stunning personality.) I was impressed by his professionalism. My experience with his clinic was great. Pleasant staff, minimal pain, speedy procedure. As stated multiple times in this post, these opinions are my own. I would hope that they are adequately educated to be of some use to a person considering hair transplants. I encourage any criticism or additions to these opinions. This leads me to my final point: 4. Disclaimer: Not every hair restoration surgeon can be found on these forums. Many are new and many have no interest in participating in this medium. Not every hair restoration surgeon is part of professional societies. Many of those surgeons probably have great results. Some of those who are part of those societies or who participate in these forums do not have a great track record. In the end, many of the successes (or disasters) of a surgeon can be attributed to technical ability. I just believe that keeping up to date and active in the field are key factors that increase success rates and eliminate disasters. I plan on doing the same in my practice. Finally, I suspect that some members of forums and chatrooms are paid representatives for certain physicians. Given the fact that a physician can be smeared unfairly on forums like these, I can't say I blame them for taking this countermeasure. As a precaution, accept no one piece of information in isolation. Do your research as best you can. I will update the forum with my results.
  2. I updated my profile (my story section?) just now with images of my scalp before the surgery. I will post with the results in a few months. ------------------------------------------------------------------------------------------------------------------------------------------------------------------------------ Spanker- all fair points, and I appreciate you bringing them up. Some clarification: 1. I said that HT doesn't require as much training and education as some other procedures, not that it requires less skill. To perform breast reduction, you have to complete a plastic surgery residency, which is 6 years (other routes require more than this). Then you need to complete a breast fellowship, which is 2 years I think. To perform hair transplants, you don't need to complete a surgical residency. You can do a residency in psychiatry and then perform hair transplants. I admit I am not as familiar with HT certification process, but from what I can tell it is not as extensively regulated. I appreciate any info you have on this. That said... 2. I agree that performing this procedure well is no easy task. I agree that there is an artistic component to it that should not be overlooked or minimized. Perhaps there are only a few masterful surgeons out there performing HTs (I hope you include Dr. Charles in that list). However, complications from this procedure are relatively rare. The main complications are folliculitis and scarring. The surgeon never goes below the dermis. I am not ignoring the fact that almost any procedure can end up in a total disaster. Botox injections can result in death. I am just pointing out that there are more complex procedures than HTs. 3. At 28 I can't really say I have extensive experience in anything, so you have me there lol. I have helped perform several breast reductions, and other smaller plastic surgeries, so I have seen some of what goes into them and what complications can occur. I don't want to give the impression that I believe I am an expert. I have absolutely no experience performing hair transplants and I have tremendous respect for physicians who do this procedure well. 4. I think your point about a Dr's online presence is fair. I would keep in mind that doctors receive no formal training in how to run a business, however. There are a variety of reasons they do not participate on this platform. I think we will have to agree to disagree on this one.
  3. Louisjames, I didn't really consider being worn out. Thanks for your reply, I'll keep that in mind. Dr. Charles, thank you so much for your reply. I'll take your advice into consideration when I'm booking time off of work. Great to hear that I can get back to work within a week!

    Hair Miniturization

Like Dr. Charles said, there's no crystal ball. However, surgeons can examine the miniaturization levels of your hair, and based on your age, rate of hair loss, family history, hair characteristics, donor density, etc. they can make an educated guess on where you may be heading and what the best plan of action is. I'd recommend having several consultations with trusted surgeons and seeing if they come to the same conclusion.
  5. I went for Hair Transplant with Dr.Glenn Charles(Boca Raton) on 7/24. It has been wonderful experience so far. To be honest, I did my part of research before consulting him. And after reading blogs and consulting with few other HT surgeons I felt comfortable going ahead with Dr.Charles. Few things that I would like to share from my experience that : 1. During my first consultation, Dr.Charles didn't up-sell himself and honestly did the assessment. He didn't pushed me to come back to him, which made me fell comfortable. 2. Before HT Dr.Charles Patient Advisior was very helpful and able to answer all my questions /concerns over the phone and on the email. Not even one time I felt like I was being pushed over his staff member. I was very impressed by communication and able to address my concerns in very timely fashion 3. During day for surgery Dr.Glenn explained clearly the game plan and what was going to happen. 4. His surgical staff made me feel comfortable. And there were 5-6 staff members doing the microscope grafting. I believe that makes sure that more grafts survive. Staff was really good, informing constantly on the procedure. 5. Anesthesia part of little painful but i was informed before about it. Once local anesthesia was injected, after that it was relatively less painful. My heart rate went up little when I saw strip from my back on under microscope. I guess it was more my physiological reaction , seeing your own skin laying out. 6. Entire surgery took 6 hrs.. Watched two movies , lunch was provided .. 6. I was able to drive myself back after the op 7. Dr Glenn called me twice personally the same day to follow to make sure I was doing good. It was really nice of him doing that and made me comfortable, that I can reach out to him incase of any emergency or urgent question. 8. As I write this I'm in Day 6 today , it seems like receipt area is healing . Of course I'm little anxious to see how many grafts have survived and have I damaged any so far. I have been taking good precaution like not wearing hat, not going out except for walk in evening with my dog, washing hair with cup blow and gently patting shampoo on grafts .. So far I'm satisfied .. I'll keep ya'll posted on journey.. So far, I can say good work Dr.Charles and his staff. Forgot to mention Dr.Charles only charged me 2000 grafts and gave rest 116 grafts free.. I have been looking at mirror and curiously see how many incisions i have on my head
  6. After a good bit of research, thanks in part to you guys, I decided to go with a transplant with Dr. Glenn Charles in Boca Raton, Florida. The cost was a big part of the decision for me, and he was the most reasonable of the real doctors (i.e., not Bosley) in the Palm Beach area. After meeting with him, I felt very comfortable. He was no-BS, didn't promise me anything, and generally made me feel very good about my prospects. He decided that 2200 grafts would be a good start for me now, and I decided to do the procedure less than 2 weeks later (there happened to be an opening). On procedure day, I showed up and was pretty quickly in the chair ready to go. The chair was comfy and there was a TV set up right in front of me. The staff had set it to morning Nickelodeon, though of course I could have changed it or put in a DVD had I wanted. As is normally the case, the only pain I felt through the whole procedure was the lidocaine injection, but that was minor. After that, Dr. Charles removed the strip, sutured it up, and made the recipient area incisions. After I had a nice sub for lunch and the techs had dissected out the grafts, they dropped the grafts in as I tuned in to some old Walker Texas Ranger reruns. Man, they're bad. So the procedure was easy, and the recovery has been mostly easy. My donor site has been seemingly extra tender, and it made for crappy sleeping for almost a week; I really couldn't find a good sleeping position. However, I went to work the day after the procedure and left work early only on the second day because I could not stay awake. Other than that, it's generally been easy and the donor site pain is fading. Yesterday, on day 10, I went in for suture removal. That was easy, and he gave me the go ahead to start getting back to normal (his early instructions would allowed me to a couple of days ago, but I figured I'd wait until I saw him first). Last night I splashed on some baby oil and removed the scabs from the recipient area. I'm ready to go out without a hat for the first time, though my head is still a little red. I have posted some pre-procedure photos. I figured there's no much to show right now, but I'll keep you guys posted. Dr. Charles did a great procedure and made me feel very comfortable throughout. Hopefully the results will be just as good.
  7. I just had an excellent Transplant experience today 05-04-15 with Dr. Glenn Charles in Boca Raton, Fl. He transplanted 2560 grafts to my hairline all the way back to my crown. I had a previous transplant with another Dr. (who is no longer practicing), 11 years ago. 2200 grafts. Back then it looked really full but over time I continued to experience hair loss which made for a much thinner look. The hair line was nicely done and it just kept thinning out. Alas, it was time to remedy this. I did a lot of research and found that there were so many positives regarding Dr. Charles and his team I had to contact him and make an appointment. Dr. Charles and I discussed the best approach to my issue and I must say that he explained all avenues of approach in detail so I could make the best decision for me. I experienced very little pain and enjoyed Dr. Charles and his professional team. They were all very engaged with me on a personal level making me feel welcomed and very much at ease. His office and operating room were exceptionally well maintained and very clean. I would highly recommend that if you are looking for an excellent surgeon with an exceptional staff, make an appointment with Dr. Charles. His pricing is exceptional for what you receive. I will add my before pictures and the post surgery pictures within the next day or two.
  8. Hi guys, I am 28 years old guy with frontal and temporal recession, started loosing hair 3 years back and wanted to stay ahead of the game. I have very thick and long hair and was able to hide my baldness any how by combing forward but didn't want to struggle anymore. Did my research for almost an year and talked to several physicians but finally choose Dr. Glenn Charles because of his good reputation and quality of work. I must tell he is a great man and will explain you every aspect of it, i was initially given an estimate of 2000-2300 grafts as per my expectations but on the day of surgery he suggested me to go for 2000 grafts to get optimal results (I was under constant communication with him before my Surgery day). He explained me why i can't i get it done as per my expectation as it would look very unnatural and kind of female hairline. He did a dense packing in the marked area as you can view my attached pics. The procedure went very fine, although i was very sacred of the anesthesia procedure. The staff was very friendly and was talking to me all the time. I am healing fine and its just 10th day, sometimes i get little discomfort due to stitches but its OK, I recommend you to get a neck pillow for few days for sleeping. Please look at my pics and comment on the quality of work done by Dr. Glenn Charles 2000 grafts FUT Its a waiting game for me now and will keep you guys posted. I do recommend Dr. Glenn Charles for the procedure
  9. Hello guys, I just signed up on this forum but I’ve been an observer for a while now. I finally pulled a trigger and I am going for my first procedure with Dr. Glenn Charles at his Boca Raton location. I visited him over 6 months ago but I’ve been in touch with his office staff over emails. Once I decided to go for the surgery, I had few chat with their office staff to go over the procedure. They said I don’t have to visit the office for any prior consultation and just show up at the day of the surgery. They emailed me information that I need to follow pre and post procedure. My question to this forum is that is there anything else I need to either ask or do before and on the day of my surgery? I’ve heard in the past that certain doctors like to see your old pictures before hair loss to make sure the new hairline is natural. I was not asked to bring in any old pictures. Does anyone else think going for 2500 graft is good idea or I should do more or less?

Dr. Glenn Charles is the best

Hello everyone, I wanted to share with everyone my experience with Dr. Glenn Charles (Boca Raton, FL). I am a 31 year old periodontist in Vero Beach, Fl. I have been losing hair on the crown of my head for 6 years and I have been thinning on the top of my head for the past 3-4 years. As a periodontist I treat people surgically on a daily basis. I know what to expect for my patients but this procedure was relatively unknown to me. I did, however, become very comfortable with Dr. Charles' office after my first conversation with Alexia (front office) as well as telephone conference with Dr. Charles. My biggest concern was predictability and intra-op/post-op discomfort. I cannot comment on the success of the procedure because I had the procedure completed yesterday (4/8/16). I am writing mainly to describe my surgical experience and post-op pain. This is a FIRST CLASS office. Patient comfort is their #1 priority and Dr. Charles did not initiate any treatment until confirming that I was 100% comfortable. They offered me a valium to relax me prior to beginning (I had to have a driver to drop off and pick up) and administed local anesthesia throughout the procedure. He was constantly confirming that the area was numb. Being a surgeon myself, I know how important this is. The grafting procedure took approximately 5 hours and I received 2250 grafts. His staff seemed incredibly well trained and we never once encountered a hiccup. The day after I can confirm that post-op pain is at a minimal, no need for narcotics to control any pain and I plan on returning to work on Monday (3 days post-op). I have a very strict level of expectation given my occupation and Dr. Gleen Charles, and his staff, exceded those. I have shared photos from before my procedure and immediatly following my procedure.
  11. I figured I would post the before and after pictures of my procedure for the community. I thought about having a hair transplant for a long time and Dr. Charles and I stayed very conservative with our approach, but I'm still expecting great results in a few months. I had a very receded hairline, and if you look at my Propecia album from 10 years ago you'll see it has always been extremely receded but has since stabilized thanks to Propecia. And yes, the penis works just fine. I thought about getting this procedure since I was 26 (currently 35), so it is bizarre to finally be on the other side of having a hair transplant. The procedure was fairly painless. I didn't mind the anesthesia at all, but the last hour of the surgery became slightly uncomfortable because the donor area was tender and I was still on my back as they were inserting the final grafts. Apart from the first two nights where I definitely needed the painkillers to lay down on the donor area, recovery was a breeze. I had another doctor, who works with a plastic surgeon in Jacksonville, take out the sutures and both she and my hair stylist( who is from Miami and has seen a lot of bad transplants) believe Dr. Charles did an exceptional job with the donor area. It's honestly a pain in the ass to attempt to find the donor area to show family and friends-- I believe that is a very good problem to have. I have nothing but good things to say about Dr. Charles and his staff and I hope the final results match the quality of my procedure. My first contact with their office was in 2011, so to say that they were patient and took time to answer all my questions is an understatement. Overall, the procedure was fun! People back home cannot believe how clean and minimal the procedure appeared when I drove back to Jacksonville the following morning. I am not going to update every month, but I will update at month 4 or 5 as the results begin to manifest. My fingers are crossed that I am a fast grower, but I've read enough of these threads to know to trust my doctor at this point and enjoy the recovery and growth, even if my growth is on the slower side of life. I added more pictures, but they did not show up for some reason. If anyone is interested, they are in my photo album. Videos of my procedure can also be viewed at drglenncharles on his Instagram page if you are interested. I look pretty dumb, but if it helps you it's worth it If anyone in Florida is considering a hair transplant and has any questions please feel free to message to me.

    Dr Lupanzula FUT

Hi @Dr. Glenn Charles Thanks for your reply. Dr Lupanzula does offer both. I don't have fine hair and my donor has about 9,000 grafts with FUE according to Dr Lupanzula in an in-person consultation. I want FUT to get the most out of my donor. I don't care about the scar and will get FUE put into it anyway. I have several other in-person consultations this month with the other main Belgian doctors and Dr Feriduni.
  13. Melvin-Moderator

    My transplant :) 6500gr.

Dr. Charles what is your view on getting a strip surgery after extensive FUE? Do you feel that the grafts will be difficult to dissect due to sub dermal scarring. Which is best to perform first FUSS or FUE and why?
  14. I am looking to go to one of these "approved" doctors for a first time appointment, assessment and to see what they recommend for medication, if surgery is necessary etc. I live in Orlando so the drive to either one is about the same, so distance isn't a factor. Looking for recommendations from those of you who have been here enough to have seen others reviews, or those with personal experience good or bad from either of these two. Thanks gents!
  15. I definitely agree that one side seems higher than the other and could see how that could drive you crazy. The biggest concern is that the doctor seems to be saying that it would put existing grafts at risk? I don't see how that's the case, as people do two-day procedures all the time when they require more grafts than can be done in a single day. I also don't see how the swelling alone could cause such a different visual appearance in the height/placement of the hairline. As Dr. Charles said, a little asymmetry can be a good thing, as you don't want anything to be too "perfect". Not only that, but even doing a couple hundred extra grafts is going to be quite the ordeal for you (going back under, etc.), but if you're really not satisfied with this, I think you should really push for the doctor to at least discuss this more with you and see if there's something you two can work out. Ultimately, you're right in that if you're going to have to address this at some point, you might as well get it over with.
  16. I'm not sure how good mine will look but I posted. I think it will be fine. There is always a possibility that I need a second one. However, posting seems like a good defense mechanism. Publicity favors the patient. So, I never understood why more people do not post. I can tell you when researching Dr. Charles that the majority of patients I talked to didn't even know sites like these existed. Most of the world does not research things like crazy! A wealthy person will go in to see a surgeon, the surgeon tells him what he wants to hear, he schedules and has the transplant. I went with Dr. Charles because I spoke to a few consultants at OTHER CLINICS who told me I was in good hands with him. We shall see. He certainly didn't butcher me and my friend who is a plastic surgeon here thinks both my donor area (FUT) and recipient area look amazing. I'm really curious how dense my transplanted areas will be. Time will tell. I also wanted a surgeon within driving distance in case there are problems that arise. The thought of going to Turkey or taking a 6-8 hour flight somewhere horrifies me. I can literally go beat down an office door if I need to, But I could actually just call Dr. Charles on his cell phone. I'm also beginning to believe different people respond differently to transplants. NW3s always seem to have the best results. People with thick hair seem to gain insane coverage, etc. Also, I think bad transplants are far more likely to be posted than good ones. People who do no research are blissfully unaware that they should be thankful for good results.

Dr. Glenn Charles is the best

Surprised no one has commented on this but I would love to see your final results. I just recently had a procedure with Dr. Charles.
  18. I had my procedure with Dr. Charles a month ago--2052 graphs. So I'm looking forward to seeing some results in a few more months.
  19. I actually do not have any pictures of my scar. I attempted to take a picture for you but I can't seem to find it. I had a different plastic surgeon in Jax remove my sutures since Dr. Charles office is four hours away and she thought he did an excellent job. I can still feel the internal sutures as I don't think they are completely dissolved yet. It feels like there is a raised section of my head where the scar is, but I'm struggling to photograph this area. I can barely get my friends and family to see the scar when I show it to them. In a few months, when I update the rest of the pictures I'll get someone to run a comb through my hair so we can get a better idea of the quality of the scar. I hate to say this, but it does seem like I'll get close to that "pencil thin" quality scar that people claim does not happen.

2052 grafts to the temporal areas with Dr. Glenn Charles.
  21. Well if you had an in person consultation with Dr. Glenn Charles, and he said you have no miniaturization in the donor zone than you should be good to go. However, you may want to look at other surgeons if Dr. Erdogan does not want to take on your case. I see that he’s told you no twice, at this point it’s best to look elsewhere. Most start off taking 1mg per week whilst phasing out finasteride. Of course specific dosages is something you’ll want to discuss with your doctor.
  22. Thanks for the reply's I just had a local inperson consult with Dr. Glenn Charles here in Boca Raton Florida not even a week ago and he didn't see any miniaturization donor hair issues he looked though my head with the magnifying glasses and said my donor area looks strong and good to go Thinking maybe were over analyzing things a little with this demarcation line separations? Hair loss comes in all shapes and sizes some have more and some have a less distinct separation or as you said demarcation line separating the donor zone from the recipient zones I'm 59yrs old been loosing my hair very slowly since my late 20s its finally getting to the point where its becoming more noticeable and difficult to camouflage thinking I want to get something done while I"m young enough to enjoy it The Dutasteride is maybe not a bad idea whats the typical dose people are taking of the stuff ?
  23. Dr. Charles is quite knowledgeable and does good work!
