My experience using the FUE hair transplant technique ??“ Dr De Reys - Prohairclinic I was introduced in the field of FUE hair transplantation back in 2005 by Bart, manager and co owner of Prohairclinic. He setup training sessions with Dr. Ilter who had been doing FUE since 2003 and has become one of the worlds leading FUE experts. Since 2005 I have been performing FUE exclusively on an almost daily basis. Instruments and setup. After performing about 10 FUE sessions using a surgical steel 1 mm punch I quickly moved to 0,7-0,8 mm instruments. I find that I am able to extract intact grafts with smaller instruments without increased transection rate. Since FUE is about minimal damage to the tissue I discard the use of any instrument larger than 0,8 mm for the extractions. During the extraction process I make use of a 5,5* magnification loupes. I prefer a sitting position behind the patient, scoring the grafts. I do not make use of any mechanical driven device to help the scoring process. A mechanical device would distort the finger feeling that is needed to ensure quality extractions. In fact, no special tools need to be invented to execute an FUE procedure without problems, all it needs is freely available on the market today. The punches used are of the shelve, available to any drs who wishes to use them. I select the punch size based on the patients graft/hair thickness. Since the patients pays for hairs to be transplanted we take great care to select the best grafts in the donor area. Therefore a graft must contain 1 up to 4 hairs, in rare cases 5 hairs. Our average hair/graft is 2,3, for non hairline work it can go up to 2,8 hairs per graft. The procedure After selecting the right size instrument it takes a few trial scores to determine the angle and depth. The scoring is performed in a twisting motion with virtually no downward pressure. The rotation speed is preventing the grafts to adhere to the punch and suffer from any twisting motion. This has been verified under microscopes by ourselves and other surgeons. The sharpness of the punch is a critical choice in respect to the patients tissue characteristics. Sometimes a sharp punch is needed, sometimes a blunt and most of the time something in between. Depending on the patient skin characteristics more saline is used to harden the treated area, in other times less to no saline is used. Furthermore I use finger pressure if needed to stabilize the scoring area. A correctly scored graft does not require any real traction force that could harm a graft, they literally almost pop out. It is unnecessary to use brutal force of any kind to properly extract viable grafts. Mind that hair follicles are very strong tissues and can withstand great forces. Most major damage is caused by improper handling grafts outside the body. FUE grafts have virtually no protective fat tissue. Therefore extreme moisturizing protocols are used to keep them from drying out. Another factor is squeezing the grafts to death. The solution to this problem are basically : limit grafts handling to an absolute minimum, thorough training for the technicians and wearing magnification at all times. The key issue however is that with experience a certain FUE fingertip feeling is developed. In my opinion this special feeling counts for 70 percent of a well executed FUE extraction with virtually no transection. It is scientifically not possible to explain how this finger control is felt. I guess it is like learning how to drive a bike. At first the gravitational forces are uncomprehending. After a while most can drive a bike, but some people become experts and drive like mad on any terrain without falling. Donor area limits The limit of the donor area has been shown in a document by Petermac and dr. Bisanga. It is possible to harvest about 25% of the donor grafts available without having to harvest adjacent grafts. If we go higher than this limit we inform the patient that at extremely short hair length some visible aspect of FUE extractions may be visible. By now we have several patient where 4000+ grafts were harvested without causing visible thinning at 5 mm hair length. Patient selection FUE as practiced by myself can be offered ANY patient who is a hair transplant candidate, regardless of hair/skin type characteristics. FUE Mega sessions We believe that it is better to have less but larger treatments for the benefit of the patients. It means less time off and faster results. However it takes an HIGHLY trained team to accomplish FUE mega sessions. It is not just the drs that is responsible for a well executed FUE, it is a team effort. I am glad to have worked with my unchanged team for many years now. Our routines have been perfected over the years. My extraction speed of 400-600 grafts per hours is achievable with a transection rate of less than 5% (usually about 1 percent on most patients). Therefore we can perform 2500 FUE grafts in one day on non transplanted patients. FUE on previously strip operated patient In many cases it is difficult to extract grafts from the area surrounding the strip scar. This is caused by underlying tension that shifted the grafts natural position. In most cases reducing the extraction speed and compensate for the angle/depth changes is sufficient to solve the problem. In some cases however it is extreme underlying scar tissue that makes it virtually impossible to extract those graft. FUE on previously FUE operated patient In general there are no problems going back to harvest more graft if the punch size used was 0.8 mm or less. The existing scaring does not interfere with the new extractions, nor has there been angle changes or underlying scars for the new grafts to be scored. Additional information. Our mega sessions are build upon our experienced and routines and do not exceed 12 hours. We have disclosed our technique and instruments for anyone who is interested and will continue to do. FUE is not a technique that is learned in a matter of weeks. FUE is best performed on a daily basis since it is the only way to get enough experience to handle each and every patient characteristics. We are aware some disagree with us, but if you look further you will notice that they either have limited skill, limited endurance, or are not practicing FUE on a daily basis. We understand that is very difficult for non FUE experts to handle every FUE case. We urge surgeons who run into trouble with FUE to resort to FUT. It makes no sense to needlessly wreck a patients donor area as bad FUE can be as devastating as bad strip surgery.

A few of you guys have also met him either in London or NYC at the meets. Here's his write up/research/info. Gasman: Heres my story... I've roamed these boards for a few years now, and it's proven to be a very valuable source of information. I'm 31 years old and first noticed my hair receding at the early age of 18. My hair seemed to slow or even stop receding in my late twenties. I take 1mg Proscar daily to help maintain what I have, however it was never going to repair what I had already lost. I first considered Dr Cole as I was impressed with FIT/FUE technique. I then came across Spex and met up with him about 2 years ago. After yet more research I considered other doctors like Bisanga, Woods and even our very own UK Dr Rogers. After much deliberation, I decided to have a HT with Dr. Feller in New York on December 11th 2007. On the day of the HT I arrived early at Dr Fellers with my girlfriend at 7.30am. I had some photos taken by the Doctor and a new hairline drawn, a conservative hairline which is what I wanted. Dr Feller then arranged a taxi for my girlfriend to go shopping for the day which is what I would recommend for any partner coming along. No point in them hanging around all day. I wanted her there at the beginning to see the hairline drawn in but didn't want to bore her by sitting in the waiting room all day.... I then proceeded to a room to have a section of my hair buzzed down ready for the FUE to be done. After that it was time for the injections. Not sure how many, I think about 15 or so to numb the area.... These are painful but the pain only hurts for a a second as the needle first goes in. The hardest bit I found was laying face down on a massage table. I had to lay still on this table for the morning while Feller made the FUE holes. I had a few rest breaks in between, and luckily for me Feller said I was a good FUE patient. Once we had a good number of FUE's we stopped for lunch (a lovely ham sandwich from the deli). The afternoon started with all the grafts now cleaned, trimmed and counted. A total of 1032 grafts some 3's, 2,s and lots of singles for the front hairline. I then sat in a dentist style chair (much more comfortable than the mornings table I was on). Dr Feller then made all the tiny incisions in my head, and once he had finished the techs started straight placing the grafts in my head... Three techs in total, one on each side of my head working, with one assisting behind my head who kept the areas clean with lots of saline. Once Feller was happy with the techs work, I had a few more photos, some post HT instructions. The day I had been planning for 3 years was finished by 3.30pm. I then went back to the hotel (The Andrew) and relaxed that evening watching tv and ordering room service. I stayed in Great neck for a further 2 days to relax and recover before returning home to the UK. I have to say I now feel great and its like a huge weight lifted off my shoulders... Now its the waiting game! I just want to say a thankyou to Dr Feller and his staff. The day went smoothly and I'm now glad that I did all my research as I was confident I was in good hands.   4. I'd like to have a strip HT procedure, 2000 grafts about (implantation in front-line and on top). I've not yet decided. I'd like to stay in Europe (I live in Italy). Who can help me to choose between HDC (Cyprus) and Bisanga (Brussels) ? I see that their prices are similar, which of them can offer to me the higher quality ?   5. For FUE -- I suggest consulting Dr. Bisanga, Dr. Mwamba and Dr. Ilter(prohair clinic) if you are staying in Europe. Any of these docs will tell you if FUE is right or wrong for you. Take Care, Jason
  6. I've seen Dr. Devroye's skills and technique at many of the surgery workshops and he does very nice work. Dr. Devroye is extemely conscientious of his work and is compassionate with his patients. I've seen handful of intra-op and post op pics from HDC and they look very good. I've heard good things about Dr. Bisanga but have not seen any results yet. Consult with them all and you should get a sense of which doc is best for you. Best of luck with your research.

I am 34yr old who has spent the last 3 years lurking on forums on different sites.I have now decided the time is ripe to put my scalp under the needles (no knife, I do not want a strip). I am what Dr. Mennetrey refer as Norwood 5. My hairloss seems to have drawn to a closure and my father has pretty much the same hairloss. I am looking for doctors who can perform fue. Preferably in Europe, but I am not averse to travel. Money is a constraint but I realize quality comes first. Before you ask, I have tried Rogaine and propecia and the sides put me off. Plus, did not get much benefit either. The doctors I have shortlisted from different sites (based on results and consistency) Dr. Cole, Dr. Woods, Dr.Jones, and Dr. Bisanga. Please feel free to add to the doctor list. I especially look fwd to reading other patient's experiences about these doctors. Thanks in advance. Greg.

  12. blessed is the correct word since I am a practicing christian man. Bro I prayed for along time!!! I prayed to god to lead me to the right doc who was an expert in his field and a giving Doctor thats not all about the $$$$$. One of the reasons I chose him is because he has helped children in far worse situations than us in hairtransplants. These kids where 6 to 8 years old. I knew he had a heart and cared about his patients. God gives man wisdom and direction if your willing to pray,wait and listen. He was the only Ht doctor that I consulted with. I did try to talk to feller but his office did not get back to me and I was also praying on which guy to maybe go with. I try to walk in faith and I looked at it as God shutting down a door because Dr. Epstein was the man for me,plus God knew that he was going to give me some freebies. God places desires in peoples Heart and so be it he did it through Dr. Epstein. Dr. Epstein did not have to listen to that little voice that said give the kid some extras. He was listening also. Got to love that quality in people. I do it all the time in my business and give great deals in Real Estate. What goes around comes around and im sure Dr. Epstein is just lining himself up for more of Gods Blessings,because he is a giver and you cannot out give God.
  13. Christian Bisanga worked with Dr. Rose several years ago assisting with surgery. Dr. Rose was complimentary about his skills and, from what I've seen and heard, Chris has a nice little practice going now.

  15. Hi, i think he is doing quality work, but not all the time. When i research, i found the most unhappy patients in europe from Bisanga. If there are any complaints after your surgery - u re still alone. So i have choosen to go to Dr.Heitmann in netherland. nevertheless there are some good results of Bisanga but the aftercare was important for me, too ! good luck !
  16. Graff, From what I have seen from Dr. Bisanga, he appears to be doing quality work. I know that when Pat was traveling to Europe that he was hoping to meet up with and observe a live surgery of Dr. Bisanga, but unfortunately, he was in the states at the time so it didn't work out. I hope to contact him sometime over the next few months and learn more about his practice. Be sure to research thoroughly before taking the plunge.   17. Kyle Christian, If I remember correctly, Dr. Hasson and Dr. Wong do have an office in Las Vegas but only do surgeries in Canada. In regards to Propecia I have not experienced sides from it and the dozens who use it say the same.

  19. Wow, I was just talking to someone about Dr. Devroye 2 min. ago. I'm familiar with his work and he's tops. I know others are familiar with Dr. Bisanga, but I have not seen his results to comment one way or the other.
  20. Hi hairdude, B Spot is an experienced forum member and also consults for Dr. Shapiro. He recommended above that if you are looking into FUE/CIT/FIT, then begin researching Dr. Feller, Dr. Wolf, Dr. Harris,Dr. Rose and Dr. Bisanga. These guys all use less than 1mm extraction tools which helps minimise donor scarring. Further, they have a track record of good results with this technique along with solid ethics. I believe that what Dr. Cole seems to be alluding to in his email is than his new CIT technique avoids transecting the follicules. However, if you choose any of the aforementioned Drs. they will all produce high graft survival of likely > 95%. From your pics it seems you will just require hairline work. This will require mostly single hair grafts with perhaps some 2's in behind. So the reference to splitting grafts should not be relevant here sice they should all be singles. Again, any of the above Drs will give you good results. You need to continue to research and become comfortable with each Dr. Use this forum along with others to understand each Drs. reputations and profiles. This will help you make a more educated decision. good luck.
  21. You have to be careful when choosing a doc, especially with fue. Fit is simply the extraction method coupled with proprietary tools created by Dr.'s Rose and Cole--- As Bill alluded to, Dr. Cole has now renamed his procedure CIT-- which I have not seen. You need to be careful when choosing a fue doc--there are those who will over-quote you grafts and there are those who will use larger extraction tools to make the process easier. Please consider my advice and continue researching your options-- Dr. Feller, Dr. Wolf, Dr. Harris,Dr. Rose and Dr. Bisanga would be good choices to start.   24. You guys are great! This topic has had me laughing from the first post to the last post. Certainly I would not want the title of "Dr." as it is deceiving. Nor would I want "God" in my title, because as a Christian, I will continue to worship the one God in three distinct persons! I really like Folicular Grand Wizard - it has a nice ring to it! And as other forum members like Mrjb (who is almost at 2000 incredible posts by the way), we can add the word "club" to the title which still flows nicely! Besides, I really need other members up here with me - it's lonely out here in celestial orbit. Funny stuff! Bill

I am a three time hair transplant patient who has done a lot of research over the last three years about hair, hair growth, hair loss, hair transplantation, and hair loss treatments. I have been in numerous discussions with expert hair restoration physicians, hair restoration consultants, and hair loss patients. I am also in the process of reading "Hair Transplantation", a book edited by Dr. Walter Unger and Dr. Ron Shaprio. Does this make me an expert? No. Am I knowledgeable about hair and hair restoration? Yes. You should always do dilligent research and consult with a medical expert to confirm anything you are told. My advice should not be taken as medical advice. I work as an IT Manager and for the Hair Transplant network as the Associate Publisher/Moderator of this community. I have a degree in Elementary Education and 3 classes short of a masters degree in Christian Counseling. I agree with wanthairs. 50-60% growth yield is terrible. In the hands of a first-rate hair restoration physician, FUT (strip surgery) yield has been said to be around 95%. In the hands of a first-rate hair restoration physician, FUE (Follicular Unit Extraction) yield has been said to be around 90% (the risk of follicle transection is greater, and placement of the hair graft is more difficult because of the lesser amount of skin cells protecting the graft - increasing the risk of damaging the hair graft).
