Advice for Hair Loss Sufferers Considering Hair Transplant Surgery

  1. First emperor I want to thank u on a great right up. I too agree that your results are less then optimum. This gives prospective patients and idea that less the perfect results are possible. Potential ht patients should weigh the pros and cons of this procedure. Now you have to ask yourself is this result an outlier of the bell curve or not? Typically, you see far better results posted on this forum from H&W. It seems to be that no top doc is immune from producing less than stellar results. Since you are worried about a second strip procedure, I would research fue. It seems that Dr. Bisanga has a good reputation here and might be able to help. Although I would tread lightly on this. Before you commit to 2000 grafts, I would do a test patch maybe of 500 just to see the outcome. Obviously, you want to be careful with your donor hence the 500grafts. I know you're worried about downtime but conserving your donor should you choose a second procedure is vital in your situation. I wish you best of luck?
  2. Stingray, While Dr. Bisanga hasn't been prescreened by this community, he appears to be doing very solid work. I hope you'll share your experience and chart your progress with photos. Best of luck in your upcoming procedure. Bill
  3. Hey Ray...glad your going in to get your situation resolved mate. Your going to achieve your results with Dr. Bisanga. You did your research and I believe chose a top FUE clinic to get the results you want. I hope your able to continue to counsel others as they go through their journey and continue to share yours with us. Can't wait! Jason
  4. you can find many hair transplant surgeons that are experienced and qualified (promotional link removed) i would personally recommend bhr's dr. bisanga
  5. folliclediabolical, I too have been very impressed with everything Coalition member Dr. Feriduni has presented on this community. He's also been visited and observed in live surgery by Patrick Hennessey, publisher of the Hair Transplant Network. Pat was very impressed with Dr. Feriduni's ultra refined technique during his visit. See highlights from Pat's visit with surgical photos. I too like what I've seen from Dr. Bisanga however, it's a shame that he still hasn't accepted our invitation to present examples of his work on our forum to be considered for potential recommendation. Good luck in selecting a quality physician that impresses you the most. Best wishes, Bill
  6. You should look at Dr. Bisanga as well. He does both methods, does not push one over the other. Take your time Balboa....your going to make a proper decision mate....then you can say "Adrian, We did it!!!!" Jason
  7. hmm, never even heard of that doctor Really? Dr. Bisanga runs the BHR clinic in Belgium. Does very nice work, clear presentation as well. If your considering beard hair I would certainly check him out. I have not seen any of his beard hair results however, but there are just not a lot of them floating around. Jason
  8. Yeah, Dr. Bisanga seems to be doing some realistic beard hair transplants. 800 is about the max I could expect...I do have a fairly dense beard, but my beard hair is only slightly more coarse than my scalp donor so in my case, I think it will look good spread into my existing work...I would not want a whole section of beard hair however. Jason
  9. Rollins, there is no debate..there is a dearth of great FUE docs. However, please look at Dr. Feller, Dr. Harris, Dr. Bisanga, Dr. DeVroye Dr. Mwamba, HDC to start. At 39, your options are probably wide open if your only a NW2...but if you want to keep your hair short and you need 2000 grafts or less, FUE seems like a proper choice. Take Care, Jason
  10. Hi Guys, To be straight with you I wouldn't recommend any in Australia at the moment and that's after consulting, being in contact, seeing results first hand. Obviously as you should know, Dr Barry White should be avoided at all costs and in my opinion, your best bet (at this current time) would be to look in Europe or Nth America. I know Sunnycoast is looking into Dr. Path and like many I believe he does solid work, but I am not completely sold on him yet. Doctor's to look at in my opinion would be H&W (Strip), Shapiro (Strip), Bisanga (FUE and Strip) and a few others.. Hope it helps, if you need more info. Holler me
  11. Lost, I have talked to many many people about the exact points you have raised. I think what happened is you have Dr. Shapiro with his reputation for excellence and his online transparency collided when we began our FUE process. Another point is aside from Dr. Feller (and more recently a few others) there has always been a "shroud of mystery" associated with others concerning FUE---clinics refuse to disclose punch sizes, refuse to acknowledge poor results, over-hype the benefits and downplay the potential drawbacks. Also, from the beginning we acknowledged our infancy in the FUE arena and we readily offered our support of excellent FUE docs such as Dr. Feller, Dr. Bisanga and Dr. Harris to patients seeking FUE, but were uncomfortable choosing SMG due to lack of results. I think people just look at what we do and they trust us to "do right" by them. We talk about what we do and why. People can see the immediate post-op work and they can see how minimally invasive we keep the procedure. I think your right on point that people are looking at the past work we performed and feel with Dr. Shapiro and our staff, the results will be the same. Again, when I go back to mid 2007 and we started to really set the groundwork one of the fail-safes we talked about was NOT to overwhelm our staff, over-book, over-promise and under-deliver. I have to admit, I am excited about our FUE work, but your 100% right..we should NOT be considered FUE experts...until we have the results to back it up. I believe the guys who are members here who are going to share their experiences will help as well. Take Care, Jason
  12. It already has. SMG placed their FUE at $6 per graft (with a travel discount of $500 I believe), Dr. Devroye is $7.25 (with the exchange rate), Dr. Feriduni is $5.77 (same), Bisanga is around that price (same), Dr. Wolf is $7, Dr. Umar is $6. If the dollar would ever get stronger than the Euro, FUE in Europe would be cheaper than strip surgery in the US. The days of $10 per graft FUE are on their way out. With top notch clinics offering the same surgery for 40% less, the clinics still charging those exorbitant fees will either have to adjust their prices accordingly, or lose business. You gotta love SMG for putting the patient ahead of the bottom line. We all know that they could have set their FUE price much higher than $6, but didn't so that they could offer more patients, more options. That's admirable.
  13. "" Dr. Bisanga has adapted and improved his FUE punch technique over the years, one obvious example is the size of punch he uses has dropped in diameter to minimise traumatic impact on the scalp and surrounding follicular units. Another aspect is the technical ability and objective technique, the ability to feel the follicular unit and surrounding tissue when entering the scalp and gauging the depth and angles of each FU. During my FUE sessions I was always aware of the changing positions Dr. Bisanga used to make, moving himself and adapting to changes in the angles and directions of the FU. Now watching FUE punching from the other side I have seen more. FUE has and will always be technically a very difficult technique, the fact that each FU has to be removed individually and the average op is 1000 grafts plus obviously multiplies the room for error as each extraction can be different in some cases. Dependent on the area of the scalp, the angle the FU exits the scalp changes, dependent on the area it can be easier to extract the FU as the lay of the FU can be more consistent and also the position of the head when resting on the operating chair allows for easier extraction. For example the back of the head, around the occipital area is much easier to punch. It is important the doc can understand his position needs to change otherwise there will be a greater scarring impact and potentially increased transection. The constant need to change position is not something that can be avoided regardless of tool size or the type of tool, this is about the technique itself. Pressure control is also important to achieving minimum transaction and the least potential trauma to the surrounding FU. A largely forgotten aspect to FUE is the effects on the surrounding FU that are left in the donor. A FUE donor maybe be spread over the entire donor safe zone and if used well can leave minimal to almost no signs hair has been removed, but incorrect punch and extraction can greatly reduce the potential for future FUE and the graft numbers and even hinder Strip extraction. The greater the friction created when the punch enters the skin the harder to balance the direction of the hair with any potential changes as there becomes less sensitivity between the fingers and the tip of the punch. The tension can be micro but Dr. Bisanga has found through changing his technique that even manually the tension can be reduced and this improves graft survival and collateral damage. The finger to punch pressure helps gauge the skin characteristics and even possible to feel the FU with practice. With this technique of punching Dr. Bisanga has minimised transaction to as little as 1-2% in most cases and even on demanding cases rarely increases to above 5%; this is only made possible by the touch sense gained through minimal transference of distance and resistance between his fingers and the punch. The next aspect that is over looked much of the time is miniaturisation existing in the donor and the potential to create miniaturisation. Dr. Bisanga feels that anything near 20% miniaturisation in the donor can rule you out from surgery, certainly having FUE. Miniaturisation does not affect the density of the donor hair as such because that is just the calculation of FU per cm2 but it will affect the number of FU that can be removed. We have seen miniaturisation spread all over the donor and also just in isolated areas, either way it will reduce the numbers that can be extracted. By definition the miniaturised hair cannot be used so they must remain in the donor, this creates the problem of how close to the miniaturised FU can the extraction take place. Too close and the thickness of coverage will drop and could potentially traumatise the miniaturised hair and cause it to fall out. So, the extraction pattern has to become wider thus less FU per cm2 are removed to retain the integrity of the donor, especially for the future. Miniaturisation can also be caused with the extraction, going back to the tension or resistance in the punch movement, this can create a ripple effect, like a 'skin quake', the punch hole being the epicentre and the vibrations moving out from the central point. As above Dr. Bisanga has minimised aspects of this by his punch motion technique, he has certainly found less resistance was caused creating obviously less contortion of the wound and the surrounding Follicular Units. This is very important, we have seen miniaturisation created post FUE surgery that has possibly rendered the areas non- harvestable due sometimes to over harvesting but also peripheral damage creating miniaturisation. Dr. Bisanga's feeling on these problems is to lessen the motion when making the punch action, the least friction or motion allows for greater sensitivity and feel between punch and fingers and this in turn has reduced transection, decreased peripheral damage either to the skin or surrounding FU with changing directions as well as miniaturisation. With the many documented FUE cases he has to his name as well as the many more he has performed and internally documented he feels confident to say that in respect of physical technique he has reduced these negative factors a great deal but then research is obviously ongoing." 14. No prob, hairadv....there is only the notion of lower FUE yield because the surgery was hijacked by hacks (Armani, e.g.) and they don't have the skills and wherewithall to produce good yield when they perform. FUE is being un-hijacked by the likes of Dr. Feller, Dr. Shapiro, Dr. Devroye, etc. So, I wouldn't agree that FUE has "worse" yield; certainly, on the larger sessions it likely would, but any of the doctors I just mentioned would not be performing this on you. In the right hands FUE can and likely will produce excellent yield, comparable to strip. I'm just a layman who has spent a good chunk of a few years researching HTs and immersing myself in the field from a non-professional perspective. Don't take what I say as gospel; very little of what I say I would even want to be viewed as such. I mainly intend to propose my own perspective that I myself would honestly follow in your shoes, which you can then cross-examine with your experience and experiences with others -- specifically, that of esteemed doctors, who I would strongly recommend you consult with. Dr. Feller, Dr. Ron Shapiro, Dr. Devroye, and Dr. Bisanga would certainly constitute an example array of "esteemed doctors" whose opinions I would def want.
  15. Bill, I applaud Dr. Bisanga for stepping up to the plate and doing the right thing. But, I still would like to know why the gap was left immediately post op? Could you please get an answer to that question?
  16. Dear members, I just spoke with Phil from Dr. Bisanga's office and he confirmed that he and Luca have made contact privately and are working to resolve his concerns. Dr. Bisanga has agreed to put in the extra grafts he needs free of charge. Ultimately, even the best surgeons won't bat a thousand with perfect results. However, it's overall consistency and how patient concerns are handled that makes a clinic great. In my opinion, BHR clinic has done very well at working to help this patient to help him meet his hair restoration goals. Also, due to Dr. Bisanga working to resolve the patient's concerns amicably, Luca's overall experience with Dr. Bisanga can no longer be considered "bad", even if he wasn't happy with his initial results. Thus, to make this thread more fair and unbiased, I've removed the word "bad" from the forum title. All the Best, Bill
  17. I've never heard of him, which is something of a red-flag. Hopefully, others can comment + or - on him. But, to be frank, not finding much negative -- or positive -- to say about a clinic is a pretty huge negative in and of itself. You want reliability and consistency of excellence with a clinic possessing a strong, transparent background. FUE is also such a delicate procedure, which has a lot of cool attributes to it, but as a result has garnered a lot of attention from hacks and greedy doctors who hawk it while having little clue what they're doing (other than pillaging patients' donors and wallets). I would still say to continue looking into Dr. Manzanares, but I would *strongly* encourage you also look into: Feller, R. Shapiro, Bisanga, Devroye. You should have a simple road investigating into their methods, trackrecord, and results, while being treated professionally and honestly.
  18. Luca, You are at a crossroads here and ultimately you have the power to decide what to do next. You and BHR have both posted and on this and at least one other forum regarding your case. But continuing to post your dissatisfaction isn't going to change the way your hair looks. You and BHR can share your side of the story until you are both blue in the face. But it all comes down to this... BHR is offering to help you. In my opinion, consider letting them resolve your concerns. However, if you choose not to, considering consulting other respected surgeons who can help you. Also, if you haven't already, please make sure Dr. Bisanga has been given your full name, date of surgery, and granted full permission to discuss your case online. Best wishes, Bill
  19. BHR Clinic, I took it upon myself to do a bit of research into this case on some European forums, as I was very interested in Dr. Bisanga's work. As you stated correctly, Luca has gone on many forums and posted about his displeasure regarding his transplant. On one of the sites, you responded to his posting by saying that Dr. Bisanga recommended 3,000 grafts, but that the patient capped Dr. Bisanga at 2,600, and that he received 2,536. Implicit in the post was the belief that Luca was partially to blame for the ineffectiveness of his his transplant because he did not comply with the doctor's recommendation. I have some questions about that, and some others regarding this particular result. 1. 400 grafts should hardly make THAT much of difference in the outcome of a transplant, so why even mention the fact that the the patient did not follow the doctors recommendation? The only reason I can see then, is that it is an attempt to absolve the physician of blame by implying that if the patient had followed Dr. Bisanga's recommendation, he would not have had the problem. 2. I'm not quite sure what went down between the patient and the clinic, but if the patient could only afford 2,600 (as he mentioned on another forum), but Dr. Bisanga really felt that 3,000 was the only acceptable number, why not either give 400 grafts for free, or refuse surgery? 3. Regardless of anything else, why weren't the grafts placed throughout the thinning area? On the other site, whomever it was that was representing BHR stated that - "He was told that even though the grafts were placed into the existing hair (pictures can be shown that prove this) Dr. Bisanga would have preferred to place even further back. It was explained that when placing in and around miniaturised hair there can be a contrast in quality between the two hair types, and it was said the shorter the hair sometimes the more obvious the difference." - That's not true. On the site where that post comes from, and on this site, Luca posted immediate post op photos that show that the grafts were NOT placed far back enough. When he left his surgery, he had a gap. Now that his hair has grown out, the gap is very noticeable. I would be very upset if I were him too. 4. Why not use a higher hairline to compensate for the fact that the patient could not, or would not use 3,000 grafts? 5. The area was a relatively small one. Why didn't Dr. Bisanga just go thinner throughout the area and not leave the gap? We have all seen many doctors use 2,600 grafts for much larger areas, and yet still achieve cosmetically pleasing results.
  20. Dear Luca, We have never closed the door to you, we are as always happy to see you in person and extended the offer to help you when we see you but I am afraid you have never replied in anyway that wanted to really resolve this; you have decided to make a campaingn against us on many forums instead of coming to see us and talking openly and honestly and to see what we will do. If you want nothing from us and wish to persist in defaming the name of Dr. Bisanga there is little we can do for you personally as you have obviously shown a great intention in your actions; I have lost count how many forums you have posted on and all under different usernames. You are not a million miles from us, if you wish to have a personal consultation with the doctor, sit and discuss properly then we can all move on I hope, unless that is your intentions are not to resolve your concerns with us and in preference wish to continue with your present actions. Frustration is understandable but when we have never shut the door on you I do not understand your actions, if we had said no to talking or meeting for sure, but that has never been the case, and although with your actions there is one intention we are still happy to see you with the only intention of resolving your concerns, no different to anything we have ever said to you in the past. We have no wish to see an unhappy patient, and not for reasons you may think of sheer bad publicity but because we are in this industry to try to help and improve on people's life; if you really want this to be objectively resolved for you then I suggest you would come and see us when it is mutually good for both. Either email us or you were emailed earlier today with a phone number you can call and discuss moving on. Regards,
  21. Luca, I have been in touch with Phil from Dr. Bisanga's clinic and I expect he'll be fashioning a reply to your concerns shortly. In the meantime, I do see from other posts on our discussion forum that they have been in touch with you encouraging you to go the distance and wait until your one year anniversary before making a final judgement on the result. You may not like this answer, but this is standard protocol from every clinic. The gap in the frontal area can likely be easily fixed with a small under of grafts. Are you saying they're refusing to correct this for you? That's not the impression I got from Phil. In fact, he claims you've refused their help...is that true? Members of our community are welcome to share their genuine experience, opinions, and photos on our forum. However, we do expect patients to contact their doctors and be sincere in trying to resolve their concerns. Additionally, we give clinics the right to reply to patient concerns on our forum. This may require you to provide the clinic with your full name, date of the procedure, and verbal or written permission to discuss your case online. To see how we handle these types of concerns, visit "Maintaining a Fair and Safe Environment on our Forum for Patients and Physicians. Since your clinic has expressed the willingness to work with you, I expect that you'll work with the clinic to resolve your concerns. Best Regards, Bill
  22. That is a fantastic result. I have been researching HT's for 3 years now and there are only 4 doctors that I would let touch my head. Dr. Bisanga is one of them.
  23. It's true that in the UK there is Farjo and then there is everyone else -- and you don't want the latter. Belgium has a lotta options, indeed. I think Dr. Devroye is extremely impressive, and I have a lot of faith in his work and his ethics as a doctor. The more you see about him and his clinic the more impressed you get. If you decide to go the route of FUE I would still certainly consider Dr. Devroye, too; at the same time, I would absolutely research Bisanga for FUE. Unfortunately, he basically has never been involved in this forum, and like the one time his name popped up it was that peculiar situation with a patient showing a "gap" in his HT. And unfortunately once again, no new information or explanation was ever shed on things. If you consult with Dr. Bisanga I'd bring this up. But, I have seen more FUE work from Bisanga's than most anyone else (not just the stuff he shows on his site but on other forums) and it's pretty remarkable. The only other Dr. of whom I've seen as much quality FUE work from is Dr. Feller in the U.S. And for pretty much everything TC said about Prohair (as well as lackluster results), I wouldn't seriously consider them.
  24. Dr. Feriduni, Dr. Devroye, and Dr. Farjo are all outstanding. I would choose the one that is the least expensive and closest. I'm not so sure about Dr. Bisanga. Much of his work looks impressive, but a patient of his posted a result on this site a month or so ago that was very troubling. Dr. Bisanga left a noticeable gap between the transplanted and native hairs. It wasn't that the patient lost native hair and a gap was created, Dr. Bisanga left the gap. Whether it was conscious desire, or mere inadvertence, the fact remains that the surgery was unsuccessful. I know nothing about Dr. Mwamba. As for Prohair, they make some very bold statements on their website that are unsupportable. One example is "No risks, no complications, no visible scarring." There are risks with FUE, there can be complications, and there is visible scarring. Any clinic that makes false statements like those is immediately scratched off of my list.
  25. Look into Bisanga, too, for FUE. Unfortunately, he has a very low (if not non-existent) presence on this forum; but I would sincerely recommend looking into him. I think TC gave you a very good list of people to consider and consult with. You are essentially a "strip candidate", yet you aren't because you simply won't go a route other than FUE. Thus, a very large FUE session is what you want; the problem here, of course, is that getting a lot of grafts in a single FUE session isn't the simplest thing, and there is a ton of hype in this realm. So, where am I going with this.....I would give particular investigation into both who is *willing* to do a larger FUE session for you, and who has the *actual* capabilities (re: yield). My impression is that: DeVroye does FUE sessions upwards of ~2500 (this is from something I remember him saying); Dr. Feller ~1800-2000 (this is from what I've actually seen); and SMG on a slightly smaller scale, ~1000-1500 (this is just from what I've anecdotally observed). But ya, the only docs who I've seen megaession-FUE from that rivals strip pound4pound is from Bisanga and also Dr. Mikelas from HDC Cyprus. I personally wouldn't go to them tomorrow, so I wouldn't recommend you do either; but, in your circumstance, I would absolutely investigate in that direction.
