Advice for Hair Loss Sufferers Considering Hair Transplant Surgery


My Story

  1. FUE performed on a 29 year old Caucasian male with: - NW II beginning III Class N - Donor density of 68-72FU/cm2 in the temporal region and 80-88 FU/cm2 in the occipital region - Hair diameter of ~46-56 µm Treatment plan: A Follicular Unit Extraction with 1600-1800 FU in high density to restore the entire hairline Performed procedure (September 15th, 2014): A Follicular Unit Extraction procedure with 1811 FU: 376 single hair FU 983 double hair FU 452 triple hair FU Extraction performed with a 0.85mm FUE punch with CIT Manual Punch Handle. No microscopic preparation of the follicular unit grafts (FUE); Parallel incisions (single, double and triple units) were made in custom-sized blades technique (~Cutting Edge blades of 0.75mm – 0.85mm) Dr. Bijan Feriduni

  3. SUMMARY 4105 graft FUE performed over two days (About two weeks ago, 14th and 15th July) to restore my hairline and reinforce the mid-scalp and vertex. Dr. Erdogan strongly recommended staying on Finasteride to maintain density in the vertex, and that is my plan for the foreseeable future. Day 1: • 2000 grafts placed in hairline (density of 50 FU/cm2 over an area of 40cm2) Day 2: • 1200 grafts placed in mid-scalp (density of 24 FU/cm2 over an area of 50cm2) • 900 grafts placed in vertex (density of 18 FU/cm2 over an area of 50cm2) So you can see what my hair was like, with receding hairline and loss of density throughout: And here is a photo they took during extraction where you can see the three recipient areas marked (hairline, mid-scalp, and vertex) MY BACKGROUND I am 31 years old. Hair loss started around 20 or 21, and I started on Finasteride by 22. I have taken it consistently since then, for awhile doing 1.25mg/day. For awhile I was dissolving Proscar in alcohol to get a solution and I experimented with lower doses (as low as 0.2mg once every three days. I think that dosage was less effective for me, though I can’t say I studied it rigorously. In the weeks before my FUE I ramped back up to 1.25 mg/day, and plan to stay at this dose for at least the next 15 months as I understood this time after the FUE to be critical. After that point I might try again to lower my dose, though probably not as much as before. CHOOSING ASMED Advice on this forum was very useful; ASMED stood out for quality and price-competitiveness. I had two online consultations done: • Dr. Feriduni quoted 2500-3500 grafts for the hairline and mid-scalp • Dr. Erdogan quoted 3400-3800 grafts for the hairline and mid-scalp Both clinics have great reputations and undoubtedly do great work, at this point the price difference was too much to ignore. Making the decision even easier was that ASMED could schedule me in at a perfect date so I confirmed with ASMED. COMMUNICATION AND LOGISTICS My emails to the clinic were all handled by Sema, who would be my point of contact before, during, and after the procedure. She was by my side during the procedure to take care of any need, translate when needing to communicate with the technicians, etc. The arrival was quite easy: the ASMED driver picked me up at the airport and drove me straight to the clinic. Depending on when you arrive, you might have your consultation immediately, or maybe not until the next day. I arrived at 7pm so no consultation that day. Instead, once I was in the ASMED car the driver had me speak by phone with Sema so that she could ask me what food I wanted for dinner. As you may know, you have two options for accommodation: the clinic itself, and the nearby Radisson Hotel. I recommend the following: • For the nights before, during, and one night after the procedure I strongly recommend staying in the clinic. I did this, and was so glad. After sitting in the same position for hours, having thousands of holes poked in your scalp, what a relief it is to just go up a few floors and be able to lie on a hospital bed that you can recline perfectly. I was exhausted and wouldn’t want to imagine having to get into a car (even for just the five minute drive) and go into a hotel, call room service, etc. It is just far more comfortable and safe to be right in the clinic during those few critical days. I wish I took some photos to show you guys, but the room in the clinic looks basically like a nice business hotel room (Starwood, Hilton, etc.), but with a hospital bed instead of a hotel bed. You have breakfast and lunch downstairs, and dinner they will order whatever you want and bring it to your room. Every afternoon they bring you a nice healthy snack of things like watermelon, hazelnuts, etc. • The day after the surgery I went down from my room to the clinic for removal of the bandage on the donor area and hair-washing. I then packed up my things and they dropped me off at the Radisson, where I stayed for 3-4 nights. It was nice to be in a hotel, so I felt a little bit more in the real world. While here I basically just laid around doing post-op care (dealing with swelling, spraying ATP every hour), reading, and ordering room service. • Once my swelling was totally gone and I felt I had a bit more energy, I checked out of the Radisson and moved into an Airbnb on the European side of the city. This I did because I wanted to stick around for about 10 days before flying. While on the point of logistics, the recent terror attack and coup attempt in Turkey might give some of you second thoughts but honestly I don’t think you should worry. The clinic is on the Asian side of the city, quite far away from anything touristic or that could be considered a target. The coup attempt happened the night after my procedure was finished and I slept through everything (other than being woken up in the middle of the night by loudspeaker announcements to loyalists to take to the streets) CONSULTATION This consisted of some photos, basic medical tests, analysis of my follicles, and also blasting my face and hair with some laser that apparently helps them make calculations for the hairline. Then I met with Dr. Koray Erdogan, who was really very friendly. We discussed a few things such as: • My use of Finasteride, and he asked if I was willing to continue with it. He was quite glad to hear that I was planning to stick with it. • My vertex. From the online consultation based on the photos I sent, he thought that my loss in the vertex was miniaturization and could be controlled with Finasteride. But in person he saw that part of the loss there was actual hair loss so he asked if I might be alright going above the 3400-3800 graft quotation to do 4100 grafts, in order to transplant some there. • My expectations, and plan for future loss. In looking at my photos and asking me about family hairloss, Dr. Erdogan suggested that I might be progressing towards Norwood VI eventually. He calculated a donor capacity of 8100 grafts so we briefly discussed what a future procedure might look like. Here I wish I had pushed a bit more for calculations on surface area and follicle density, so that I could understand in a more quantified way what a second procedure in years to come might look like. PROCEDURE Here is where some people criticize ASMED as Dr. Erdogan is not personally involved in every step. As you may have read elsewhere, his role is to plan the transplant, design the hairline, and make the incisions. You may be uncomfortable that the doctor does not do the extractions or transplantation, but the clinic’s results speak for themselves. For the extraction, they used 0.7mm punches for the parietal/side and 0.9mm for the occipital/rear. I had planned to pay the EUR300 for the optional PRP treatment but they threw it in for me. After first day of procedure: After both days were finished: Donor area after both days: RECOVERY Day 1 after procedure: They gave me some Advil for pain but I took my last Advil this morning and didn’t use any after then. Swelling starts to appear by the end of the day in the forehead. Sleeping on the neck pillow sucks, especially with the donor area still raw and a bit sore. Days 2-4: swelling moves down my face, and I pass through distinct phases, looking first like some random alien in a low-budget sci-fi with a bulbous head, and then later on looking like some brute who’s had his face bashed in at the pub a few too many times. During this time I figured out the neck pillow would be more comfortable if I have it barely inflated, so that the central part where my head rests has no air in it, but there is air in the sides, so that in case my head turns sideways it gets stopped and cushioned. Days 5-8: Swelling is gone; at this time thousands of tiny scabs fall off the donor wounds. Some scabs in the recipient area start falling off. Day 5 photos: Day 9: The recipient scabs all feel loose so today while shampooing I rubbed them all off. Day 9 (before removing scabs): Day 10: Day 17: Day 21: Now in the ugly duckling phase as shedding has started but redness has yet to really fade. They told me my skin is a bit sensitive so my redness is a bit worse than would be typical. And the high density implanted into the hairline makes it worse. 4 weeks out: Really in ugly duckling phase now as most of the transplanted hairs have fallen out. Luckily the redness seems to have started to fade. I tried to manage lighting conditions in these photos but I have moved over the past week so I couldn't keep things identical. (will happen again in a few weeks as I'm just in an airbnb while I search for a permanent apartment). And now here I am popping biotin and MSM to try to get these hairs to sprout up a little more quickly (in addition to Finasteride of course). I have to admit to being a bit nervous, worrying about sunburn from 90 minutes spent outside even though I was wearing a hat, etc. I suppose the best thing to do at this stage is to try to not overthink things, and just be patient. I know this was long, but hopefully it helps some of you. I have a huge debt to this forum from the many, many detailed posts here so I’m hoping that my experience can give someone else here the confidence to go for it and do it!

  6. Dear forum members, I want to give you a small update on my progress at 8.5 months. I am thrilled with the development so far! My hairline has taken a great shape, and I am delighted with the hairline design and its naturalness. I am very thankful for the great and detailed work of Dr. Feriduni and his team. I am happy I followed his advice regarding the overall design and the naturalness of the growth is what strikes most. Regarding the very smooth experience, I cannot recommend him highly enough.   7. Hi! I have been reading more about it on the forum but I am still not entirely sure what it is and/or how it works. I am in my early 30s and would like to put off surgery as long as I possibly can, as Dr. Feriduni has recommended to me. I have thought of a few questions then. 1) What is it? The info I find online is mostly marketing/promotional stuff so I am not going to believe that. 2) Are there brands? If so, which are the best/worst? 3) Is this a skill in itself i.e. are there doctors that are better in it than others? 4) Side effects? 5) Is it a long-term solution? Any assistance would be much appreciated!
  8. Hi there, just thought i could piggyback off this thread. Thank you for sharing and this treasure trove on information, some of which can be slightly overwhelming! I'm currently based in the U.K. and my main consideration at this point is deciding who to go with... geography, cost and timing all play a slight factor in my overall consideration but quality is obviously paramount. You obviously speak highly of Dr. Erdogan and Asmed clinic and also of Dr. Yaman (who i havent heard much of till now), but is this relative to Turkey alone? where do we rank some of the best Turkish hair transplant doctors along the best in the world? The great disparity of quality and experiences in Turkey leaves me a bit apprehensive and I am trying to keep an open mind as to ensure I am not missing out on great treatment based on my prejudice. I'm fairly new to the forums but have been reading a bit and i understand even leading clinics in Belguim and the US/Canada will utilise technicians for placement of the grafts, but am I correct in saying that the reason Turkey is so much cheaper is in part due to the technicians carrying out the graft harvesting and placement? (with some doctors placing the recipient incisions) at the moment I am leaning towards Belguim, possibly Dr. Lupanzula (who i have a consultation with soon), Feriduni, Bisanga, or Devroye. I can't say I have much faith in the U.K. based clinics, following some consultation visits at Da Vinci clinic and Dr. Mays clinic, both fell short of what I was hoping for.

  10. Hey Speegs, Thanks for the great comments! Yes I do agree that I'm somewhere in between Norwood II and Norwood III I appreciate the comments about finasteride and minoxidil, I will definitely discuss those options with the doctor going forward and might try them again for a longer period than 6 months and see if anything works differently (better). I do agree that prevention is key. But we have to find what works individually, the right type of product. We're all different and not everyone has the same results, as we all know. I will definitely try more stuff from now on, to see what makes a positive and lasting impact on my hair loss. Including laser therapy. I appreciate your opinion on my eligibility for surgical hair restoration, but I really don't think Dr. Bisanga would accept me as a good candidate if he didn't genuinely think I was one, just like he was very honest about FUE being the (only) sane option for me vs FUT, even though I wanted FUT initially. He's a pretty honest guy. And I also got feedback from other reputable doctors - like Dr. Feriduni and Dr. Devroye - who told me I am eligible for another procedure, besides all the research I've done watching videos from patients who've done several procedures sharing their experiences and advice, exchanging e-mails with them, reading a ton of articles, etc. I think I've made a great decision I feel very positive about it.   11. Hi David! Thanks so much for your feedback. The Dr. I consulted with is Doctor Christian Bisanga. I consulted with 3 other doctors, but only online, they didn't get to feel my scalp in person so obviously they couldn't say "scalp is too tight, we rule out FUT" Also, besides this doctor, the other one I'd want to do my procedure would be Feriduni, but he has a 6 month waiting period just for a consultation in person and over 1 year of waiting period to have the surgery itself... so that one is out of the picture, I don't want to wait a year and a half to have the procedure done. I'm not working at the moment, so this is the perfect timing to have it done, as I can give it a few months while the hair grows back. Also, since I still have a lot of hair, I think it's wiser to do the procedure sooner rather than later, as I am sick of styling my hair for 30 minutes every day to give it a fuller look every day, or having the same haircut that is more favorable to my receding hairline. Yet I am in doubt. Funny how our mind works! lol I've written to the doc and shall see what I get back.
  12. Hi All, I am a 27yo male and have been suffering from hair loss for over 10 years now. I have very fine hard to maintain hair and they have never looked good all my life. Now, I am finally considering a hair transplant with Dr. Feriduni after having reading such awesome reviews. However, he told me that I can never get a full head of hair given my low density and I will need to keep getting hair transplants many times in my life to maintain a natural look. I have attached my photos. I need advice on what to do ? what are my best options . Can finasteride or something else help me ?
  13. Much of the FUE vs FUT literature I have read on both sides is barely high school level and shows utter disdain for the scientific method and understanding of controlled variables. For example the Beehner study on FUE cited by Feller and Bloxham regards a surgeon who considers himself experienced and yet also claims to have performed less than 100 FUE surgeries in a decade. To believe anything a talented surgeon writes even if the work is utter garbage and the evidence does not reflect evidence it is nothing more than an appeal to authority fallacy. We can look at the generalities on the other hand like the hundreds of Erdogan 5000-5500 graft cases on the international forums in which norwood 5s are achieving full coverage and the results are arguably more consistent than any FUT doctor in the world. Hair transplant surgery is primarily cosmetic, we can deduce that FUT is clearly the better procedure by the nature of dissection under a microscope vs blind removal of tissue, however the results of doctors like Erdogan and Lorenzo show that large megasessions and massive cosmetic improvements that equal and better 95% of FUT surgerons in aesthetics and yield is possible. You cannot explain away all these cases with studies from American surgeons who are clearly having there business hit by superior and cheaper FUE surgeons who offer a more appealing service to the average patient (whether or not it is better). The studies are clearly inaccurate and high school level in many of their claims. Erdogan and Lorenzo have some of the most readily available results to view online, feel free to disprove with evidence of failure rates, poor yield and cosmetic mediocrity by looking at their results. Citing study data done by FUT surgeons who are incompetent at FUE and against it for business reasons is piss poor evidence against genuine results. Why are these and a number of other FUE surgeons such as Couto, Freitas, Lupanzula and Feriduni acheiving these results that so many highly intelligent and skilled FUT doctors claim is impossible? The explanation that it comes down to can be nothing other than competence. Why are doctors like Erdogan and Lorenzo able to get 6000-7000 on a regular basis from patients with no visible damage to the donor even with a fade cut? Why is there not a single case of an Erdogan 5000 with visible donor depletion when Feller claims this is impossible? Competence, experience and use of a manual punch. In regards to FUE, Feller lacks all of these in contrast to some of the top full time FUE surgeons. Either that, or he is simply lying. The claim that FUE cannot rival FUT for high norwood work and achieving similar cosmetic results for similar graft numbers is blatantly disproven.
  14. Some members might remember the story about the “Interview” with Feller, Feriduni (as a FUE/FUT clinic) and myself, which finally never realized. Nevertheless, I went to a second consult with Feriduni. Please be aware, I made an appointment for a consultation and not (!) an interview. Therefore, I only could ask some questions directly related to my case. I also did not record (of course) the discussion, but made notes before and after which I want to share. I also mentioned that I am in an online discussion about strip vs. FUE. - Dr. Feriduni recommended FUE as the right procedure for me (same did Bisanga and Lupanzula but the latter is performing purely FUE so maybe that doesn’t count). - This time I mentioned directly to Feriduni that I am open for strip if the results woud be significantly better - I also mentioned, that in the past I always had my hair long enough to disguise a good strip scar - Feriduni was confident that he could get results with FUE very close (or even similar) to his FUT and I had enough grafts available (which makes the lifetime graft count not a decisive factor for me) - He mentioned that I am (most likely) a good FUE candidate (hair type) - From what I understand it required many years (starting in 2003?) to come from 100-200 grafts a day to the numbers/results he does nowadays - He also mentioned that he would defend his opinion (almost similar results with FUE vs. FUT ) against anyone and actually there is a lot of discussion in the FUE conferences about this topic - Interesting point was that he told that on both sides (pro strip and pro FUE) there are “extremist” in both their opinion and their language (he mentioned the clinics names, but I will not publish them) - He admitted that there is very poor scientific data (FUE vs. FUT) and we had a brief discussion about the scientific articles and how they are edited (would be worth an extra discussion). - Interesting side note: Feriduni briefly explained different FUE extraction methods depending on the patients grafts (which helps reduce trauma but reduces grafts/session) Background to put the recommendation of the clinics into context: - I am 38 years old - Currently require totally 2500-3000 grafts - From my age and family history lifetime grafts required are ~ 5000 (if meds stop working) - All clinics estimated around 10 000 grafts available via FUE (and above average thickness and number of multis) As I am always suspicious and do not trust any doctor blindly (not Dr. Feller, not Dr. Bloxham nor Dr. Feriduni): - I used the “coverage value” from Lorenzo/Erdogan to calculate the available grafts myself and also ended up with around 10 000 grafts - Before I go for a big FUE session, I will do a minor fix for my W-Shape hairline (<300 grafts), which will hopefully tell if I am a good FUE candidate (how easy the grafts can be extracted) - If I am not a good candidate I will do FUT for the major area (crown) Maybe some will find this summary helpful.

Parallel incisions (single and double units) were made in custom-sized blades technique (~Cutting Edge blades of 0,85 mm). I generally do not advise a crown reconstruction in patients younger than 35 but since this patient only suffered from minor hair loss and since his donor situation was still intact, I made an exception after informing him about the possible risk on future hair loss.   16. All norwood 5s with high sides and back on both sides of the family. In terms of FUT I would put Dr. Feller among the top options with, Bisanga, Hasson + Wong, Konior and Feriduni. My only issue with him is that in the (rare) cases I have seen of a very poor yield there are occasions where his conduct is approaching blatantly unethical, and he is very averse to refunds or touch-ups.
  17. decan, I'm sorry you're not getting any responses, perhaps it's because you're selecting between so many great surgeons that people don't know how to answer that. Honestly, Dr. Feriduni, Dr. Bisanga and Dr. Mwamba are all doing such great work that I would trust them all with my scalp. I would personally look at all of their work on their recommendation profiles (see the drop down box of surgeons in the right hand sidebar) and this forum, consult with each and then decide. Forget about lists, i've been seeing Dr. Feriduni listed as a top doc ever since I signed up to this site, and I haven't seen more than 2 patient results published here in the past 3 years, everyone tends to regurgitates the same lists, what YOU as a potential patient have to do is actually research, look for REAL patients who've posted their results on websites such as this or through other platforms. As far as FUE goes the doctor with the most published results from real patients is probably Dr. Erdogan, while researching you have to understand what holds more weight, clinics only put out their best work obviously, so it is essential to see unbiased reviews of patients, if a clinic doesn't have enough unbiased reviews then it might be best to look elsewhere. Also be very wary of someone posting a review of them already at 12 months, you want to look for doctors who have patient's posting their results since day 1 up to the 12th month mark. Good luck on your research and remember this is YOUR head and it's YOUR responsibility to research.

"1) your position on FUE seems to just be based on your own experience performing the surgery 2) I understand that you have not attended an ISHRS or other conference in person in years 3) you appear to be unaware of who exactly the new and upcoming FUE surgeons actually are. Dr Lupanzula is a well respected FUE surgeon recommended by this very site and who actually presented/gave a demonstration at a FUE conference hosted by Erdogan in 2015 and which was attended by some heavy hitters like Shapiro, Wong, Feriduni, Lorenzo and others. Yet, you yourself admitted you had never heard of Dr Lupanzula until he posted on this thread. 4) I am not even convinced you have seen a live FUE from one of these top FUE surgeons in recent years. I could be wrong on this point. " Not being aware of Erdogan and Lupanzula as a hair transplant doctor in 2017, especially considering their work on the research and FUE conference circuit is outright shameful. Not only is it a terrible look, but it looks very, very hard to believe. It looks like desperation, particular considering Dr. Feller's claims that FUE isn't popular or growing in popular. Clearly an outright lie or pure ignorance of what the available ISHRS data about the growth in FUE procedures indicates. Shouldn't a top doctor be paying attention to facts and statistics? One would hope so. Again I think everyone agrees with the crux of Dr. Feller's argument as to why FUT is the better procedure for most, however there is clearly a lot of dishonesty and inflammatory, unrealistic rhetoric going on. There are FUE surgeons like Erdogan and Lorenzo who are consistently producing better results than 90% of FUT surgeons when looking at similar graft counts and hair textures, regardless of whether FUT is the better procedure.
  20. I had an in-person consultation with Dr. Konior and we talked about FUT vs FUE. I need 1500-2000 grafts and Dr. Konior told me that he would choose FUE for me without a doubt because of the way I like to style my hair (keeping it very short on the sides). He explained that there have been many advances in the FUE procedure over the years and he now thinks it yields excellent results. I didn't go in to the consultation requesting FUE, and even though I prefer FUE due to less visible scarring at shorter hair lengths, I was open to the idea of getting FUT if Dr. Konior thought that was my best option. He didn't, and confidently reassured me that FUE was the right procedure for me. Dr. Feller, I know you have a fantastic and well-deserved reputation for your FUT results, but in my honest opinion, using such strong, emotional language like "FUE ... rips the graft from the scalp", "FUE is internet hype", and "FUT [allows for] the least amount of trauma known to man" doesn't really help your case. When there are top-tier surgeons like Konior, Hasson and Wong, Rahal, Erdogan, Lorenzo, Bisanga, Feriduni and many others recommending FUE and getting home-run results, it’s hard to make such strong claims against FUE without inferring that you also believe these surgeons are running unethical practices. Finally, speaking from a personal point of view, I have no problem with the *idea* of FUT. I’m not concerned at all about having a strip removed from my head. I’ve had larger medical procedures done and to me FUT surgery seems like no big deal. My biggest concern, by far, is that the FUT scar prevents me from wearing my hair short on the sides. I’ve seen some FUE patients with undetectable scarring at a grade 1 which I just don’t believe is possible with strip. I want as few limitations as possible after my hair transplant, and for me I think FUE is the way to go.

FUT yields better FUT done well provides a far smaller total area of scarring FUT does not diffuse the donor FUT takes the best section of the donor FUT does less harm to the grafts FUT maximises the potential of the donor - All that being said, there are some FUE surgeons - the usual suspects (Erdogan, Couto, Lorenzo, Lupanzula, Bisanga, Feriduni) who are getting better cosmetic results than the vast majority of FUT surgeons with the same amount of grafts, but even inarguably rivalling the cream of the crop FUT surgeons like Dr. Feller. And that's all there is to it. FUT is better in the vast majority of regards, but for the best FUE surgeons the vast majority of the gap is closed. Even in regards to yield and consistency, Lorenzo for example has inarguably more results than any FUT surgeon in the world available for viewing online and has the track record to rival any FUT surgeon in cosmetic improvement per graft and consistency of high yields. Some people just don't want a strip cut out the back of their head when they can instead just go to a surgeon like Erdogan or Lorenzo who 49/50 times is going to get results equal to ANY FUT surgeon.
  22. In my opinion, the scar looks a little irritated and red. Otherwise you can not say after three weeks how the next weeks and months will. Look at my scar from Dr. Feriduni. No longer visible. You have to be patient.
