  1. Hello all, I finally bit the bullet. To give everyone a rundown I’m 39 years old this year. I live out of Austin, TX but I’m from closer to the Houston area. I started thinning when I was 18 and I tried everything to slow down the process, admittedly without a lot of knowledge back then. This was 1998 and I didn’t know much about propecia or minoxidil. I tried shampoos and all kinds of gimmicks. I did try propecia and minoxidil liquid but because I didn’t know enough about it and didn’t see immediate results I just stopped and got on with my life. It all started in my crown and then over the course of a decade marched its way forward toward my hairline. While my crown was very noticeably thin my hairline and frontal third stayed noticeably strong until my mid to semi late 20’s. At 24 I went and had my first consultation at a Bosley clinic in Houston and the whole experience made me feel uneasy. The doctor wasn’t really concerned with my crown which is where all the thinning was happening and wanted to lower my hairline which was still a teenage hairline. It was less than four fingers down on my forehead and still really thick. Ah, how I miss that wall of hair... lol. I politely passed but they called me every single day for two years straight trying to get me to come in for the transplant. They wanted to charge me 20K to lower my hairline and sprinkle some grafts in the crown. Knowing what I know now I think back about how they didn’t even bother to mention propecia to help slow loss. Bullet. Dodged. I got married in 2010 which ended really badly very soon after and my hair really started to go after that. Around 2012 I went to Dr Brad Limmer and got on Propecia/biotin/minoxidil foam to slow things down. Sadly he passed away the next year almost to the day of my consultation with him. I’m an actor and I was staying in Houston with a friend in 2014 while filming a movie in Louisiana when someone mentioned Dr. Arocha. I scheduled a consultation and we immediately got on very well. I loved his attitude, how much knowledge he wanted to share, that he was very honest about what we could achieve. I was traveling a lot though and it just wasn’t the right time to pull the trigger. While I traveled around the states for jobs I would schedule consultations with other doctors. I spoke with Feller and Bloxham, Gabel, Hasson and Wong, and others-some through email, FaceTime, in person. Ultimately it came down to me feeling the most comfortable with Arocha although Gabel was pretty close. I’m a diffused thinner and if my father and maternal grandfather are any indication I’m going to end up a 6 on the Norwood’s scale. I was going to go ahead with the procedure last year but my wife (I got married again in 2017) and I found out we were pregnant with twins so I decided to wait until things settled down with them. Side note: I did not stop taking finasteride when we found out we were pregnant and our babies our beautiful and healthy and at the time of this post almost 6 months old. The Big Day! I got to Arocha’s office a little after 7:30 am (traffic in Houston sucks) I signed the paperwork. We took some preop pics. Dr Arocha designed the hairline basically within my existing one. The plan was to work through the existing hairline, fill in the temple corner recession, and bulk up the frontal forelock. We discussed what we would do next year, reassess the front or go ahead and work on the crown. Everybody was really nice. I was visibly nervous and my blood pressure was a little high due to my being a little antsy. The Valium quickly helped out with that. When they were injecting the numbing solution into the back of my scalp I noticed I started shaking and feeling queasy so I spoke up immediately. They turned me over and raised my legs and put ice on my chest. I almost immediately felt better. Dr Arocha was there and cracked a joke about how I was his first patient of the day but probably his 7000th patient altogether so there was really nothing to worry about. TBH I wasn’t nervous about his ability to help me as much as I was just nervous about this giant step I was taking. After this procedure there was no going back. I turned back over when I was ready and they took the strip out. They kept complementing me on how good my donor was however I never got a number from them. I asked but they were just like, “don’t worry, it’s really really good.” I believe I heard say as they were looking at it that it looked like 80 to 90. Dr Arocha started to make the incisions and we talked for awhile before I dozed off and the next thing I know is I’m waking up to salmon kabobs, a killer salad, and potato and black bean soup. I eat up and then I go back to the chair where the techs start to insert the grafts. We chat for quite awhile. Dr Arocha was there for quite a bit of it as he would interject in our conversations. Every once in a while he would remind them about this or that concerning my grafts. I dozed off again and when I woke up we were basically done. They informed me that while I was out they put PRP all over my head. They went over cleaning instructions and we took some post op pics. My wife came in to pick me up and everyone came out to meet our twins. After that we were off and back to Austin. The ride back was pretty rough. There was a lot of pressure in my head and I got a pretty bad headache. Even with the pain pills. It didn’t even last the entire three hour drive though. I’m attaching some pics. Some preop through day two. I don’t have any pics with the design yet. I’m going to speak to Arocha’s office and see if I can get my hands on the hairline design.
  2. This patient had a previous FUT procedure at another clinic prior to coming to Dr. Arocha. The grafts looked pluggy and sparse. To correct this, Dr. Arocha and his team performed a 3000 FUT procedure, working throughout the existing transplanted area. The after images were taken 5 years after his procedure at Arocha Hair Restoration. The patient recently came in for a 2nd procedure with Dr. Arocha to restore his crown. We look forward to updating you with those results in the future.
  3. Thanks hairlossPA! You know, the unshaven procedure just seems to be Dr. Arocha’s thing. I didn’t even think to ask for him to shave because it seems that’s what he always does. I went into it knowing it was his thing so I just went along with it. I’m all about what ever makes it easier for the doctor to do their work. Our heads are their palette so if my head needed to be shaved I would’ve shaved it. I said in my initial post but I’ve been on fin since 2012 so I’m hoping to avoid a lot of shock loss but even if I do suffer from it my job allows me to wear a cap or beanie so it’s not really that big of a deal.
  4. Hey LaserCap! I did not get a laser but only because the transplant and PRP were in my budget. I did want a laser, though, and Dr. Arocha said he would work out some kind of deal with me. We talked a bit about it. I was really hoping to walk out with one but it just wasn’t in the cards for me that day. We’ll see. Maybe one of these days during a check up I’ll be able to walk out with one. Thanks for the reply!
  5. I don’t mind you asking at all. The donor doesn’t really hurt much. Every once in a while I get a slight ping or pang but I wouldn’t say it hurts. Arocha’s office has reached out everyday to check on me which is awesome. Dr. Arocha himself called me while I was driving back to Austin but I wasn't able to answer as I was napping while the wife drove. I knew from other users that this was a thing some people deal with and the office said that it’s pretty prevalent during that first week. I’ve heard a few people say that they deal with it quite a deal longer, however, by no means would I say that it hurts. If anything, and this is not a knock, my scalp is feeling really tight with these sutures. Not to the point of hurting or anything but I don’t think I’m alone in wanting them to be out already.
  6. It’s really hard to get a good pic of the incision. I had me wife help me comb through and we did the best we could but here it is on day 3. When I go to get my stitches removed I’ll see if Dr. Arocha will take a picture or two with my iPhone.

Hey guys, I had posted my results in another thread over in Open Discussion, but I thought I would consolidate and update my poor pictures with some of the before/after photos the clinic was kind enough to share with me. If I miss any information recapping, the original thread will be linked under "Patient Results" I had initially stopped by to consult with Dr. Arocha about surgery. After taking a look at my hair he recommended that I try adding PRP+Acell to the Finasteride and Minoxidil treatments. I had been on Finasteride 1mg daily for around three years by this time, and Minoxidil for several months. The doctor felt that my generalized thinning might benefit from the treatment, so I agreed to give it a shot. We also discussed the possibility of LLLT since all of these things seem to be more effective when used in conjunction with one another rather than standalone treatments, but I wanted to evaluate the PRP first since there were already a number of variables in the mix and budget was a concern. That was back on 11/5/18. I went back to his office last Monday, 4/8/19 around the five month mark, so the after pictures are from then. A couple of things to note: 1. To reiterate, I am also on Finasteride 1mg and Minoxidil twice daily. I added dermastamping a few months ago, which I do once a week with 2 mm needles. 2. Dr. Arocha performed PRP+Acell injections into my scalp around five months ago. In theory I will continue to receive benefits up to the twelve month mark if not a bit longer, but twelve months is what the doctor recommended for maintenance. He did confirm that a booster session could be beneficial for to accelerate "lagging" areas like the corners in the front, but he didn't feel that was in any way necessary unless I was feeling impatient. I decided to see how things progressed without the booster this time. Most of the PRP injections were done in the frontal area with just a little in the crown. 3. I am actually a redhead of sorts. My hair is that kind of dark auburn color with red highlights mainly visible when the light catches them. As a result, my sprouting hairs tend to be very light, fine, and nearly invisible before they thicken and darken up to the deeper brown-red of my more mature hairs. There are actually a bunch of them growing in the hairline and corners that you can't see in these photos, and based on my experience thus far there's a very real chance they may end up becoming terminal. We'll find out. 4. All pictures are taken without any kind of concealer or special product other than the leave-in conditioner I use after showering. The photos were all taken in the same spot, same lighting, with the same camera. If anything, the lighting does me a few favors in the before pictures as you can see from the harsh lighting pictures. Hair is combed back in both pictures, but I think it was just a tad longer in the "before" images. I hadn't actually realized how rough the crown had started to get, so I'm grateful to be able to see my scalp from angles I'm not usually privy to. It's still lagging a bit, but in fairness it was looking rough to begin with and the PRP injections were not concentrated in that area. Beyond obvious regrowth and filling in of sparse areas, the thickness and strength of the existing terminal hair seems to have improved quite a bit as well. It seems to have much more volume and strength to it at the moment, and I think that actually comes across in how limp my hair looks in some of the before pictures. PRP treatments can be somewhat treacherous territory because based on my research there is absolutely no standardization for how it is performed. Technique, equipment, etc. can vary wildly from place to place and that means if you're going to try it you absolutely need to go somewhere reputable that can show you a history of good results. There are a couple of doctors on this site that fall into this category, and based on my experience I would highly recommend speaking with Dr. Arocha if you're looking to give PRP a shot.   8. On the first night I propped some pillows up on the arm of my couch and got myself at a 45° angle. Even though I slept with my neck pillow I still woke up the next morning with a little irritation where my incision was made. I took some of the pain medication that Dr. Arocha gave to me to alleviate it. Swelling had started to kick in but had fallen back behind my ears and down my cheeks due to the angle I had been sleeping in. The techs at Dr. Arocha’s office gave me plenty of tape to replace on my forehead after each cleaning. I keep it on there so that the swelling doesn’t fall below my eyes. I much prefer that as my wife is working during the days and I’m still taking care of the little ones. These pics were taken on day 2 with my iPhone and you can see that there is much more post op ooze.

BACKGROUND I'm in my late 20s and started receding around the age of 18. my scalp is very light compared to my forehead. my natural hairline sat right on the line where the pigmentation changed color. (Pictured) I stalked these forums regularly but never posted until recently. I'm Latino with African/Italian ancestry, my hair grows straight for about .5cm before it curls over. I'm located in NY Moving Forward I consulted online or in person with various doctors that were recommended on these forums the likes of (Arocha, Bloxham, Diep, Vories, and Frank). after many Q&As with each I felt most comfortable with Dr.Arocha in Texas. I flew to TX on Thursday met him and the staff that same day we went over my loss and future strategies as well as design. He recommended a higher hairline than the one I received based on the rule of thirds which I understood but felt it wasn't near my original hairline. we agreed on a lower line (pictured) The Big Day After medications and local numbing was administer a strip was taken from back of the head – 3140 grafts were harvested. Dr. Arocha mentioned he was aiming for 40 grafts cm2 coverage and that my hair was straight at short length. He did the strip removal, stitching and incisions at recipient site. Technicians did the graft cutting and placement. The procedure was done on FRIDAY I was very pleased with the service. everyone from medical staff and administration were very welcoming and ensure I had an excellent operation. it was to such extent that Dr. Arocha came in on Saturday to review the post op and ensure things were okay. he even gave me a tour of the city in his personal car. he even had one staff member come in on Saturday - the same day as her son's birthday just in case some work was needed (thanks Michelle). On Sunday (Father’s Day) he called to check up on me. Again, I’m extremely pleased with the service. Has First week post op I had swelling which is normal which has gone down considerably between days 4-7. I have followed the post op instructions to the T. I have used the shampoo and bacitracin provided by the Doctor as per instructed no harsh touching and trickle water. I feel like the treatment and operation were world class and I’m inclined to use the doctor again Pictures posted include my original hairline, my baldness pattern, hairline design and post op results at day 5/6.   13. This patient came to Dr. Arocha to re-establish a hairline and increase density. As you can see in the before images, he had fairly severe thinning throughout the top of his head. Dr. Arocha and his team performed a 2800 FUT procedure. The after results were taken one year post-procedure.

    Yes, I was talking about the dermarolling. At 2 mm it takes my scalp a few days to look and feel like it is back to normal, so I think at most I could try pushing it to twice a week, though even that might be too much. Even the frequency of PRP seems to vary wildly depending on the doctor you talk to. Some will say four times a year. Some say once a month. Dr. Arocha said refinements in the technique mean it is only necessary every 6-12 months depending on patient response. I've also seen people advocate for 3-4 treatments over the span of a few months to jumpstart with annual maintenance treatments thereafter. Again, that's why I'd encourage anyone considering PRP to do extensive research on a given doctor's results and consult with a few of them to see what they'd recommend for you. I'd also recommend checking in with your doctor every few months to see if their opinion has changed based on progress. I'm actually checking in a little later than I'd planned, but I've been a bit busy.

    I started on Minoxidil three or four months prior to the PRP. I've been on Finasteride 1mg for three years. I started dermastamping with 1.5-2 mm needles once a week around the same time as the PRP. I have been using Nizoral 1% three times a week as well. Dr. Arocha indicated that Minoxidil, PRP, and LLLT had mechanisms that would enhance one another when used in conjunction. I haven't personally tried the LLLT yet but it's in consideration. If you're going to try PRP and add nothing else to the mix, try the dermastamping. There are a number of studies indicating that PRP is beneficial for wound regeneration and recovery in addition to hair growth. Microneedling creates superficial damage on the scalp to essentially prompt regeneration and repair functions in your body that can actually have positive effects on hair follicles (RE: hair growth). So, in theory, the dermastaming is regularly acting as a catalyst prompting the growth factors you received from PRP to do work rather than simply wait around to let your body regulate that on its own. From what I've read, LLLT may have similar, albeit lesser effects on wound regeneration, so a solid laser device MAY add to that effect. Again, I haven't tested that for myself yet.

    Okay, so again, please forgive inconsistencies in the images here. Working with what little I have. The "current" photos were snapped today. The hairline comparison is actually about the same. There is no concealer in my hair in any of these photos. Hair bare bones. This was somewhere around July. Hair is combed back with zero product. Notice the appearance of the hairline if you can. Top down from August. There is some leave in conditioner but otherwise nothing in my hair. Note thinness in the front and the temples. Those hairs were very short and scraggly to boot. The crown is not too visible here due to length and angle which I apologize for. Wouldn't start Minoxidil for a couple of weeks. August Hairline. Note the scraggly looking thinness in the hairline and temple areas. These are the "shaded" looking areas in the top down shot from above. Top down from today. Same room, under similar lighting. Not quite the exact spot. Leave in conditioner here as well. The hair stands up a bit because it's a tad shorter than it was in the August photos, but ironically August was much more carefully combed to be as flattering to the frontal loss as possible. One more top down with a slightly better angle in the harshest light I could manage to find. Note you can see some thinness in the crown. I want to note something interesting here, which is that the majority of my PRP injections were concentrated in the front. I did receive some into the crown, but we agreed that round one needed to emphasize the front of the scalp. Current hairline. You can see that a lot of the gaps in the hairline have filled in, the hairs have thickened and grown out a bit, and the temples are coming in as well. What you can't really see in this picture are all of the small vellus hairs growing just below this. I mention them because a lot of the "new" growth you see in the hairline here looked similar when it started. Some of these may actually grow out as well, and some of these other hairs may have some maturity to come. The best part of the comparison here can be seen with the difference in the temples/hairline since those photos are actually decent comparisons and it's also a place where non-surgical treatments tend to see "lesser" results. Hopefully this is helpful. I have a check-in with Dr. Arocha on April 8th to assess my progress and maybe get his thoughts on what the next move might be. I am inclined to think the crown might benefit from receiving some more targeted injections, but we will see what he thinks. If the clinic shares any of their before/after shots with me I'll see if I can get copies.

    Well, you guys are in luck. Back in early November I received a PRP+Acell treatment from Dr. Arocha during hid visit to Dallas. That puts me around the five month mark. Some important things to note: I had started on Minoxidil about three months prior and was just exiting the crazy shedding phase. Hair was probably at an all time low for multiple reasons. I've been on Fin for three years. Around December or so I started working in dermastamping as well since my research indicated it was very synergistic with the mechanism of PRP along with the Minoxidil. I declined at that time to try LLLT as well. By three months I definitely noticed that I was no longer seeing my scalp in harsh light, even in the weakest areas. I also noticed that the quality of my hair in general seemed better and my shedding was reduced. Lastly, and perhaps most noticeable, is my hairline has crept down just shy of half an inch and has gradually filled in. It's not bulletproof by any means, but I continue to see new growth. Many of these new hairs are not as strong and thick as I would like, but there is still time yet for them to get there considering where they started. I've taken note that a number of the hairs shedding in the shower are miniaturized to the point of invisibility on one end and at the root have thickened considerably and almost look like they were dipped in ink. They're not all like this, but I've definitely seen enough to feel encouraged that some of my existing hair is being salvaged. A couple of additional notes: Dr. Around said his treatments needed a refresh every 12 months, maybe 6 depending on the situation. I need to schedule a check-in to see what his thoughts are on my current progress. Now, for those who are about to jump in and say I can't single out PRP as the cause of my success...you're right. Dr. Arocha was very candid that the PRP was just going to be one component of the treatment and I suspect that is true. I'm waiting until about twelve months post treatment to determine "final" results and see if I notice strengthening of the newer hair within that timeframe. If I didn't grow any additional hair and just saw some thickening in the existing regrowth, I think that could potentially add up to a substantial improvement. For me, that will help determine whether it was "worth it." Questions welcome.

Dr. Arocha's long history of excellence is based on two factors: 1) Refined hairline work that typically completely reframes the face and knocks several years off one's appearance. 2) Consistency with lots of documentation. This second factor, I admit, is lacking in consistency with regards to lighting. We don't have an empty white wall to use, and the aesthetic of our clinic overall is darker in general than a typical clinic. If you've ever stepped foot through our doors, you will know that Arocha Hair Restoration is not your typical clinic. This has to do with the artistic appearance that Dr. Arocha designed in the overall construction of the building. Part of this issue also has to do with timing, in that we don't always have time to pull up the before photos to sync with our after photos and sometimes the exact comparison will suffer. I still like to show these images because I believe, while not exact, they provide a good sense of the before and after of the patient. We will endeavor, as always, to improve upon this in the future, as we do take constructive criticism seriously.

  23. This patient came to Dr. Arocha and team to restore his hairline. He underwent a 2500 FUE procedure, and Dr. Arocha performed a 500 enhancement for a total of 3000 FUE. The after images were taken 4 months after his 500 graft enhancement. Please note: I'm editing this post for clarity. I mistakenly put that the patient underwent a 2500 procedure, but Dr. Arocha let me know that these photos were actually taken after his additional 500 graft enhancement - for a grand total of 3000 FUE.
  24. I just went through all the answers posted on this topic and I think, overall, it represents a bit of what is believed about laser therapy overall. It does work at the cellular level, and it is yet another modality to hair combat hair loss. It is synergistic with other modalities and you will lose the benefit if you stop it. It is recommended to do 3 times per week for 30 minutes each time. The only cap that does have studies is LaserCap. The array of 224 diodes allows for optimal results. There is also a 300 diode unit that drops down in the occipital area. This cap is ideal for patients that are thinning in the donor area, (experiencing global thinning). The laser comb by Hairmax that came out years ago was the first to get FDA approval. Once the approval is given, no one else can get "FDA approved." The can get "released." The problem is this modality is the fact that the doctors selling these products do not know how to market them. Go to a consultation and see what say and and do. Did they scope? No! So, if you have a decent set of hair, for example. You are shedding big time and end up purchasing a unit. Do you think that 6 months later you will see a difference? NO. If they do scope, however, you now have a base line to compare the same area. How many times have I seen dejected patients only to get really excited when they confirm that miniaturized hair has actually improved in caliber. Dr. Alexander and Dr. Arocha do sell LaserCap. Have a consultation with them to learn more about these units. The most important thing in my opinion is to take photos and keep records. This can take 6 -12 months to really see results. And it works great when combined with PRP, at least for what I've been able to observe so far.
