Advice for Hair Loss Sufferers Considering Hair Transplant Surgery


My Story

  1. This patient came in for both a beard and hair transplant. He was experiencing diffuse thinning and wanted Dr. Arocha to add density to the hairline. Background: Date of Procedure: 9-18-2020 Procedure: FUT Grafts: 2,500 Doctor: Dr. Arocha, Houston, Texas Age: 35 Current Preventative Measures: Propecia, Minoxidil, Laser Cap, and Micro Needling First, I want to say that my goal here is not to bash the doctor. He has been receptive to my concerns and presented some possible solutions. My hope here is to tap into the community for guidance. Issue: Unfortunately, the procedure left me with a very wide FUT scar. The scar is largest above the ears, but it is fairly prominent throughout. Prior to surgery, the doctor notified me that scars can be unpredictable, but I was also told that a reasonable expectation is to be able to be able to cut your hair with a guard 3 without the scar showing. Currently, if I cut my hair with anything less than a guard 7, the scar is very visible. Even with longer hair, multiple people have commented on seeing the scar when my hair falls to the side, and this has been a source of frustration and embarrassment. Objective: Repair the scar to the extent possible, while also leaving open the door for future procedures. I am planning on getting another procedure (FUT or FUE to further fill in the frontal hairline, temples, and crown). Combining both the scar correction and this second procedure is ideal. The next procedure will likely be 2,000 to 2,500 grafts. Conversation with the Doctor & Proposed Solutions: The doctor suggested that my wide scar may be due to the high elasticity of my skin. He also suggested that merely sleeping wrong can result in a widened scar. Further, he pointed out that, though I do not have keloids (thankfully) the scar is slightly thickened. The doctor proposed the following solutions: FUE Procedure to fill in the scar area, which would require about 300 grafts. Another FUT procedure in hopes of cutting out the scar and using additional staples to help minimize stretching. Combination of an FUT and FUE. The FUT would be near my neck, which has the least amount of stretching and the FUT would be used to fill in the point most prone to stretching, which is above the ear. Option 1: FUE Procedure My concern here is the loss of grafts for future procedures, as well as the fact that so many grafts will be spent just to conceal the scar (300+). I will likely need a third procedure as my hair continues to thin, and so I would like to maximize my available donor area to the extent possible. Option 2: FUT If the procedure successfully cuts out the scar and can result in a smaller linier scar, then that would be ideal (especially considering that 80 grafts would be enough to conceal a small scar). The doctor suggested a special tempur pedic pillow that may help reduce head movement (and thus the chance of scar stretching). The issue is that I have evidence that my skin stretches, and I am also fearful of a keloid (though I do not have a history of this). In regard to my prior FUT, I was studying for a major exam for a few months after the surgery. My head was tilted downward while reading, which may have contributed to the scar? This is just a thought regarding one factor that won't come into play on my second procedure. Option 3: FUT/FUE Here, the number of grafts in the wide areas around the ears would be around 100, while the remainder of the scar (near the neck area) would just be excised and more closely monitored. I am open to this option, but the optimist in me wants me to go with the second option (a complete FUT with scar revision) in hopes of attempting to excise the scar and remain extra vigilant. What do you think? Your thoughts are appreciated. In your experience, how bad is the scar? Was there something that I or the doctor could have done to avoid this issue? Is there an option that I am missing? Should I consider another doctor for the procedure? Note: I live in Houston, which makes Dr. Arocha especially appealing (particularly in regard to follow-up visits). I have heard fantastic things about Hasson & Wong, but the distance is a concern. This has been a source of stress, and any guidance the community can offer is really appreciated. Thank you, everyone.
  4. That's an unfortunate scar but luckily you have options. Honestly, I think another FUT with scar excision may be your best bet but I wouldn't go back to Dr Arocha. You need a scar specialist and I don't think there's anyone in the world better at FUT scars than Dr.Konior. Of course, there's always risk of the scar stretching again but in the skilled hands of a great surgeon you lower that risk. Your donor does look thick so you will have FUE options down the road but I wouldn't use those grafts at the moment to fill in the scar.

I am not currently in the industry and I am selling nothing. I am here for support and to educate the ignorant and those who need help. A lot of miss-information out there. With regards to my name, I am sorry you feel it is a "little suspect." I use LaserCap and I believe in it. That's why I chose the name.   14. Seems your front even improved with the use of medical therapy when you started. You then started playing with the meds and used less Fin. Seems it's been a gradual process, but now your front is gone. You may want to consider returning to the 1mg, add PRP and laser and consider a transplant. PRP combined with laser therapy has shown reversal of the thinning and help with enhancement of the native hair. That should keep what you have and perhaps even see some visual enhancement. It is unlikely, however, that your front will improve greatly. In the old days, say 20 years ago - and still today - there are some doctors who are in the belief that a separation between grafts is required for survival of a graft. So, you do a procedure and 9-12 mo later you do another and fill in the gaps. Now-a-days, there are others who can achieve great density with a single procedure. Not sure if it's graft placement, but the effect is unreal. Visit Dr. Arocha's website and look at photos of results. 2500-3000 grafts may be possible. (Always do as much as you can afford). Seems you keep your hair rather long. FUT would allow full use of the donor. FUE would always be available in the future if needed. The other thing that benefits you is the fact that you are anglo and have blond hair, (less contrast), which gives you the look of a fuller set of hair. Are you open to travel?
  15. I am now crossing Dr. Nader off the list (well there's only one) and going back to square one. I wanted to tell my wife if it's safe to go to his area and I saw the news that there were multiple dead recently with drug cartel violence. My next candidate might be Dr.Shapiro or Dr.Arocha - I think this will about double what it would cost with Dr.Nader, and yes, I know that cost isn't everything, hence continued research.
  16. A couple of things. There was a finding years ago indicating Min can help but, at about the 7th year, it looses some efficacy. Most, when they see this occurring, they'll stop the med all together. Don't. You are obviously one that is taking advantage of what the med can offer. It is time, however, to add another modality into the mix. Propecia, PRP and laser are commonly what we discuss to help with this debilitating condition. Now about PRP, (and laser). Not sure why the medical community does not get together to discuss the subject. I believe most are just either using the wrong equipment, solution, percentage....but some doctors are far more successful than others. I've encountered practices that tried it with no success and others who are doing it very effectively. Contact Dr. Arocha in Houston. His results doing PRP combined with LaserCap are phenomenal. One last thing.....Nothing regrows hair. NOTHING! The meds are helping to halt or slow the loss. From time to time you'll get enhancement of the native hair. It looks like regrowth but it isn't.

  18. Hello all, I finally bit the bullet. To give everyone a rundown I’m 39 years old this year. I live out of Austin, TX but I’m from closer to the Houston area. I started thinning when I was 18 and I tried everything to slow down the process, admittedly without a lot of knowledge back then. This was 1998 and I didn’t know much about propecia or minoxidil. I tried shampoos and all kinds of gimmicks. I did try propecia and minoxidil liquid but because I didn’t know enough about it and didn’t see immediate results I just stopped and got on with my life. It all started in my crown and then over the course of a decade marched its way forward toward my hairline. While my crown was very noticeably thin my hairline and frontal third stayed noticeably strong until my mid to semi late 20’s. At 24 I went and had my first consultation at a Bosley clinic in Houston and the whole experience made me feel uneasy. The doctor wasn’t really concerned with my crown which is where all the thinning was happening and wanted to lower my hairline which was still a teenage hairline. It was less than four fingers down on my forehead and still really thick. Ah, how I miss that wall of hair... lol. I politely passed but they called me every single day for two years straight trying to get me to come in for the transplant. They wanted to charge me 20K to lower my hairline and sprinkle some grafts in the crown. Knowing what I know now I think back about how they didn’t even bother to mention propecia to help slow loss. Bullet. Dodged. I got married in 2010 which ended really badly very soon after and my hair really started to go after that. Around 2012 I went to Dr Brad Limmer and got on Propecia/biotin/minoxidil foam to slow things down. Sadly he passed away the next year almost to the day of my consultation with him. I’m an actor and I was staying in Houston with a friend in 2014 while filming a movie in Louisiana when someone mentioned Dr. Arocha. I scheduled a consultation and we immediately got on very well. I loved his attitude, how much knowledge he wanted to share, that he was very honest about what we could achieve. I was traveling a lot though and it just wasn’t the right time to pull the trigger. While I traveled around the states for jobs I would schedule consultations with other doctors. I spoke with Feller and Bloxham, Gabel, Hasson and Wong, and others-some through email, FaceTime, in person. Ultimately it came down to me feeling the most comfortable with Arocha although Gabel was pretty close. I’m a diffused thinner and if my father and maternal grandfather are any indication I’m going to end up a 6 on the Norwood’s scale. I was going to go ahead with the procedure last year but my wife (I got married again in 2017) and I found out we were pregnant with twins so I decided to wait until things settled down with them. Side note: I did not stop taking finasteride when we found out we were pregnant and our babies our beautiful and healthy and at the time of this post almost 6 months old. The Big Day! I got to Arocha’s office a little after 7:30 am (traffic in Houston sucks) I signed the paperwork. We took some preop pics. Dr Arocha designed the hairline basically within my existing one. The plan was to work through the existing hairline, fill in the temple corner recession, and bulk up the frontal forelock. We discussed what we would do next year, reassess the front or go ahead and work on the crown. Everybody was really nice. I was visibly nervous and my blood pressure was a little high due to my being a little antsy. The Valium quickly helped out with that. When they were injecting the numbing solution into the back of my scalp I noticed I started shaking and feeling queasy so I spoke up immediately. They turned me over and raised my legs and put ice on my chest. I almost immediately felt better. Dr Arocha was there and cracked a joke about how I was his first patient of the day but probably his 7000th patient altogether so there was really nothing to worry about. TBH I wasn’t nervous about his ability to help me as much as I was just nervous about this giant step I was taking. After this procedure there was no going back. I turned back over when I was ready and they took the strip out. They kept complementing me on how good my donor was however I never got a number from them. I asked but they were just like, “don’t worry, it’s really really good.” I believe I heard say as they were looking at it that it looked like 80 to 90. Dr Arocha started to make the incisions and we talked for awhile before I dozed off and the next thing I know is I’m waking up to salmon kabobs, a killer salad, and potato and black bean soup. I eat up and then I go back to the chair where the techs start to insert the grafts. We chat for quite awhile. Dr Arocha was there for quite a bit of it as he would interject in our conversations. Every once in a while he would remind them about this or that concerning my grafts. I dozed off again and when I woke up we were basically done. They informed me that while I was out they put PRP all over my head. They went over cleaning instructions and we took some post op pics. My wife came in to pick me up and everyone came out to meet our twins. After that we were off and back to Austin. The ride back was pretty rough. There was a lot of pressure in my head and I got a pretty bad headache. Even with the pain pills. It didn’t even last the entire three hour drive though. I’m attaching some pics. Some preop through day two. I don’t have any pics with the design yet. I’m going to speak to Arocha’s office and see if I can get my hands on the hairline design.
  19. When this patient came to see Dr. Arocha, he had undergone a previous transplant with a different hair restoration surgeon, which did not produce desirable results. Dr. Arocha took a conservative approach because the patient's donor was not in great shape. He performed one initial 1,000 graft FUE procedure, followed by an SMP procedure later that year to help give the appearance of more coverage. And, about 1.5 years after his first FUE with Dr. Arocha, we performed a second 1000 graft FUE procedure to add additional coverage. Before images were taken before he had any work done with Dr. Arocha, but after his procedure with a different surgeon. After images show the result after the SMP and both FUE procedures (and about 9-months after his second 1,000 graft FUE). So, he's still got a bit of growing to do, but we are thrilled to see this result come full circle.   20. Hey everybody! It’s been kind of a hot minute since I’ve really been on the forum and I wanted to pop in with a quick update. I’m just over 28 months post-op and I couldn’t be happier. I’ve found a really good length for my hair to cover my crown sufficiently while still complimenting the frame of my face. When I meet new people now I hear a lot of people talking about how nice my hair is. I always play it cool but I’m not going to lie; it makes my heart swell. Lol Dr. Arocha really did right by me and while I know I’ve said it before I really can’t wait to get back in the chair and get some more hair moved around. I haven’t been able to get back in to get anymore work done (obviously) due to the snow/freeze that that Texas had last February. It pretty much wrecked our house and ate up all my hair transplant savings. Maybe next year. I’ve got a few pictures to share. The office I currently work in has a giant window with really great diffused lighting. I would also like to add that @Melvin- Moderator suggested sometime ago that I try adding dermarolling to my regiment, which I was initially hesitant about, but it sat in the back of my mind for who knows how long and I finally started giving it a try. I started doing it on April 16; exactly two years post-op. So while it’s only been 4 months, and I haven’t noticed any results just yet, I am looking forward to seeing why that should bring.   21. I use LaserCap. It is the only one that offers a lifetime guarantee. It is by physician only. Contact Dr. Arocha. I've experienced retention and some enhancement of the native hair. I've also seen positive results when combined with PRP.
  22. You may want to check out Dr. Arocha in Texas. I don’t know of any quality surgeons in New Mexico. Never choose a surgeon because of location.
