Hi Bill,
I use 2-0 Vicryl for subcutenous closure, then I use the 5-0 monocryl suture for the dermal layer which you have referred to as double layered closure, however at the end I add the trichophytic technique by trimming both the upper and the lower edge before wound closure. I use this technique on all of my patients regardless of if it is their first second or last procedure. I use the double layered technique because it reduces the tension and leaves minimal scar line and I use the trichophytic technique to allows for hair to grow through the scar line. I have found that the combination of double layered trichophytic technique gives excellent cosmetic results for my patients.

Dr. Meshkin I have not seen many of your results but I must say that closure seemed amazing!!! I would love to see a one year scar pic, if you could even see it in a year!n
