This man came in for a followup and is very happy. He had frontal triangle reconstruction and lowering of his anterior hairline fairly conservatively in January. We packed true follicular units into 0.72mm slits and did a 2 layer trichphytic closure.

For comparison, I show a neck scar from spinal surgery that exhibits some widening. I did not do that scar, but do show his donor closure and 7 month scar for your review.

Right at the hairline and into it for 1/2 inch, we pack as tightly as we can. Also, we tumesce the scalp with saline prior to making slits. An analogy is if you draw on a balloon which is blown up, and then let the air out, the lines are sharper and more compact, thus slits placed into somewhat dilated scalp are even closer when edema has left the area. Depending on the nature of the donor root shafts, we go from 50 to 70 grafts/square cm. An asian with thin straight hair follicles gets smaller slits more densely packed than an african american with wide follicles, often containing 2 or more roots per follicular unit. Similarly, density requirements are more with thin hairs and less with kinky curly hairs.
