This patient had old style mini/micrograft work done years ago by another clinic. When he came to use had considerable thinning, but mostly on the risht side of his head.

After spending some time with him we felt the best thing to was to transplant only half the head as the other side was already mostly grafts.

We implanted 1600 grafts just into the right side of his head and it all worked out.

This didnt get commented on but that doesnt take away from the fact that its still a very good job. Both sides look around the same density now.

That is a dramatic improvement. I wonder about patients that get fairly low graft count procedures. For 1600 grafts the results are amazing.

It looks like he could have really drove it home with double that amount as he still has some thinning area in the top back. I would think approx 1600 more in this area would have given him an overall full head of hair appearance.

Can you comment why this patient didn't opt to fill in the top back area? Was it a donor/financial issue? I'm sure he is happy, but I wonder why patients like this don't go the extra bit to have more complete coverage.

Money, donor area, and understandable skepticism were the issues that limited the size of his session.

He had already spent alot on his first surgery (more than I charged him which is remarkable) which left him with a thin, unbalanced, and stalky look. His first HT doc used minigrafts everywhere so he was understandably shy about making the same mistake again on a big and costly scale.

I think he really viewed my repair surgery as a "test" case for him.
I understood that very well during his consultation which is why I told him I was going to offer him something I normally would never offer... transplantation of only HALF the frontal scalp.

Because his hair was so fine I knew spreading it over a larger area would be a waste and I'd have a very very unhappy patient, so instead I insisted on packing it into one area on one side.

That did the trick. His hair on the right side has been restored, and so has his confidence in hair transplantation.

Looks great! I'm getting the impression that subtle and perhaps non-standard approaches like this are a particularly effective showcase of surgical skill...

I'm sure he's happy.

Yes, he's very happy.
Only a non-standard approach would have given him any chance. Each patient presents with their own challenges and it's this that adds alot of color and satisfaction to this field.
