This is my current hair line as of 10/8/2008. I would like the bald spot in the back to fill in as well as my hairline to be restored completely.

Restored completely? Then you'll probably need 4,500 grafts or more. Be sure to talk with a doctor about reasonable expectations.
However, I think you could get a great looking result with about 3,000. 2250 in the front 1/3 and 750 in the crown. But, I'm just a stockbroker so I don't really know a damn thing right now......
